fix(server): streamed usage limits survive a failed probe - #31135
When the usage probe fails, Claude limits that a turn streamed in stay invisible. Usage → Limits shows "Claude: Could not read limits." even though the provider snapshot holds real windows. The next failed probe then replaces them with an empty list.
This is permanent for accounts whose org pushes CLAUDE_CODE_DISABLE_NONESSENTIAL_TRAFFIC=1 through Claude remote managed settings (~/.claude/remote-settings.json). The CLI then answers get_usage with rate_limits_available: true, rate_limits: null every time (#27492). The streamed rate_limit_event is the only source of limits, and the user can't override the flag.
Observed provider cache (~/.t3/caches/claudeAgent.json) on the released app, after a few turns:
probeFailed is kept as a marker, as #12170 intended, so a sparse streamed snapshot is still not treated as a clean read. It no longer discards or hides data:
resolveUsageLimitsAfterProbe (provider-core): a failed probe keeps a published snapshot that has windows, even if that snapshot is itself marked probeFailed. Before, only unmarked snapshots survived, so streamed windows were wiped on the next refresh.
limitsNotice (shared): "Could not read limits." only appears when there are no windows to show. Every surface (web Limits page, composer, thread details, mobile) gates on this function.
Duplicate-account merging used limitsNotice as its "usable" test. It now uses a stricter clean-read check, so a clean snapshot still beats a probe-failed one with windows (existing test unchanged).
Independent of #30999, no shared files and no ordering requirement. #30999 makes Claude events with only unifiedWindows produce windows. This PR makes produced windows survive a failed probe. With only this PR, org-managed accounts get the Weekly window (their events still carry flat utilization). With both, Session and Weekly show.
Why this is a fix, and what it changes from #15116
Accounts whose org sets the managed flag can't see Claude limits in T3 at all, although T3 receives them on every turn. This PR fixes that one defect.
To fix it, the PR narrows one rule from #15116 (fixing #12170): a failed probe no longer discards streamed windows. #15116 made the drop deliberate, so a partial list couldn't "survive later failed probes as though it were a complete account read". Its managed-provider test asserted the drop, and this PR updates that assertion (see the test(server) commit).
fix(server): a mid-turn usage update no longer hides a failed probe #15116 assumes a later full probe will recover. For accounts with the org-managed CLAUDE_CODE_DISABLE_NONESSENTIAL_TRAFFIC, it never does. Dropping streamed windows means those users never see any limits, although T3 has real data.
If you prefer a different shape (e.g. a "partial" label next to the windows), I'm happy to adjust. Refs #30310 and #27492; root cause in this comment.
Verification
New assertions in existing cases, which fail on main and pass here:
packages/provider-core/src/server/usageLimits.test.ts: "keeps the last good windows through a failed probe…" covers a streamed probe-failed snapshot with windows (kept) and without windows (replaced).
packages/shared/src/usageLimits.test.ts: limitsNotice returns null for probe-failed with windows, and "Could not read limits." without windows.
packages/provider-core/src/server/managedProvider.test.ts: the fix(server): a mid-turn usage update no longer hides a failed probe #15116 case now expects the sparse snapshot (windows plus the failed-probe marker and message) to survive a failed refresh. The full-probe recovery assertion is unchanged.
vp run check:changed passes. Also ran vp test run packages/provider-core packages/shared/src/usageLimits.test.ts apps/server/src/provider: 1111 of 1112 pass. The one failure (ClaudeSkills.test.ts, plugin registry order) fails identically on main in my environment and is unrelated.
Root cause confirmed by toggling the flag in our org's managed settings (I'm the org admin). With the flag, every probe returns rate_limits: null and the released app's cache stays windows: [], unavailable: probeFailed. Without it, the released app gets a clean probe with Session, Weekly and Weekly · Fable.
Manual: vp run dev --home-dir <isolated> on macOS arm64, Claude Code 2.1.318, SDK 0.3.284, Claude Team seat with the org-managed flag, together with fix(server): Claude limits update from rate_limit_event unifiedWindows #30999. Before any turn, the dev cache held windows: [], unavailable: probeFailed. After one Haiku turn it held five_hour 9%, seven_day 93% with probeFailed, and Usage → Limits showed both windows. A manual refresh (failed probe) kept them visible.
When the usage probe fails but a turn has streamed limit windows, the
windows were hidden behind "Could not read limits." and the next failed
probe replaced them with an empty list. Keep and show them; a clean read
still wins over a probe-failed one when merging duplicate accounts.

Walkthrough
Usage-limit selection now ranks snapshots by read status before freshness. Failed probes can retain published limits that contain windows. Tests cover merging, notices, sparse updates, refreshes, and complete probes.

Account and native snapshot selection now ranks snapshots by read status, then uses freshness to resolve equal ranks. limitsNotice reports a probe failure only when the snapshot has no windows. Tests cover merged limits, notices, and provider ordering.
Provider probe resolution and recovery packages/provider-core/src/server/usageLimits.ts, packages/provider-core/src/server/usageLimits.test.ts, packages/provider-core/src/server/managedProvider.test.ts
A failed probe retains published limits when they are available or contain windows. Tests cover sparse updates retaining unavailable metadata, duplicate updates, refreshes retaining partial limits, and complete probes clearing unavailable metadata.
The usage-limit fallback and snapshot ranking are consistent with the intended behavior, with no remaining merge-blocking risk identified.

Prefer clean reads when merging duplicate accounts.
The filters at lines 341 and 363 skip any snapshot whose limitsNotice is non-null. A probeFailed snapshot with windows now returns null, so it reaches the merge. The merge then picks the winner by checkedAt alone. A newer failed snapshot can replace a clean snapshot for the same account. The clean snapshot's windows are dropped, and the failed snapshot's unavailable marker is carried into the account.
collectProviderUsageLimits already prefers clean reads. Apply the same rule here.
Add a case to the existing duplicate-account tests in packages/shared/src/usageLimits.test.ts for a clean snapshot followed by a newer probe-failed snapshot with windows.

Re the outside-diff finding (usageLimits.ts:301, collectLimitAccounts merge): valid, and introduced by this PR. Fixed in 7b6f791 with the same ranking as collectProviderUsageLimits, so a newer probe-failed snapshot no longer displaces a clean one. Covered by an added assertion in "merges one account reported natively on two environments…".
