Terminalize foreground tools on idle failure.

If an ACP v2 agent reports an unfinished stop while a foreground tool is still running, this path finalizes the turn as failed without terminalizing that tool. The tool remains running after the turn ends. The prompt-response failure path already calls terminalizeOpenForegroundTools; apply the same handling here.

Review comment at @packages/provider-acp/src/server/adapter.ts at line 4588:
In the ACP v2 unfinished-stop failure path around stopReasonFailure, call
terminalizeOpenForegroundTools before finalizing the turn, matching the
prompt-response failure path so any running foreground tools are terminalized.
