fix(acp): refused and limit-stopped turns no longer show as completed - #28797
ACP agents end a prompt with a stop reason. Besides end_turn and cancelled, the protocol defines refusal, max_tokens and max_turn_requests, and its schema says a refusal "should be reflected in the UI" because the agent drops that prompt from its next turn. The ACP adapter only special-cases error and cancelled, so all three of these end as a completed turn. A refused prompt or a reply cut off at a limit looks the same as a finished answer, and nothing tells the user the agent will not remember it.
Change
refusal, max_tokens and max_turn_requests now fail the turn with a provider failure, the same way an idle error already does. The failure message says what happened (a refusal also says the agent's next turn won't include that message or its reply), and its code is the stop reason. The mapping is shared by every place a stop reason becomes a status:
the prompt response (ACP v1 agents, Grok, and v2 agents whose runtime reports idle there),
the idle state_update for agents that follow agent work state (Claude Agent on ACP v2),
a native child session's idle state_update, and Claude's subagent_update child state, so a refused or limit-stopped subagent shows as failed with that message instead of its partial reply. This matches what an idle error already does for a child.
end_turn, cancelled, interruption and unknown stop reasons keep their current handling. No contract, status or client change: the existing failed terminal and its error row carry the message.
Scope and approval
This is a small, focused fix for an obvious bug under CONTRIBUTING.md § prior approval: the adapter records stop reasons the ACP schema defines as unsuccessful as success. It follows #24615, which made an idle error fail the turn, and reuses that path. No issue or discussion covers it.
One choice for maintainers to confirm: these three reasons use the existing failed status and provider error row, not a new status or notice type. Claude's adapter already reports its own token-budget stop (budget_exhausted) as a failed turn with a message, so this matches.
Open #26959 adds instruction-delivery logic just above the prompt-response mapping this PR edits. The two may conflict textually but change different behavior.
Verification
vp test run apps/server/src/orchestration-v2/Adapters/AcpAdapterV2.test.ts -t "fails a prompt that stops": 3 new cases, one per stop reason, on the prompt-response path. On the base commit all 3 fail with expected 'completed' to equal 'failed'. With the fix all 3 pass.
vp test run apps/server/src/orchestration-v2/Adapters/AcpRegistryAdapterV2.test.ts -t "fails Claude's turn and child": 4 new cases for Claude Agent on ACP v2, where a child stops through either its own state_update or the parent's subagent_update, then the root goes idle with the same reason. On the base commit all 4 fail the same way. With only the adapter change, the 2 subagent_update cases still fail. With the full fix the root turn fails with the reason as its code and the child subagent is failed with a message.
The full AcpAdapterV2, AcpRegistryAdapterV2, GrokAdapterV2, AntigravityAdapterV2 and FxAdapterV2 test files plus the provider-acp-registry tests: 204 tests pass.

…eted
ACP agents report refusal, max_tokens and max_turn_requests as stop
reasons, but the adapter recorded all three as a completed turn. The
prompt-response, idle state_update and native child mappings now fail
the turn with a provider failure naming the reason, so the user sees the
refusal or limit instead of an apparently successful turn.

Claude Agent also reports a child's stop through its subagent_update
extension, which still mapped refusal, max_tokens and max_turn_requests
to completed. Reuse the shared stop message there, type the helper with
ACP's StopReason, and assert stop codes rather than copy text in tests.

Macroscope's review found this PR approvable — Refused and limit-stopped ACP turns and subagents now report failure with an explanation instead of appearing completed. The change reuses existing failure handling without changing provider limits or product defaults.

Walkthrough
The ACP adapters now treat refusal, max_tokens, and max_turn_requests as failure stop reasons for prompt turns and idle child sessions. Tests cover prompt failures and Claude child-session updates.
The adapter maps the three unfinished stop reasons to failure messages and applies failures during idle-work and prompt settlement. Prompt tests check that each reason sets the failure code.
Idle child sessions and Claude subagent state events fail for unfinished stop reasons. Child results use the provider error message when present, then the mapped stop message. Tests cover both child-update formats.
A refused turn can leave a tool showing as running and cause the next prompt to omit guidance. Correct both paths before merging.

Preserve instruction state when runtime.prompt returns refusal.
Both cache updates run before stopReasonFailure classifies the response. When the refused prompt included instructions, the first update caches its state and the second removes its compaction marker. The next prompt then passes the same state to t3AcpPromptWithInstructions, which returns only the user prompt for equal states. The next turn can therefore omit instructions that the refused turn did not retain.
Gate both prompt-response cache updates on result.stopReason !== "refusal". This keeps the previous state and compaction marker for a refusal. The next prompt then includes the instructions again. This correction applies to the runtime.prompt path; the inspected idle-settlement path does not write these caches.

Terminalize foreground tools on idle failure.
If an ACP v2 agent reports an unfinished stop while a foreground tool is still running, this path finalizes the turn as failed without terminalizing that tool. The tool remains running after the turn ends. The prompt-response failure path already calls terminalizeOpenForegroundTools; apply the same handling here.
