Problem

When a run stops on a usage limit, the auto-settle sweep can settle the thread in the same second. The run that kept the thread active has ended, and settle-on-merge passes whenever the PR merged after the last user-authored message. That is common when PR-watch or system wakes keep a thread working without a new user message. Settled threads are deliberately excluded from limit recovery, so "Resume at reset" and "Snooze until reset" then always fail, and the thread silently drops out of the active list right when it needs attention.

Seen in a real thread: run 4 failed with usage_limit at 20:46:13Z, and ThreadSettlementServiceV2.sweep dispatched thread.auto-settle at 20:46:13Z. Four recovery attempts at 20:49 to 20:51Z were all rejected. The user never sent a thread.settle.

Change

A usage limit interrupts work rather than finishing it, so the sweep now leaves such a thread alone, the same way it already does for pending runtime requests and link requests.

  • getSettlementCandidates (ProjectionStore.ts) skips a thread whose latest executed run failed and whose latest root error is usage_limit. It uses the same run selection as the shell's blocked join and getLimitRecoveryCandidates, so a message queued behind the limit does not hide it. Like threadErrorSummary, a newer session error that differs from the failure message replaces the limit, so the thread is then treated like any other failed thread. A cheap EXISTS on usage-limit error items runs first, so other threads skip the per-thread run sort; on a real 834-thread database the clause costs about 3 ms of CPU.
  • isAutoSettlementCandidate applies the same rule to shell threads (status: "failed", lastErrorClass: "usage_limit"), so the candidate and the shell keep agreeing. ProjectionSettlement.test.ts asserts that parity.

Only an interrupted thread is affected. A thread that failed for any other reason still settles as before, and once the user sends a message or recovery resumes the thread, the latest run is no longer a usage-limit failure, so it becomes eligible again.

Scope and approval

Fixes #28362. A small fix for an obvious bug: the settle sweep defeats the existing usage-limit recovery feature. It adds no setting or new behavior beyond keeping these threads unsettled. The banner and error-message side of the same report is a separate PR.

Verification

  • ThreadSettlementService.test.ts › never settles a thread stopped by a usage limit: fails on main (expected true to be false) and passes here.
  • ProjectionSettlement.test.ts › sql: discovers settlement work with the same activity and background semantics as the shell now seeds a usage-limited thread, a thread with a message queued behind its limit, and a usage limit that ended after a newer failed run, all excluded, plus a provider_error thread and a limit superseded by a differing session error, both eligible. It fails on main, and the queued, out-of-order and session-error cases each fail on the revision before their fix. It passes here, with the shell and the SQL candidates agreeing.
  • ThreadSettlementService, ProjectionSettlement, SubagentProjection, PeerLinkRequests and runtimeLayer tests, plus ProjectionStore.test.ts: 212/212 pass.
  • vp run check:changed upstream/main: format, lint, knip, graphql, typecheck and tests pass.
  • Not checked: a live provider hitting a real limit after the fix. The trace above is from before it.

A run that stops on a usage limit ends the activity that kept the thread
active, so the settle sweep could settle it in the same second (for
example when its pull request had merged). Settled threads refuse limit
recovery, so Resume at reset and Snooze until reset then always failed.

Settlement candidates now skip a thread whose latest run failed with a
usage_limit root error, and isAutoSettlementCandidate applies the same
rule to shell threads.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — Usage-limited threads are excluded from automatic settlement to preserve recovery controls, changing their default lifecycle behavior. Human review must confirm that policy; the unresolved finding also reports a newer-run scenario that can still hide those controls.

Needs human approval because:

…d threads

A message queued behind a usage limit becomes the newest run, so checking
only the newest run let the sweep settle a thread the shell still shows as
limited. Use the latest executed run, as the shell and the recovery worker
do, and its latest root error.

Walkthrough

The projection query excludes qualifying usage-limit failures from settlement candidates. The auto-settlement predicate also rejects failed threads whose lastErrorClass is usage_limit. Tests cover provider errors, provider-session errors, queued runs, and run completion order.

Full details: Title check

Explanation

The title "fix(server): a usage-limited thread no longer auto-settles" states the main change. The change keeps threads stopped by a usage limit from being auto-settled by the settle-on-merge sweep. It uses a conventional commit prefix and plain language, and the scope (server) matches the changed files under apps/server.

Full details: Description check

Explanation

The description follows the template. It has Problem, Change, Scope and approval, and Verification sections. The Problem section gives the expected behavior, the failure mode, and a real trace. The Change section names the affected functions and explains the run selection and session-error rule. The Scope section links the issue (#28362) and says the fix is small and focused, with the banner work split out. The Verification section lists specific tests with fail-on-main and pass-here results, a full-suite count, and a note on what was not checked. The template asks for the model and harness used if an agent did the work, and the description does not state this. That item is minor, so the description is mostly complete.

Full details: Linked Issues check

Explanation

Issue [#28362] requires the auto-settlement defect to be fixed for usage-limited threads. ProjectionStore.ts excludes threads whose latest executed failed run has root error usage_limit, including queued and out-of-order runs. ThreadSettlementService.ts applies the matching lastErrorClass: "usage_limit" rule. Tests cover recovery eligibility and preserve settlement for other failures. The banner and error-message defects are separate scope identified in the issue.

Full details: Out of Scope Changes check

Explanation

The changes remain within the settlement objective in [#28362]. The SQL candidate query, shell candidate check, and focused tests keep usage-limit behavior consistent. Tests for provider failures and superseding session errors support the required unchanged behavior. No unrelated product change is shown.

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.

Check the latest root error, not any root error.

If one failed run has an earlier usage_limit root error and a later provider_error root error, this EXISTS excludes the thread. The shell selects the later error and treats the thread as provider-failed, so the two settlement paths disagree. Apply the shell’s error ordering before comparing the failure class. …ttle

The shell drops the usage-limit class when the provider session reports a
different, newer error, so it shows no recovery controls. The settlement
query now applies the same rule instead of holding such a thread.

Add coverage for a matching provider-session error.

createFailure sets the usage-limit failure message to "usage_limit". The test covers an absent session error and a different error ("Session crashed."), but not a session error equal to "usage_limit". If the SQL condition regresses to absent-only, that regression passes the current assertions. Add a matching-error fixture and keep it out of the expected eligible set.

Nitpick Cover the reverse completion order in the SQL fixture.

298-318: Trivial · Functional Correctness · Quick win

Cover the reverse completion order in the SQL fixture.

The fixture covers a lower-ordinal usage_limit run that completes after a higher-ordinal provider_error run. It does not cover a lower-ordinal provider_error run that completes after a higher-ordinal usage_limit run. Add this case and assert that the thread remains eligible. This distinguishes completion-time selection from ordinal selection.
