fix(server): PR watches wake when checks finish after an early failure - #28020
When a PR has no required checks, the watch only sends checks-passed once every check passes. If one check fails early, for example a cancelled SonarCloud run, the agent wakes once with "checks failed" while the rest are still running. Nothing wakes it again when they finish. The agent ends its turn waiting for CI and stays idle until a person nudges it. Fixes#28019.
Change
evaluatePullRequestWatch now records the gate checks that are still pending when a failure is reported, in the existing owedChecks field. Once all of them have finished, and none failed (a new failure already sends its own checks-failed wake), it sends one checks-finished wake that lists the gate checks still failing. When only required checks were owed, the wake says "the required checks still running when one failed have finished", because advisory checks may still be running. Only a failing gate check owes this wake. An advisory failure next to a required check's rerun does not. The notification outcome is failed, matching checks-failed. The contract change is a doc comment only. owedChecks already exists and is persisted.
Scope and approval
This is a small, focused fix for an obvious bug, which CONTRIBUTING allows without prior approval. It touches one pure function in the watch evaluation. #23904 fixed the same kind of missing wake for checks still pending after the required ones pass; this covers the case where the gate fails first. The bug is filed as #28019.
Verification
Real incident replayed as a test. The new test follows the check-run timeline from the PR that hit this: sonar cancelled while unit and e2e are pending, then unit passes, then e2e passes. Expected wakes are checks-failed, nothing, checks-finished listing sonar, and nothing on a repeat read. It also covers a later failure among the owed checks, which sends checks-failed. If another owed check is still running, the final wake lists both failures.
Required gates. A second test fails one required check while another required check and some advisory checks are pending or failing. Only the required check is owed. The wake lists only the failing required check and ignores the pending advisory one. pullRequestWatchMessage gives outcome failed and text that names the required checks as finished while advisory e2e is still pending. Rerunning the failed required check while an advisory check is failing sends no wake. A new advisory failure keeps the owed set as it was instead of owing a required check that started later (from CodeRabbit's review). An owed check also stays owed while it runs when the failed check turns required and the gate shrinks to the required checks (CodeRabbit). Another gate failure adds the gate's running checks to the owed set instead of replacing it, and the last owed check finishing still wakes the agent when an unrelated check fails in the same read (CodeRabbit). That case fails if the condition goes back to "any check failed".
The test fails on main. With the source change stashed, vp test run src/orchestration-v2/pullRequestWatch.test.ts reports × wakes once when the other checks finish after one failed where none is required (1 failed, 20 passed). With the change, 22/22 pass.
Checks.vp test run src/orchestration-v2/ passed 1983/1983 across 113 files (one earlier run on this loaded host reported 1978/1983 and passed in full on rerun). tsc --noEmit on apps/server is clean. The pre-commit hook passed vp fmt, vp lint and vp run knip:check.
Unverified
A live wake on this head. Maria's run at 3fc9251 (comment below) watched a real thread receive checks-finished and resume. Since then the only behavior change is the "required checks" wording.
Model evals. The checks-finished text is new model-facing text. evals/mcp-tool-use has no case for it, and this machine has no eval API keys, so I didn't run one.
Decisions
Follow-ups from the November 3 review stay out of this PR. A failure that arrives in the same read as everything else finishing already wakes the agent, the same as on main, and adding "nothing is still running" to that wake is a separate text change. Advisory checks stay unowed when a required check fails, because the gate can't pass until the required check is fixed, and an advisory failure still sends checks-failed. Runner-up: owe advisory checks too, which brings back the advisory churn fix(server): wake PR watches when remaining CI checks finish #23904 avoids.
Reuse owedChecks instead of adding a field. Its meaning, checks still pending that are owed one wake, already fits. A new field would need a contract and Orchestrator change for the same behavior.
Watches saved before this change that are already failing miss this one wake. gpt-6.1-sol (Codex review, P2) pointed out that such a watch has failedChecks but no owedChecks, so it never records what was still pending. The alternative is to owe pending checks whenever the gate is failing, but then every advisory check that starts later re-arms the wake, which is the churn fix(server): wake PR watches when remaining CI checks finish #23904 avoids. The gap closes on the next push or failure, the same transition the no-checks clock accepts for watches saved before it existed.

The PR watch now tracks gate checks that remain pending after a failure. When those checks finish, it can emit a checks-finished notification that lists the failing checks and reports a failed outcome.

Summary
Track pending checks after failures apps/server/src/orchestration-v2/pullRequestWatch.ts
The watcher retains previously owed checks that are still pending, adds pending gate checks after a new gate failure, and emits checks-finished when previously owed checks finish.
Format and validate finished-check updates apps/server/src/orchestration-v2/pullRequestWatch.ts, apps/server/src/orchestration-v2/pullRequestWatch.test.ts
The watcher formats the update, labels it “checks finished,” and classifies it as failed. Tests cover required and non-required failures, reruns, and changes to which checks are required.
pingdotgg/t3code#25497: The PR covers the reported missing wake after remaining checks finish following a required-check failure, but not the broader request to wake after every check settles.
pingdotgg/t3code#30530: The PR adds a wake when pending gate checks finish after an early failure.

Explanation
Issue #28019 is closed and completed. Under the assessment rules, it supplies background only. No active directly linked targets remain, so no linked-issue coding requirements apply. For context, the head change still implements the behavior that #28019 describes. In pullRequestWatch.ts, when a gate check has failed, pending gate checks are recorded in owedChecks. When they finish with none newly failing, evaluatePullRequestWatch emits one checks-finished change that lists the gate checks still failing. The message text and the failed outcome follow from this change. The changes since the previous review (checks-finished wording, required-check owed logic, and the owed-set handling for later failures) do not change the no-requirements result.
Full details: Out of Scope Changes check
Explanation
Compared with the merge base, the whole-PR diff of apps/server/src/orchestration-v2/pullRequestWatch.ts adds the checks-finished variant to PullRequestWatchChange, the owed-check tracking in evaluatePullRequestWatch, the matching message lines in changeLines, a SUMMARY entry, and the failed classification for checks-finished. The test file adds focused tests for these paths. All of these changes serve the watch wake behavior that the PR describes. The PR description says the contract doc-comment edit was reverted, and the diff I read does not include packages/contracts. No unrelated change is demonstrated. The shipped follow-up decisions (existing watches without owedChecks, advisory checks not owed when a required check fails) are documented scope choices and do not add unrelated code.

f6fcf7b (gpt-6.1-sol, sol-loop round 1): only a failing gate check owes the checks-finished wake, so an advisory failure next to a required check's rerun no longer sends an empty "Still failing:" list. It also adds tests for required gates, a second failure while another owed check is still running, and the wake message.
006682a (CodeRabbit): a new advisory failure keeps the owed set instead of re-arming it.
Open P1/P2s: none. gpt-6.1-sol approved round 2, and CodeRabbit's last review had no actionable comments. All 28 checks pass.
Scope: this changes only the watch evaluation and a contract doc comment. Watches already failing when this deploys miss the extra wake, by design (see Decisions in the PR body).
Checked and fine: no reactor-level test is needed, since there are no new persistence, restore, or equality paths (gpt-6.1-sol). Required checks passing early with advisory checks owed (fix(server): wake PR watches when remaining CI checks finish #23904) is unchanged, and its tests pass. A head move clears owed checks through the existing reset.
Keep pending owed checks after another gate failure.
Suppose sonar fails while e2e runs, then sonar becomes required. This change preserves e2e. If a new required check then fails while e2e still runs, this branch drops e2e. Its completion produces no wake.
Preserve pending owedBefore entries when adding pending gate checks for the new failure.

Pushed fixes, after b3d09a1 (the fix itself):
81feec6 (gpt-6.1-sol, sol-loop): only a failing gate check owes checks-finished. Adds tests for required gates, partial failures, and the wake message.
215d61c (CodeRabbit): a new advisory failure keeps the owed set.
2b5bbaf (CodeRabbit): owed checks stay owed while they run, even when the gate changes.
3fc9251 (CodeRabbit): another gate failure adds to the owed set instead of replacing it, and the last owed check finishing still wakes when an unrelated check fails in the same read.
Open P1/P2s: none. gpt-6.1-sol approved, and CodeRabbit confirmed or withdrew every finding. All checks pass and no review threads are unresolved.
Declined: keeping owed checks through a pending rerun of the failed check. Every rerun outcome already wakes the agent, and CodeRabbit withdrew the finding.
CI note: one earlier "Build and Typecheck" failure came from main (AgentSessionScanner.tshomePath, fixed upstream). The rebase picked up the fix.
Checked and fine: no reactor-level test is needed (no new persistence, restore, or equality path). The fix(server): wake PR watches when remaining CI checks finish #23904 path (required checks passed with advisory checks owed) is unchanged. A head move clears owed checks through the existing reset.
merged current main (merge commit, no rebase). no conflicts.
merge-review change: reverted the owedChecks doc-comment edit in packages/contracts/src/threadPullRequest.ts, so the PR no longer touches packages/contracts. the gate-failure branch in pullRequestWatch.ts already has a comment explaining the owed wake, so no extra note was added.
remote checks: pullRequestWatch.test.ts (23 passed), fmt, lint, knip, and apps/server typecheck all clean.
all review-bot threads were already addressed or withdrawn; nothing new to fix.
🤖 Claude Opus 5.5 responding on behalf of iamthebestcodeer
This fixes both variants in #25497, including the no-required-checks case I reported there. I ran probe tests against evaluatePullRequestWatch at 35b741a (vp test run: probes 5/5 and pullRequestWatch.test.ts 22/22 pass). Each probe asserts the current behavior, so a passing probe confirms the gap. I'd treat these as follow-ups, not blockers:
The failure arrives last and gets no finished wake. Sweep 1: a and b pending, so no wake. Sweep 2: a failed and b passed, which sends ["checks-failed"] only. Sweep 3 with the same state sends nothing. The wake text doesn't say whether anything is still running, so the agent can't tell that every check has finished. One option: when nothing is pending, add a line like "No checks are still running." to the checks-failed wake.
Watches saved before this change stay silent. A watch saved with failedChecks: ["vercel"] and no owedChecks sends nothing when verify goes from pending to success. A fresh watch in the same state does send checks-finished. The PR description notes this trade-off. I'm listing it because it's the state any in-flight watch will be in when this ships.
Advisory checks are never owed when required checks exist. Case one: required lint failed, required unit passed, advisory e2e pending. owedChecks is [], so e2e passing later sends nothing. Case two: required unit still pending. checks-finished fires when unit passes, while e2e is still running, and e2e finishing sends nothing. That's the opposite of the green path on main, where advisory checks still pending after the required ones pass are owed a wake.
With no required checks, the watch only sends checks-passed once every
check passes. A check that fails early, such as a cancelled advisory
analysis, woke the agent while the rest were still running, and nothing
woke it again when they finished. The gate checks still running at a
failure are now owed one checks-finished wake.

Replying to @iamthebestcodeer's three follow-ups. I rebased onto main at 4b87a45770 and pushed one fix. The other two stay out of this PR. The new head is f2544f8dd6.
[P3] Finding: With required checks, the checks-finished wake said "the checks still running when one failed have finished" while an advisory check was still running (your 3b). Cause: With required checks the owed set holds only the gate's (required) running checks, but the wake text claimed every check had finished. Impact: Required lint failed and required unit and advisory e2e were pending. When unit passed, the agent was told the running checks had finished while e2e was still pending. Model: opus-5.5 (iamthebestcodeer's probe) · confirmed by the existing test, which asserted that text with e2e pending. gpt-6.1-sol and gpt-6-astra confirmed the fix independently. Action: pushed a904b1f. When every owed check is required, the wake now says "The required checks still running when one failed have finished".
[P3] Finding: A failure that arrives in the same read as every other check finishing sends only checks-failed, with nothing saying whether checks are still running (your 1). Cause:checks-finished needs an owed set from an earlier read, and this read has none. Impact: The agent wakes on this read with the failure. It isn't told that nothing is still running. main behaves the same. Model: opus-5.5 (iamthebestcodeer) · confirmed by gpt-6.1-sol and gpt-6-astra, who traced pullRequestWatch.ts at base and head. Action: comment-only (pre-existing, and the new wording is model-facing text that needs its own eval case).
[P3] Finding: A watch saved before this change with failedChecks set and no owed checks never sends checks-finished (your 2), and advisory checks are never owed when a required check fails (your 3a). Cause: Only a newly failed gate check records owed checks. When the gate is required checks only, advisory checks are outside the gate. Impact: A watch that was already failing when this deploys misses the extra wake until the next push or failure. An advisory check finishing after a required failure sends no wake if it passes. If it fails, checks-failed still wakes the agent. Model: opus-5.5 (iamthebestcodeer) · confirmed by gpt-6.1-sol (probe tests) and gpt-6-astra (source trace). Both judged it by design for this fix. Action: comment-only (design decision: an empty owedChecks array can't tell an old watch from a new one, and owing advisory checks would bring back the churn #23904 avoids. See Decisions in the PR body).
Review summary at f2544f8dd6 (rebased onto main at 4b87a45770, this replaces the earlier summaries)
Pushed fixes:a904b1f7e8 makes the checks-finished wake say "required checks" when only required checks were owed. f2544f8dd6 drops a copy assertion on the notification summary (gpt-6-astra, P3).
Open P1/P2s: none. gpt-6.1-sol: PASS at a904b1f7e8. gpt-6-astra: PASS at f2544f8dd6.
Rejected: removing the scenario-label comments in the multi-step tests (gpt-6-astra, P3). The base file labels its scenario steps the same way, and astra accepted that on re-check.
Scope: the three November 3 follow-ups are answered above. One was fixed, and two are pre-existing or by design.
Unverified: no model eval ran for the new wake text (there is no case for it, and no eval keys on this machine). Maria's live wake run was at 3fc9251, before the wording change.
Checked and fine:fix(server): wake PR watches when remaining CI checks finish #23904's advisory-owed path, the head-move reset, empty same-head reads, the optional owedChecks schema, and the persistence and delivery paths (both reviewers). check:changed and 159 PR-watch tests passed locally.
Please review current head 9a48b3481d2e382b0b6245328cc38b7694b63be5.
I verified that it contains the council-reviewed f2544f8dd64a679442ab35d5ac7edad06f162aed. The watch implementation and its tests are byte-identical to that head. Maria's update merges current main and restores the contract comment, leaving packages/contracts out of the PR diff.

A new field would need a contract and Orchestrator change for the same behavior.Watches saved before this change that are already failing miss this one wake. gpt-6.1-sol (Codex review, P2) pointed out that such a watch has
failedChecksbut noowedChecks, so it never records what was still pending. The alternative is to owe pending checks whenever the gate is failing, but then every advisory check that starts later re-arms the wake, which is the churn fix(server): wake PR watches when remaining CI checks finish #23904 avoids. The gap closes on the next push or failure, the same transition the no-checks clock accepts for watches saved before it existed.Reviewed with sol-loop: 2 rounds with gpt-6.1-sol.
