fix(server): PR watches wake again once checks finish after an early failure - #30531
When one check failed while others were still running, the PR watch woke the agent once for the failure and never again. The agent ended its turn to wait for the rest, as the wake message tells it to, and the thread sat in "Waiting" indefinitely. In one project, five threads waited 19 to 23 hours after CI had finished. Fixes#30530, the same bug as #25497.
Change
A failed gate never reports checks-passed, so it now owes one more wake. The watch records the checks still pending (in the existing owedChecks field). When the last of them finishes, it sends one checks-failed update listing the checks that are still failing. It tracks them only while a failure report is fresh or that wake is still owed, so a later check that starts and passes with no new failure does not wake the agent again. No new wake text or schema field, so the model-facing message is unchanged.
It applies only when the gate has not passed. When required checks have passed, owedChecks keeps its current meaning and behaviour.
Scope and approval
A very small, focused fix for an obvious bug: the wake message promises "T3 Code wakes you on the next change," and after an early failure no change is ever reported. Two files: the fix in evaluatePullRequestWatch and extra assertions in the existing test case for failures. Bug report: #30530. #28020 and #25539 fix the same bug with larger changes (3 and 9 files); this is the smallest of the three, if that helps.
Verification
Extended the existing case "reports each failure at once…" in pullRequestWatch.test.ts. On main it fails with expected [] to deeply equal [ { kind: 'checks-failed', … } ]; with the fix it passes, and a repeat sweep does not wake again. A second assertion covers the Codex and CodeRabbit finding on the first version: a check that appears later and passes no longer sends the failure again. It failed before the follow-up commit.
vp test run apps/server/src/orchestration-v2/: 118 files, 2179 tests pass. That includes the owed-checks cases for required checks, which an earlier draft broke.
vp run check:changed: format, lint, knip, graphql, typecheck and tests all pass.

Macroscope's review found this PR approvable — Existing PR watches receive another update when remaining checks finish after an early failure. The change reuses existing wake delivery and avoids repeating the update on unchanged subsequent polls.

The PR watch now retains pending checks after a failure and emits a checks-failed update when those checks finish. The test verifies that the update is not repeated on a later evaluation or when a new pending check appears.

Summary
Track pending checks and report completion apps/server/src/orchestration-v2/pullRequestWatch.ts, apps/server/src/orchestration-v2/pullRequestWatch.test.ts
evaluatePullRequestWatch retains pending checks when failures exist. If those checks later finish without a newly detected failure, it emits a checks-failed update containing the reported failures. The test verifies the update and confirms it is not repeated on subsequent evaluations.
The fix wakes the agent again for new PR watches after the last check finishes. A PR watch that was already waiting before the upgrade may still not wake once; this affects only in-flight watches and is bounded.

Explanation
The title describes a fix to PR watches waking again after checks finish following an early failure, which matches the changeset (evaluatePullRequestWatch now records owed checks and emits a checks-failed update when pending checks finish). It is specific and follows conventional commit style, so it clearly summarizes the main change.
Full details: Description check
Explanation
The description follows the template: it has Problem, Change, Scope and approval, and Verification sections. The problem, the fix, and the verification steps are concrete, and it links the bug issue (#30530). The Scope and approval section cites the bug report and explains why the change is a small fix of an obvious bug. The description notes that the agent model was used, as the template requests. The template asks for approval comment evidence (maintainer approval or an explanation of why the fix qualifies without prior issue), and the author gives a qualification explanation but links only the issue, not a maintainer approval comment. The verification section lists concrete test commands and observed results, and it states what was not checked.

Explanation
Issue #30530 requires one more wake after an early failed check when the remaining checks finish. evaluatePullRequestWatch now records pending checks in owedChecks after a new failure. When no checks remain pending, it emits one checks-failed change with the failed checks and clears owedChecks. A later check that starts and passes does not create another wake. The existing required-check path remains separate. The updated test covers the final failure wake, the repeat sweep, and the later pending check case.
Full details: Out of Scope Changes check
Explanation
The pull request changes only evaluatePullRequestWatch and its existing test. The implementation changes the failure-settlement behavior required by issue #30530. The test assertions verify that behavior and prevent repeated wakes. No unrelated files, schema changes, message changes, or unrelated refactoring appear in the whole-pull-request diff.

Do not re-arm the wake for later check churn.
After a failure is reported with no checks pending, a later check can appear as pending and then succeed. This assignment records it as owed and produces another identical failure update, even though no failure changed. Repeated jobs can repeatedly wake the agent. Track checks owed by the failure update instead of re-arming on every sweep.

…failure
When one check failed while others were still running, the watch woke the
agent once for the failure and then never again: "checks passed" and the
settled wake both require zero failures. Agents that ended their turn to wait
for the remaining checks stayed waiting indefinitely.
A failed gate now owes one more wake, sent when the last pending check
finishes, listing the checks still failing.
