Add builder-upgrade: in-place macOS 15 upgrade for Intel r8 builders - #89

reprovision builder-upgrade <host> [--dry-run] [--unquarantine], and a builder-upgrade runner action so Hangar can queue it. Same shape as tart-host-update: quarantine, drain, startosinstall, Xcode 16.4 + CLT before puppet, switch to the 1500 role, puppet twice, verify, autologin back, reboot. Resumable; any failure leaves the worker quarantined. The r8 builders have no admin account, so ssh.run/write_file_as_root/ file_exists take an optional user (relops here; everything else unchanged).

Refuses T2 below 20P. Resumable; any failure leaves the worker quarantined.The r8 builders have no admin account, so
