Summary

Closes #7148. Follow-up to #4312.

When an upstream refuses a turn outright, the Anthropic adapter (stop_reason refusal / content_filter) and the Kiro adapter (CONTENT_FILTERED / GUARDRAIL_INTERVENED) emit an incomplete event with reason content_filter and retryable: false, and the Responses bridge relayed it as response.incomplete with incomplete_details.retryable: false. codex-rs never reads that field: it maps every response.incomplete to a retryable stream error, so the same refused request was sent once and then re-sent through the whole stream retry budget (five more times), each time replaying the full context. Codex only ends a turn on response.failed whose error.code is a terminal-refusal code (the mapping confirmed in #5176 / #5200).

  • bridgeToResponsesSSE gains terminalRefusalAsFailed. With it set, an incomplete that is a content filter with an explicit retryable: false is delivered as response.failed carrying invalid_request_error / invalid_prompt, retryable: false, the turn's usage and the adapter's end_turn. Open message, reasoning, tool-call and web-search items are closed exactly as on the incomplete path. The payload lives in src/bridge/terminal-refusal.ts.
  • Every client on the Responses inbound wire gets this (Codex, the Grok surface, direct SDK callers, the WebSocket transport). The opt-in reuses the enforceDeclaredToolNames wire test in adapter delivery, runTurn delivery, and the web-search and image sidecar loops. The chat and Anthropic inbound wires still receive response.incomplete, which their encoders translate into finish_reason: "content_filter" and stop_reason: "refusal"; the buffered buildResponseJSON path is unchanged.
  • Inside a combo, a zero-output refusal reaches the combo preflight as a failure, which converts it into an HTTP 400 JSON error with the same invalid_prompt code, and comboFailureDecision stops the chain instead of sending the refused prompt to the next target. A test pins this.
  • A response.failed built from an adapter error now carries retryable: false for every terminal-refusal code, not only cyber policy.

Not covered: refusals that reach the bridge as a done without a verdict stay response.incomplete — the proxy's own xAI 403 policy refusal (src/server/responses/policy-refusal.ts), an OpenAI-chat upstream ending with finish_reason: "content_filter", and a Google SAFETY finish. Each needs its adapter to decide whether the stop is permanent first. This change is independent of #7122 (reason normalization, retryability unchanged); the final head merges cleanly with #7122 after the refusal documentation was separated from the reason-normalization paragraph.

  • Maintainer follow-up (4c23fea): the web-search and image sidecar loops decided to loop from the sidecar calls alone, so a complete web_search or image_gen call followed by a non-retryable refusal ran the call and asked the model again with the refused context. They now finalize on such an iteration through the shared isTerminalRefusalIncomplete predicate in src/bridge/terminal-refusal.ts, on every inbound wire. This matches src/web-search/run-turn-loop.ts, which already stops on any incomplete. It resolves the CodeRabbit finding on src/images/loop.ts.

Verification

Independent review round 1 at 480e29f861fd021ee4c1a37af996fd79292b3c64 (Bun 1.4.3, macOS arm64):

Full cross-platform CI dispatched on mirror ci/pr-7152 at that exact SHA: https://github.com/lidge-jun/opencodex/actions/runs/38714484804 (queued at handoff, not a passing result).

  • Added public documentation of the streaming/non-streaming and alternate-wire refusal behavior. Expanded both sidecar regression tests across terminalRefusalAsFailed: true, false, and omitted: all six prove one model send and no sidecar dispatch. Removing only the two guards in a scratch merge rehearsal produces 0 pass / 6 fail; restoring them passes.
  • Focused bun test tests/web-search tests/images tests/adapters/bridge-terminal-refusal.test.ts tests/routing/combo-terminal-refusal.test.ts tests/server/server-kiro-refusal-e2e.test.ts tests/server/server-terminal-refusal-wiring.test.ts tests/adapters/anthropic/anthropic-error-stop-reason.test.ts tests/adapters/anthropic/anthropic-compatible-stream.test.ts tests/adapters/bridge-nonstreaming-terminal.test.ts tests/test-layout.test.ts tests/ci-workflows/file-size-ratchet.test.ts tests/ci-workflows/structure-ssot.test.ts tests/lab/core-lab-boundary.test.ts: 809 pass / 0 fail, 46 files.
  • bun run typecheck, bun run structure:check, bun run privacy:scan, git diff --check: pass. cd docs-site && bun install --frozen-lockfile && bun run build: pass, 577 pages / 81,200 internal links.
  • git merge-tree --write-tree HEAD origin/review-7122-r1, with fix(responses): normalize explicit incomplete reasons #7122 at 4059cb91d351eace21b9b97ff6c3cdceabbb03eb: clean after moving this PR's refusal paragraph away from the reason-normalization paragraph. The original shared-paragraph conflict and 602/600-line resolution were corrected without editing fix(responses): normalize explicit incomplete reasons #7122. Final merged structure:check passes.
  • On the isolated simulated union, bun test tests/adapters/bridge-terminal-refusal.test.ts tests/adapters/bridge-nonstreaming-terminal.test.ts tests/responses/incomplete-reason-normalization.test.ts tests/responses/protocol-direct-encoders-chat.test.ts tests/responses/protocol-direct-encoders-messages.test.ts tests/images/loop-terminal-refusal.test.ts tests/web-search/web-search-terminal-refusal.test.ts tests/test-layout.test.ts tests/ci-workflows/file-size-ratchet.test.ts: 290 pass / 0 fail, 9 files. Union typecheck passes. Only documentation changed after that union test run.
  • Every test/check used temporary OPENCODEX_HOME and CODEX_HOME; server fixtures use ephemeral ports. The new independent bun run test:changed invocation stayed queued behind another lane's user test lock and was stopped before tests ran. A full local suite was not repeated because concurrent worktrees were already consuming the test queue. Focused contract/regression coverage above is the resource exception; full cross-platform coverage is delegated to the exact-head manual CI dispatch. Prior agents' broad results below are historical evidence, not independently re-run results.
  • Manual review found no remaining runtime blocker: promotion requires the canonical content-filter reason plus an explicit false verdict; ordinary caps/stalls/EOFs remain incomplete. The failure envelope matches Codex's response.error.code parser. The buffered path deliberately remains incomplete. Verdict: merge-ready on technical review, pending required CI; keep this contributor PR draft.

Earlier maintainer/author evidence follows:

Maintainer re-check at head 4c23fea23 (Bun 1.4.3, macOS arm64, sandboxed OPENCODEX_HOME/CODEX_HOME):

  • New tests/web-search/web-search-terminal-refusal.test.ts and tests/images/loop-terminal-refusal.test.ts: 2 pass. With only the two loop guards reverted, both fail; the refused request was sent 5 times (web search) and 4 times (image) instead of once.
  • Focused: bun test tests/web-search tests/images plus the eleven files listed below: 830 pass / 0 fail across 47 files.
  • bun run typecheck, bun run structure:check, bun run privacy:scan, git diff --check: pass.
  • bun run test:changed (merge base 7640a942b, 1403 files): 31445 pass / 40 skip / 11 fail / 4 errors. The failing files (cli-access-audio-live, sibling-home-client-sync, service-sqlite-home, service, server-auth) give the same result on clean origin/dev and on this head when run together, 401 pass / 1 fail / 1 error; the service and audio failures were contention with concurrent runs and did not reproduce.
  • Merges cleanly with fix(responses): normalize explicit incomplete reasons #7122 in src/bridge/sse.ts; the only textual overlap is the shared paragraph in structure/transports/responses.md.

Bun 1.4.2 runtime with the pinned Bun 1.4.3 test runner (OCX_TEST_RUNNER_BUN), macOS 26 arm64. The original author verification was on dev at 7640a942b plus the initial fix commit; the current head includes the maintainer follow-ups above.

  • bun run typecheck: passed.
  • Focused: bun test tests/adapters/bridge-terminal-refusal.test.ts tests/routing/combo-terminal-refusal.test.ts tests/server/server-kiro-refusal-e2e.test.ts tests/server/server-terminal-refusal-wiring.test.ts tests/adapters/anthropic/anthropic-error-stop-reason.test.ts tests/adapters/anthropic/anthropic-compatible-stream.test.ts tests/adapters/bridge-nonstreaming-terminal.test.ts tests/test-layout.test.ts tests/ci-workflows/file-size-ratchet.test.ts tests/ci-workflows/structure-ssot.test.ts tests/lab/core-lab-boundary.test.ts: all pass. The new wiring test starts a real server and asserts that /v1/responses streaming ends with exactly one response.failed carrying invalid_prompt (and one upstream send), while /v1/chat/completions still reports finish_reason: "content_filter" and /v1/messages still reports stop_reason: "refusal".
  • bun run structure:check, bun run privacy:scan, git diff --check: passed.
  • Full suite, bun run test: 45,944 pass / 114 skip / 9 fail / 3 errors across 2,307 files. All failures are in four files this PR does not touch, and the same four files fail identically on the unmodified base 7640a942b on this host (123 pass / 8 fail / 3 errors for both base and branch when run on their own):
    • tests/ci-workflows/ci-crash-disposition.test.ts: the three "without GNU timeout" cases (no GNU timeout on this host).
    • tests/ci-workflows/test-runner.test.ts: "changed-mode uses the shared merge base…" exceeds the 5 s test timeout on this host.
    • tests/cli/cli-access-audio-live.test.ts: "native open and non-readiness events cannot claim readiness" (5 s timeouts, the same inherited family reported in fix(responses): normalize explicit incomplete reasons #7122); one further case in this file failed once in the full run and did not reproduce on its own.
    • tests/claude-integration/claude-desktop-remote-hub.test.ts: "connected Desktop uses hub IDs across a cold restart (stored profile=false)".
  • The same change has been running on a downstream fork based on v2.79.0, where an isolated rehearsal (real server, mock Anthropic upstream returning stop_reason: "refusal") shows one response.failed with invalid_prompt and a single upstream send per turn.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed. (structure/adapters/registry.md, structure/providers-and-adapters.md, structure/transports/responses.md)
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. No auth, credential, workflow, or release code changed; the client-visible refusal message passes the adapter detail through redactSecretString.

Summary by CodeRabbit

  • Bug Fixes
    • Responses streaming now reports non-retryable content-filter refusals as response.failed, preserving usage and any partial output.
    • Refusals with no output in combined requests now return an HTTP 400 error and stop further attempts.
    • Other incomplete responses and Chat Completions and Messages refusal formats remain unchanged.

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

… retryable incomplete

When an upstream refuses a turn outright, the Anthropic adapter
(stop_reason "refusal" / "content_filter") and the Kiro adapter
(CONTENT_FILTERED / GUARDRAIL_INTERVENED) emit an `incomplete` event with
reason "content_filter" and `retryable: false` (lidge-jun#4312), and the Responses
bridge relayed it as `response.incomplete` with
`incomplete_details.retryable: false`.

codex-rs never reads that field: it maps every `response.incomplete` to a
retryable stream error, so the same refused request was sent once and then
re-sent through the whole stream retry budget (five more times). It only
ends a turn on `response.failed` whose `error.code` is a terminal-refusal
code.

- `bridgeToResponsesSSE` gains `terminalRefusalAsFailed`. With it set, an
`incomplete` that is a content filter with an explicit `retryable: false`
  is delivered as `response.failed` carrying `invalid_request_error` /
  `invalid_prompt`, `retryable: false`, the turn's usage and the adapter's
`end_turn`. Open message, reasoning, tool-call and web-search items are
closed exactly as on the incomplete path. The payload lives in
  `src/bridge/terminal-refusal.ts`.
- Every client on the Responses inbound wire gets this, not only Codex: the
Grok surface, direct SDK callers and the WebSocket transport share the
same bridge call. The opt-in uses the same test as
`enforceDeclaredToolNames` in adapter delivery, runTurn delivery, and the
web-search and image sidecar loops. The chat and Anthropic inbound wires
still receive `response.incomplete`, which their encoders translate into
  `finish_reason: "content_filter"` and `stop_reason: "refusal"`, and the
  buffered `buildResponseJSON` path is unchanged.
- Inside a combo, a refusal with no output never reaches the client as a
stream: the combo preflight converts the zero-output `response.failed`
into an HTTP 400 JSON error with the same `invalid_prompt` code, and
`comboFailureDecision` stops the chain instead of sending the refused
prompt to the next target. The preflight itself is unchanged; what is new
is that a refusal now reaches it as a failure, and a test pins the result.
- A `response.failed` built from an adapter error now carries
`retryable: false` for every terminal-refusal code, not only cyber policy.

Not covered here: refusals that reach the bridge as a `done` without a
verdict stay `response.incomplete`. That includes the proxy's own policy
refusal for an xAI 403 (`src/server/responses/policy-refusal.ts`), an
OpenAI-chat upstream ending with `finish_reason: "content_filter"`, and a
Google `SAFETY` finish. Each needs its adapter to decide whether the stop
is permanent before the bridge can treat it as terminal.

📝 Walkthrough

The Responses streaming bridge now converts explicitly non-retryable content-filter incompletes into terminal response.failed events when enabled. Responses delivery paths enable this behavior, while Chat, Anthropic, and buffered Responses paths retain their existing representations.

Changes: Terminal refusal handling

Layer / File(s) Summary
Classify and convert terminal refusals
src/bridge/terminal-refusal.ts, src/bridge/sse.ts, src/adapters/anthropic.ts, tests/adapters/bridge-terminal-refusal.test.ts, scripts/test-layout/layout.json, tests/fixtures/test-layout-expected.json
The bridge identifies incomplete events with content_filter and retryable: false, creates a non-retryable invalid_prompt failure, and can emit response.failed with usage. Tests cover message sanitization, output and callback behavior, non-matching events, and terminal-refusal error codes.
Enable conversion across Responses delivery paths

Delivery paths enable terminal-refusal conversion for applicable inbound wires. Server tests cover streaming Responses, Kiro delivery paths, and the distinct Chat, Messages, and buffered Responses representations. Documentation describes the conversion and its scope.
Handle refusals in combo streaming
tests/routing/combo-terminal-refusal.test.ts, scripts/test-layout/layout.json, tests/fixtures/test-layout-expected.json
Tests cover a zero-output refusal becoming an HTTP 400 that stops the combo chain, and a refusal after text output becoming a streamed response.failed event.

Terminal refusals on the Responses wire now end the turn as a failure on the main paths. In the image and web-search flows, however, a refusal that follows a tool call may still trigger another upstream request instead of ending the turn. Resolve or explicitly accept this before merging.

…r call

The web-search and image sidecar loops decided to loop from the sidecar calls alone. A
complete web_search or image_gen call followed by a non-retryable content-filter refusal
ran the call and asked the model again with the refused context, which re-sent the
refusal several times per turn (lidge-jun#7148). Both loops now finalize on such an iteration,
matching run-turn-loop, which already stops on any incomplete.
Exercise the refusal loop guards with Responses failure projection enabled, disabled, and omitted. Document the streaming terminal refusal contract and the preserved buffered and alternate-wire shapes.
