### Kiro refresh attention

If an AWS SSO refresh returns `400 invalid_request` and matching Kiro CLI recovery does not
succeed, the dashboard shows the affected account as needing re-authentication once its access
token expires. The active provider then reports `loggedIn: false` from `/api/oauth/status`.
A still-valid access token remains logged in. You can sign in again through the dashboard;
a later successful automatic refresh also clears the attention state. This status does not
classify the refresh grant as revoked or disable future refresh attempts. Network and server
errors do not trigger this status.

### Kiro credential import

The dashboard offers native Builder ID, Google, and GitHub device login without Kiro CLI.

P2 Badge Preserve attention after definitive fallback rejections

When the stored AWS SSO refresh returns the targeted 400 invalid_request, a matching rotated CLI credential is found, and the recovery endpoint then returns a definitive non-400 client rejection such as 401 or 403 without an allowlisted OAuth error body, this condition throws recoveryError and loses the original refreshAttention evidence. The generic refresh handler consequently records nothing, so even after expiration /api/oauth/status continues reporting the unusable account as logged in, contrary to the documented behavior for unsuccessful matching-CLI recovery. Preserve the original attention error for definitive recovery-side 4xx responses while continuing to exempt transient network, 429, and 5xx failures.

Reject attention from a refresh superseded by an identical-token login.

If an explicit login saves the same access token, refresh token, and expiry while an earlier refresh is pending, saveCredentialWithReceipt clears attention and rotates loginId. The earlier refresh can then fail. Its generation still matches at Line 1500, so this write restores attention for the new login. After expiry, status again reports needsReauth: true. Capture and check a credential-write revision alongside the generation. Rotate that revision on replacement, including identical-token replacement. Add a regression test that resolves the old refresh after the replacement.

Review comment at @src/oauth/store.ts at line 1500:
Update the refresh failure guard that checks account state and credential
generation to also verify a credential-write revision captured when the refresh
began. Rotate the revision on every credential replacement in
saveCredentialWithReceipt, including identical-token replacements, and add a
regression test confirming an older refresh resolving after replacement cannot
restore attention.
