Summary

  • Define a shared design-time quality contract for [DOCS] briefs: searchable titles, explicit reader/task, reader-verifiable acceptance criteria, reader-focused scope, planned user-facing inputs with clear assumptions/open questions, complete contributing-story references, and valid placement.
  • Re-evaluate affected documentation stories during decomposition, revision, review responses, decomposition review, and pre-sync validation, including indirect scope/reader changes and newly relevant, removed, or renamed contributors.
  • Keep sync's no-source-edit boundary: report required source corrections rather than mutating the decomposition or updating an invalid documentation issue.
  • Add contributing-story reference-rendering coverage and bump the design workflow to 0.15.0.

Scope

Design-time changes only. No final implementation facts or implementation PR links are required before code exists. This does not add post-implementation ticket enrichment.

Validation

  • Repository pre-review (--all): passed.
  • Markdown lint and workflow version validation: passed.
  • Design unit tests: 205 passed, including contributing-story reference resolution.
  • Skillsaw from the exact CI-pinned commit c85a25ebbde5a9e8b4cae8f7854dbf7e36e59dfa: passed.
  • Offline link checks and whitespace checks: passed.
  • Full workflow review and scenario traces for reader/scope-only changes, newly relevant contributors, and removed contributors: completed.

The targeted informational review retains pre-existing orphaned-file/token-budget findings, with no new failures; the CI pre-review run passes.

Summary

Changes affect the design package only. They strengthen [DOCS] story criteria and update the decomposition, response, revision, and sync workflows. The design workflow version is now 0.15.0.

Decomposition adds searchable DOCS titles, reader-focused goals and acceptance criteria, complete contributing-story references, and consistent documentation inputs with markers for assumptions and unresolved items. It also checks stable epic and story identities. Response and revision re-evaluate affected DOCS stories when their contributors or relevant design content changes.

Sync adds cross-epic, dependency-first creation planning. It checks required references and DOCS criteria before Jira writes, skips blocked stories while allowing eligible independent work to proceed, and reports blocked work. The workflow does not edit story source files.

The helper adds extract_required_refs, plan_creation_order, and check_ref_availability, with tests for helper and CLI behavior.

Scope

No files under _shared/ changed. The diff also contains no changes to other packages or shared cross-package conventions.

Walkthrough

This PR updates DOCS story criteria and stable identity rules across decomposition and revision workflows. It adds helper commands for dependency-first story planning and reference checks, and updates sync procedures to validate DOCS stories and guard Jira story operations.

Changes

Decomposition and Jira sync

Layer / File(s) Summary
Documentation and identity criteria
design/decomposition-review.md, design/guidelines.md, design/skills/decompose.md, design/skills/respond.md, design/skills/revise.md
The DOCS criteria cover searchable titles, defined readers and tasks, verifiable outcomes, contributing artifacts, and input markers. The guidance also sets stable epic and story identity rules.
Decomposition and revision workflows
design/SKILL.md, design/skills/decompose.md, design/skills/revise.md
The decomposition steps and review cycle are reorganized. Revision guidance covers epic consolidation, story splitting, and implementation reordering.
Reference extraction and creation planning
design/scripts/sync_helper.py, design/references/sync-helper-usage.md
The helper extracts references, plans dependency-first story creation, and checks reference availability. The CLI returns structured results and status codes for these operations.
Sync validation and guarded Jira operations
design/skills/sync.md, design/references/sync-helper-usage.md
The sync procedure validates affected DOCS stories and checks reference availability before story preparation. It skips blocked stories and reports blocked work. It limits dependency reconciliation to the selected Dependency or Blocks link type.

Full details: No-Content-Duplication

Explanation

The design workflow has three architectural files: SKILL.md, guidelines.md, and skills/controller.md. The PR changes only the version in SKILL.md and adds an identity-stability rule to guidelines.md. The rule is not duplicated in the other architectural files.

Full details: Ai-Attribution

Explanation

All 28 commits in the reviewed range use Co-Authored-By: Claude Opus 4.6 for an AI tool. None has an accepted AI attribution trailer. The PR description uses Assisted-by for Claude, but the commit trailers still fail the check.

Resolution

Replace the Claude Co-Authored-By trailer in every reviewed commit with an accepted trailer, such as Assisted-by, and update the PR history.

Full details: No-Orphaned-References

Explanation

The new sync.md reference in design/references/sync-helper-usage.md:241 is dangling relative to that file. The target exists at design/skills/sync.md. The design skill and command files are reachable through the controller.

Review follow-up

Published e2c9b93 and replied to all four inline threads.

  • Promoted the mandatory DOCS validation gate to Step 4, with sequential main steps 1–8 and updated nested labels, references, and sync contract tests.
  • Moved design-dependent provenance verification to the decomposer while keeping independent review limited to its available evidence.
  • Reconciled contributor changes with Dependencies while preserving unrelated prerequisites.
  • Preserved already-synced DOCS identity and added explicit reference-availability checks before updates, with late-contributor and missing-key fixtures.
  • Included reopened stories and contributors in DOCS re-evaluation.

Local validation: 209 design unit tests passed. Repository pre-review, Markdown lint, version validation, CI-pinned Skillsaw, offline links, and whitespace checks passed. CI and automated review on the new commit are queued/running.

The separate attribution-format review item remains for maintainer disposition. Existing attribution has been retained; attribution policy and prior commit history were not rewritten.

Follow-up review update

Published 5fcf627 and answered the three follow-up threads.

  • Restored the speculative or unresolved input condition.
  • Included removed and renamed contributors in response-driven DOCS re-evaluation.
  • Applied the reference guard to DOCS creation, updates, and reopening, with no Jira write or checkpoint on deferral and helper/state coverage for retry eligibility.
  • Consolidated decomposition into 10 cohesive sequential steps, retaining the original source/structure gates, sizing and implementation references, independent review, capped review-fix cycle, provenance check, FLAG behavior, and presentation.

Validation: 233 local unit/contract tests passed. Required pre-review, Markdown lint, version validation, CI-pinned Skillsaw, offline links, and whitespace checks passed. Build/test CI checks on this new commit have passed; automated review was still processing at the last check.

The separately noted attribution-format item is unchanged and still needs maintainer disposition. Existing attribution and repository attribution policy were preserved.

Review corrections published

Published 35a86cd and answered all seven follow-up threads.

  • Added placement review with the synced-identity exception and epic-scoped author verification.
  • Aligned marker-finding evidence boundaries, corrected the Step 3 subheadings, and strengthened boundary/order and manifest-identity tests.
  • Added explicit reference-guard calls before DOCS preparation in both creation and update/reopen paths.
  • Defined next-explicit-sync-only deferral, with no synced-content checkpoint and accurate reporting for new, changed, and reopened stories.

Validation: 241 local unit/contract tests passed. Repository pre-review, Markdown lint, version validation, CI-pinned Skillsaw, offline links, and whitespace checks passed. These tests exercise instruction contracts and helper behavior; no live Jira integration is claimed.

At the last check on this commit, all 10 build/test/lint checks had passed; automated review was still processing. The separate attribution-format finding remains for maintainer disposition. Existing attribution and repository policy have been preserved.

Outside diff Cover later dependencies in the post-sync placement exception.

262-266: Major · Functional Correctness · Quick win

Cover later dependencies in the post-sync placement exception.

If a synced [DOCS] story gains a dependency on a later-numbered prerequisite that does not contribute documentation inputs, this exception names only later contributors. Criterion 7 requires the [DOCS] story to follow all referenced stories, but its synced filename cannot be renumbered. Step 4 can then reject the update even when the dependency already has a Jira key. Extend the exception to later referenced dependencies whose keys are available; keep the existing reference-availability guard. (raw.githubusercontent.com)

Latest review corrections

Published 92afc0a and answered the two new inline threads.

  • Aligned per-epic DOCS review with the full last-story rule, including noncontributing QE/CI stories.
  • Moved DOCS sync-identity verification before placement repair while preserving the normal fix path for confirmed untracked stories.
  • Addressed the review-level prerequisite-dependency case: both the canonical criteria and sync exception now permit later-numbered referenced prerequisite dependencies, not only contributors, for already-synced DOCS stories. All required contributor and dependency keys must still exist before writing. Missing keys retain the existing no-write/no-checkpoint deferral until the next explicit sync.

Local validation: 247 unit/contract tests passed, along with repository pre-review, Markdown lint, version validation, CI-pinned Skillsaw, offline links, and whitespace checks. The added fixtures exercise helper behavior and instruction contracts, not live Jira integration.

CI is not yet complete: at the last check on this commit, 10 checks had passed, none had failed, and automated review was still running. The separate attribution-format finding remains for maintainer disposition; existing attribution and policy were preserved.

Current update: stable identities from creation

Published 6d50f96. This update supersedes the placement-based wording in the initial summary and previous automated review.

  • Epic and story numbers, filenames, and containing paths are fixed from creation. Titles and content can still change.
  • New work appends identifiers above the highest existing number, including retired entries; gaps and retired numbers are not reused.
  • Obsolete files remain with status: removed. Previously synced entries follow the existing Jira closure behavior; never-synced removed entries are skipped rather than created.
  • Removed the numeric-last DOCS placement rule, its post-sync exceptions, and the renumbering/repair path. Explicit contributors, dependencies, and Jira-key availability determine readiness instead.
  • Preserved scope-driven reference reconciliation and the missing-key guard before DOCS rendering/writes/checkpoints, with deferral until the next explicit sync.
  • Sizing and implementation-reference preparation are explicit main steps within the ten-step decomposition workflow.

Validation: 257 local unit/contract tests passed. Required pre-review, Markdown lint, version validation, CI-pinned Skillsaw, offline links, and whitespace checks passed. Tests distinguish instruction contracts from actual helper behavior; no live Jira integration or new allocator is claimed.

Current CI/review state: all build/lint/test checks on this commit passed. CodeRabbit was skipped because reviews are paused; its banner explicitly says this revision has not been reviewed. Fresh review is still needed. The previous attribution-format finding remains for maintainer disposition; attribution and repository policy were preserved.

Latest review fixes

Published e9471c3 and replied to all five new findings.

  • Added evidence-limited stable-identity review and severity guidance without assuming unavailable history, design, or manifest inputs.
  • Replaced the introduced bare reads and locale-dependent fixture I/O with explicit UTF-8 handling; fixed the coverage pointer to Step 7.
  • Corrected epic-combination guidance: an epic remains active while its child work is retained. Whole-epic retirement retains the existing cascade/never-synced skip behavior. Manual Jira reparenting is not a local-ownership workaround; unsupported physical reparenting must be flagged for clarification rather than silently treated as complete.

The retained-story correction is intentionally an authoring-safety fix, not a new effective-parent mapping, manifest schema, or runtime reparenting feature. Helper fixtures reproduce the unsafe removed-parent case and verify the safe active-parent case with stable qualified identity and Jira key.

Local validation: 262 unit/contract tests passed, along with all required pre-review, Markdown lint, version, CI-pinned Skillsaw, offline-link, and whitespace checks. Ten focused tests also passed with an ASCII locale, UTF-8 mode/coercion disabled, and strict ResourceWarning handling; no warnings were observed. No live Jira integration is claimed.

CI is not yet complete: at the last check on this commit, nine checks passed, none failed, and Skillsaw Lint plus CodeRabbit were still running. The prior attribution-format finding remains for maintainer disposition; attribution, history, and repository policy were preserved.

Step-Sequencing correction published

Published da0961a. Step 9 is now one explicit numbered validation procedure:

  1. Assess the review findings using the existing reference, PRD, value, and repair checks.
  2. Always verify DOCS input provenance, including when the review reports zero findings or all findings are dismissed.

Conditional re-review follows both actions and covers edits from either finding fixes or provenance corrections. The existing two-round cap applies across the validation cycle and is checked before another review is dispatched. Reviewer fallback paths, design-free independent review, and Step 10's severity-based FLAG behavior remain unchanged. The workflow still has ten main steps; there is no separate unconditional provenance phase.

Validation: 266 local unit/contract tests passed, including nine Step 9 contracts. Required pre-review, Markdown lint, version, CI-pinned Skillsaw, offline-link, and whitespace checks passed. The targeted contracts/manual traces cover zero findings, provenance-only corrections, combined corrections, and the capped review path; no live workflow/Jira execution is claimed.

At the latest check on this commit, ten GitHub checks passed, none failed, and automated review was still running.

Make DOCS provenance classification complete and ordered.

Action 2 checks only unmarked inputs, so an existing [Assumption] or [Open: §9.N] marker can remain stale. It also gives conflicting results when an item is described by the design but depends on an unresolved open question: one rule says to leave it unmarked, while another requires [Open: §9.N].

Classify every input with explicit precedence. For example: unresolved question → [Open: §9.N]; otherwise design/PRD-supported → no marker; otherwise → [Assumption]. (raw.githubusercontent.com)

Dependency-first sync update

Published in f62a1a55c489e8f55ba4108f6c6d2ab8b508e090.

  • /sync now builds a feature-wide creation plan from Dependencies and required DOCS Contributing stories, rather than relying on per-epic creation order. Existing Jira-key mappings are reused; missing local targets and creation cycles fail preflight before Jira writes.
  • New stories are created in prerequisite-first order across epics, after parent epic keys are available. Each successful mapping is recorded so dependents can be rendered and created in the same invocation.
  • Both creation and update/reopen paths call the executable fresh-reference check before preparation, rendering, writes or content checkpoints. A failed prerequisite or parent blocks affected dependents without preventing eligible independent work from continuing.
  • Local identities remain stable. Contributing-story membership affects creation readiness only; it does not introduce additional native Jira blocking relationships. The decomposition review gates are unchanged.

Validation

  • 326 design helper/unit/contract tests passed, including actual CLI failure exits, cross-epic DOCS contributor ordering, mapped description rendering and fresh-manifest readiness.
  • Repository pre-review, Markdown lint, version validation, the CI-pinned validator, CI-configured offline link checks and whitespace checks passed.
  • These are helper/CLI and instruction-consumer tests; no live Jira writes were performed.

At the latest check on this commit, ten GitHub checks passed with no failures; automated review was still running.

Assert the ordering that the docstring claims for Step 6b.

The docstring says check-ref-availability is called before prepare. The test only asserts that the string exists in the 6b section. If the call moves after prepare, the guard no longer protects the write and the test still passes. The 6a test already asserts ordering.

Scope the assertions to the guard text, not the whole Step 6a section.

The test name and docstring say the guard checks Dependencies and Contributing stories. The assertions search the entire 6a section. Both terms can appear in the prepare recipe or the DOCS notes. The test can pass if the guard drops one of them.

Slice from Reference-availability guard to the `prepare` instruction, as test_6a_guard_precedes_prepare does. Then assert both terms in that slice.

design/scripts/test_sync_helper.py:4655-4671
Trivial · Maintainability & Code Quality · Quick win

Restore the deferral contract assertions removed from the Step 6b and Step 8 tests.

The earlier tests asserted that:

  • Step 6b contains "defer".
  • Step 6a defers DOCS stories with missing keys and skips prepare.
  • Step 8 reports deferred stories and missing keys as distinct categories.

The new tests only check guard-before-prepare ordering and the generic words blocked and "Do not claim complete synchronization". The class docstring still says the guard lets the agent defer the DOCS write. The deferral behavior in the PR objectives is "no Jira writes or checkpoints" when references are missing. No test now pins that behavior. A later edit to sync.md can remove it and the suite still passes.

The test_step8_reports_blocked docstring says "with missing keys", but the test does not check for missing keys. Add assertions for these contracts:

  • Step 6a and Step 6b state that a blocked story gets no Jira write and no manifest checkpoint.
  • Step 8 names the missing keys or references of each blocked story.

957-962: Functional Correctness

The comment's premise does not hold. The repository defines one format for the Contributing stories line: a plain line that uses the canonical Story N.NN form. The parser matches that line. The comment cites bulleted, bold, and wrapped forms, but no inspected contract allows them. The claimed silent omission of contributors therefore depends on an input that the documented format does not produce. The comment also states that the checkpoint blocks later repair. That consequence was not inspected in the available evidence.

…-time changes

Define seven numbered quality criteria for [DOCS] stories in decompose.md
as a single authoritative source: searchable title, explicit reader/task,
reader-verifiable AC, documentation scope, complete documentation inputs
with fact/assumption/open-question distinction, contributing stories
reference in Design Reference, and correct placement.

Add re-evaluation gates so all design-time paths that create or modify
[DOCS] stories check affected stories against these criteria:
- /decompose: criteria are applied during story creation and verified by
the decomposition review (criterion flightctl#8, updated to match).
- /revise: new consistency check re-evaluates [DOCS] stories affected by
direct edits or changes to their contributing DEV/UI stories.
- /respond: new re-evaluation in Step 6 when review-driven design changes
affect user-facing surfaces covered by [DOCS] stories.
- /sync: new Step 3b validates [DOCS] stories before Jira creation/update,
blocking low-quality tickets from reaching the documentation author.

Update the DOCS story template to include a Contributing stories line in
Design Reference for explicit traceability to implementation stories.
Update test fixtures and add a test verifying Contributing stories
reference resolution through sync rendering.

Bump design workflow version 0.14.0 → 0.15.0 (MINOR — behavioral change).

…arker semantics

Address review feedback on the DOCS story quality criteria change:

- respond.md: broaden trigger to include direct DOCS edits, contributing
story changes, and open-question resolutions (not only user-facing
surface changes). Reference full seven criteria instead of partial list.
- sync.md: replace five-item partial restatement with concise reference to
the full criteria in decompose.md. Add indirect contributor detection so
DOCS stories affected by modified contributing stories are caught even
when the DOCS file itself is unchanged; recommend /revise rather than
  editing source during sync.
- revise.md: add open-question resolution as a trigger. Reference full
criteria. Preserve legitimate prerequisite dependencies when updating
  contributing stories.
- decomposition-review.md: clarify that planned inputs (design/PRD-derived)
need no marker; only assumptions and unresolved questions need markers.
- decompose.md: replace product-specific command example (flightctl
rollback) with neutral hypothetical wording in template illustration.

…ed detection

- respond.md: condense from ~18 to ~9 lines, eliminating the token-budget
  FAIL (~6020 → ~5904). Affected-set now covers newly relevant DEV/UI
contributors, reader/scope/AC/assumption changes in the design, and
resolved open-question markers — not only already-listed contributors.
- revise.md: same coherent (a-d) affected-set rule; covers new, removed, or
renamed contributors whose user-facing surface falls within the DOCS
story's scope, even when references are not yet updated.
- sync.md: scope-based detection using the change plan's new/changed/removed
items, not only existing referenced file hashes. Preserves no-source-edit
  invariant.

…endencies, placement, reopened

R1: Promote sync Step 3b to Step 4 and renumber old Steps 4-7 to 5-8,
including all nested sub-step labels (5a/b/c, 6a/b/c) and ~50 prose
cross-references. Eliminates the sub-step numbering warning.

R2: Split provenance responsibility between decomposer and reviewer.
Remove impossible demand on independent reviewer (which lacks the
design document) to verify unmarked input provenance. Add explicit
decomposer-side provenance check (new Step 13) after review rounds
to verify unmarked Documentation Inputs against design/PRD.

R3: Add dependency reconciliation to respond.md DOCS re-evaluation,
matching revise.md's pattern: add new contributor dependencies,
remove obsolete entries, preserve unrelated prerequisites.

R4: Add post-sync placement exemption to criterion 7 in decompose.md
and sync Step 4 gate. Synced DOCS stories retain immutable filenames
when later contributors are appended; sync creates new stories
before updating existing ones, ensuring Jira key availability.

R5: Include reopened DOCS stories and reopened DEV/UI contributors in
affected-set evaluation across sync Step 4, revise, and respond.

Update test fixtures to match renumbered steps (205 tests pass).

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
…rd, reopened

R1: Correct test_sync_helper.py method names/docstrings/variables to
match actual sync sections — test_step5c_* inspect _step5c (epic
cascade), test_step6c_* inspect _step6c (story close). Fix class
docstring, regex anchors, and assert messages. No duplicate methods.

R2: Fix decomposition-review MEDIUM severity to scope marker checks to
PRD-observable evidence; decomposer verifies design-derived provenance
    in Step 13.

R4: Replace inaccurate global ordering claim with per-epic processing
truth. Add reference-availability guard: before rendering a DOCS
update, verify contributor keys exist in manifest; defer if missing.
    Add 4 contract tests for same-epic, cross-epic, missing-key, and
    filename-stability scenarios.

R5: Explicitly include reopened contributors in respond.md affected set.

… respond/review scope

A: decomposition-review criterion 8 now flags items that appear
speculative OR unresolved from PRD context, not only speculative.
Provenance pointer uses semantic reference to decompose Step 8.

B: respond.md affected-set explicitly includes removed and renamed
   contributors alongside listed/newly-relevant/reopened ones.

C: sync reference-availability guard now protects DOCS creation (6a),
updates (6b), and reopening (6b) — not only updates. Deferred DOCS
stories skip Jira write AND checkpoint, remaining eligible for retry.

D: Consolidate decompose.md from 15 to 10 main steps:
   old 1+2→1 (load source/feature), 3→2 (plan), 4→3 (epics),
   5+6+7→4 (stories/sizing/references), 8→5 (coverage), 9→6 (structure),
   10→7 (review), 11+12+13→8 (validate/re-review/provenance), 14→9
(summary), 15→10 (present). All actions, gates, ordering, reviewer
independence, 2-round cap, provenance check, and FLAG behavior
preserved. Update external refs in revise.md and decomposition-review.md
to semantic pointers. Add 7 contract tests for phase count/order and
   DOCS creation defer/retry scenarios.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
…lifecycle

Decompose phase contract: add existence assertions to all ordering checks
so deleting a gate cannot accidentally pass; verify source blocking,
feature identification, sizing XXL stop, IC/TC mapping, structure gate,
review independence, finding validation, max-2 rounds, provenance even
without fixes, FLAG gate, presentation, and [Open: §9.N] preservation.

Sync DOCS reference guard: use valid story ordering (contributors
before DOCS, story-03 not story-02), model skipped contributor
creation, fix detect_changes API (returns dict not JSON string),
fix compute_file_hash name, normalize line-wrapped guard text.
Add lifecycle tests: deferred DOCS stays NEW on retry, resolves
after contributor creation, checkpointed DOCS becomes unchanged.
Add instruction contract test for guard coverage of 6a/6b/reopen.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
…erral policy, tests

1: Add DOCS placement check to decomposition-review criterion 8 as a
   conditional LOW finding — reviewer flags observable ordering;
decomposer verifies sync identity before acting. Preserve canonical
   criterion 7 and reviewer independence.

2: Align MEDIUM severity evidence boundary to "PRD and artifact context"
   matching the rubric text.

3: Assert .find marker existence and correct order before slicing
sync.md guard/callsite text in tests. Missing markers now cause
clear assertion failures, not wrong-block slicing.

4: Replace both tautological filename-identity tests with meaningful
detect_changes/manifest-identity checks: synced DOCS with late
contributor stays unchanged, changed DOCS classified correctly.
   Add type annotation to _manifest_with_keys.

5: Rename stale #### 4a/4b → 3a/3b under decompose Step 3.

6: Add explicit reference-availability guard calls at 6a (create) and
   6b (update/reopen) BEFORE prepare/render/Jira-write/checkpoint.
Add callsite contract tests verifying guard reference and defer
   language in both sections.

7: Define bounded deferral: no retry in current /sync run; continue
per-epic passes; record deferred file/keys/reason; report in Step 8
including deferred count and no-complete-sync warning. Next /sync
invocation picks them up as new/changed. Add deferral-policy and
   Step 8 report contract tests.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
…ness

- Review criterion 8: cover cross-cutting placement (last-epic) per
canonical criterion 7; conditional LOW with manifest-tracked exception.
- decompose Step 8: explicit decomposer-side sync-manifest identity check
before acting on placement findings; request verification when identity
  context unavailable.
- Sync Step 8 report: qualify hierarchy verification for issues actually
created/updated; deferred stories have no Jira parent to verify.
- Test 6a/6b guard: prove guard precedes actual prepare recipe instruction
(position assertion), not just word presence. Verify skip-prepare
  semantics.
- Add placement scenario/identity contract tests: cross-cutting rubric
coverage, decomposer manifest-check instruction, review-rubric text.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
- Step 8: distinguish deferred new (not created in Jira) from deferred
changed/reopened (exist but not updated). Do not assert deferred
  existing issues lack parents.
- Deferral next-run: include `reopened` alongside `new`/`changed` since
no checkpoint clears the reopened state.
- Placement identity: match story filename within its parent epic's
story list (epic-scoped, not basename search). Distinguish confirmed
manifest absence (apply initial placement) from unreadable/ambiguous
  manifest (request verification).
- Add contract tests for reopened deferral wording and Step 8
  new/changed/reopened distinction.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
…end exception to dependencies

A: Review criterion 8 placement uses full last-story-in-epic rule
including non-contributing QE/CI stories, matching canonical
   criterion 7. Cross-cutting last-epic rule preserved.

B: Move manifest identity check inside Step 8 per-finding value
assessment — placement findings check sync identity BEFORE fix,
not after all findings. Generic non-placement fixes proceed
   immediately. Manifest-tracked→dismiss, absent→initial placement,
   unreadable→request verification.

C: Post-sync exemption and sync gate cover later-numbered prerequisite
dependencies alongside contributors. Key check verifies Contributing
stories AND Dependencies. 6b guard wording aligned ("required key"
   not "contributor key").

Add 6 contract tests: full last-story rule with QE/CI, identity gate
before generic fix ordering, criterion 7 dependency coverage,
non-contributing dependency resolution and deferral, sync exception
consistency.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
redhat-chai-bot and others added 15 commits November 3, 2026 19:55
- Step 8 placement branch now explicitly applies the fix for unsynced
stories (re-read/correct/verify) after the identity gate dismisses
synced stories. Generic fix branch handles all non-placement findings.
Both branches share the re-read/verify pattern.
- Tests normalize whitespace before position assertions to handle
  markdown line wrapping.
- Non-contributing dependency fixtures modify only Dependencies section
(not Contributing stories or Documentation Inputs) with observable
assertions proving isolation. Docstrings clarify renderer-only scope.
- Review rubric test asserts both slice endpoints exist before indexing.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
1) Unsynced DOCS placement fix now captures old→new identity and
propagates references through coverage matrix, Dependencies,
Contributing stories, and Documentation Inputs. Verifies no stale
old-identity references remain before re-review. Scoped behind
existing manifest identity gate. Contract test proves propagation
   instruction precedes re-review.

2) Genuine step consolidation to comply with <=10 main-step rule:
   - Step 4: Write Stories (story writing only)
   - Step 5: Sizing Consistency Check (promoted from sub-heading)
   - Step 6: Add Implementation References (promoted from sub-heading)
   - Step 7: Complete and Validate Artifacts (coverage matrix +
structure verification merged — coverage runs first, then
     validation gate before independent review)
   - Step 8: Review Decomposition (renumbered)
   - Step 9: Validate Findings, Re-Review, and Verify Provenance
   - Step 10: Summarize and Present (review summary + user
presentation merged — summary produced before presentation)

   Old→new action-preservation map:
   old1→1, old2→2, old3→3, old4-stories→4, old4-sizing→5,
   old4-refs→6, old5+old6→7, old7→8, old8→9, old9+old10→10

All actions, gates, ordering dependencies, review independence,
max-2 rounds, provenance, FLAG behavior, and user presentation
   preserved. Cross-references updated in decomposition-review.md
(provenance pointer Step 8→9) and test assertions.

…order contracts

A: Placement test uses actual re-review consumer boundary ("Obtain a
re-review of the updated artifacts") instead of first lowercase
match. Asserts full chain: identity gate → update references →
verify no stale refs → actual re-review action.

B: Placement instruction now captures old AND new identity plus path;
explicitly includes epic file cross-references and file links in
   update scope; scopes to specific story (not same-basename matches);
requires both references and file links to resolve correctly.

C: Add 5 producer-order contracts: sizing is main Step 5, impl-refs
is main Step 6, coverage before structure in Step 7, summary before
presentation in Step 10, all artifact steps (4-7) precede review (8).

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
Root-cause fix: story identity (epic number, story number, filename,
path) is now fixed from initial creation, not just after sync. This
eliminates the need for placement gates, post-sync exemptions,
renumber-repair machinery, and reference-propagation safeguards.

Criterion 7: renamed "Stable identity" — numbers fixed from creation,
gaps allowed, retired numbers never reused, no renaming/renumbering
at any point. Best-effort initial ordering guidance only (never a
later gate). Key-availability guard provides correctness: missing
keys defer to next explicit /sync.

Removed:
- Physical "numerically last" placement requirement and post-sync
  exemption from decompose criterion 7
- Conditional LOW placement check from decomposition-review criterion 8
- Manifest-identity read/placement-repair/reference-propagation
  machinery from decompose Step 9
- Placement-gate paragraph from sync Step 4
- Pre-sync blanket rename/renumber permission from revise.md
  (replaced with unified identity-from-creation rule)
- 12 obsolete placement/renumber/identity-gate tests

Added:
- Unified "Identity stability" block in decompose and revise
- 8 new tests: identity stability (criterion 7 contract, no-renumber
in Step 9, allocation above retired numbers, content edit preserves
key, never-synced removed skip, DOCS lower-ID deferred then resolves,
sync guard covers both fields, no position gate)

Preserved: all 10 main steps, criteria 1-6, key-availability guard
at 6a/6b, bounded deferral, no-source-edit sync, Documentation
Inputs/Contributing stories/Dependencies reconciliation, review
independence, max-2 rounds, provenance, FLAG, design 0.15.0.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
… test truthfulness

- Revise critical rules: qualify logical deletion for never-synced items
(skipped, no Jira issue to close; file retained, number reserved).
  Combining-epics likewise qualified.
- Revise identity stability: add epic allocation rule (append above
highest including retired). Prevent epic rename/reparent changing
  existing stories' qualified identity.
- Guidelines: add canonical identity-stability hard limit (loaded for
all phases via controller), covering both stories and epics.
- Tests: rename and redocument all 6 identity stability tests to
accurately describe what they test (helper classification vs
instruction contract vs renderer behavior). Remove vacuous filename
assertion. Explicitly assert never-synced removed file is absent
from plan AND retained on disk.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
…mbining-epics safety

A: Add criterion 9 (Identity consistency) to decomposition review —
   checks visible ID/path self-consistency and flags rename/reuse
proposals. Evidence-limited: no manifest/history required. HIGH
   severity for proven idempotency-breaking proposals.

B: Replace bare open().read() with Path.read_text(encoding="utf-8");
add explicit encoding to all 9 introduced read_text/write_text sites.

C: Fix 05-epics template coverage pointer (Step 8) → (Step 7).

D: Fix combining-epics: do not mark absorbed parent epic status:removed
while any child work should remain active — keep it as active
container. Marking removed cascades/skips all children via existing
helper (lines 740-742). Only remove when all children intentionally
retired. Flag physical reparent as unsupported by file-authoritative
model. Add 3 helper fixtures (removed parent cascades child,
active parent preserves child, safe path with stable key) and
3 instruction contracts (revise guard, review criterion 9,
   coverage pointer).

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
…y alignment

- Move provenance note back under criterion 8 (before criterion 9) so
  each evidence boundary is unambiguous.
- Align HIGH severity to match criterion 9's full action set (rename,
  renumber, move, reuse).
- Clarify that manual Jira reparent does not update local file/manifest
ownership and is not a workaround for retaining children under a
  removed source epic.
- Strengthen safe-path test with actual helper output metadata
  (qualified file path, epic_index, story_index).
- UTF-8/locale proof: all 10 new tests pass under LC_ALL=C with
ResourceWarning strict mode, no unclosed handles.

…re-review

Step 9 is now one coherent validation procedure:
1. Assess review findings (validate refs, cross-check PRD, fix on value)
2. Verify DOCS input provenance unconditionally — runs even when the
review produced zero findings or all findings were dismissed
3. Re-review conditionally if EITHER finding fixes OR provenance
   corrections changed artifacts

Key changes:
- Provenance check moved before conditional re-review (was after)
- No early PASS before mandatory provenance check
- Provenance-only corrections trigger the conditional re-review
- On re-review iterations, both assessment and provenance run again
- Existing cap of 2 review-fix rounds, 3 review paths (resume/new/
inline), and Step 10 FLAG behavior preserved
- Independent reviewer in Step 8 still lacks design document;
provenance remains author-only verification in Step 9

Replace 2 obsolete provenance ordering tests with 6 tests covering:
provenance inside Step 9 before re-review, unconditional execution,
provenance corrections trigger re-review, no early PASS, no extra
mandatory phase, reviewer still lacks design document.

Restructure Step 9 into explicit numbered procedure:
  1. Assess review findings (validate refs, cross-check PRD, fix on value)
  2. Verify DOCS input provenance (unconditional — runs even with
     zero findings)
  #### Re-review (conditional branch — if either action changed
artifacts AND 2-round cap not reached)

Key improvements over prior version:
- Provenance is a numbered procedure item (action 2), not a bold
sub-heading that could be mistaken for a separate phase
- Re-review is a genuine #### conditional branch (repo convention),
  not a demoted bold heading
- 2-round cap explicitly gates re-review dispatch (checked before
obtaining the review, not after recursive call)
- On re-review iterations, full procedure (actions 1+2) runs again
- No early PASS before both mandatory actions complete

Tests use actual Step 9 boundaries and consumer text (heading, action
numbers, real reviewer path text) with marker existence assertions
before ordering comparisons. 9 Step 9 tests (replacing prior 8).

Add plan_creation_order function and CLI subcommand to sync_helper.py
that plans story creation order across all epics based on Dependencies
and DOCS Contributing stories references. Uses topological sort to
ensure prerequisites are created before their dependents in a single
/sync invocation. Detects cycles and missing targets before any Jira
writes.

sync.md changes:
- Step 3: Plan creation order and preflight check before any Jira
writes (including epic creation). Blocked stories with unsatisfiable
references are reported to the user before approval.
- Step 6a: Create new stories in planned dependency-first order across
all epics (replaces per-epic numeric ordering). Reference-availability
guard for all stories with Dependencies or Contributing stories.
- Step 6b: Reference-availability guard for changed/reopened stories.
- Step 4: DOCS quality validation only (reference checks moved to
Step 3 preflight and Step 6a/6b guards).
- Step 8: Report blocked stories (replaces deferred DOCS terminology).

Key design decisions:
- Contributing stories affect creation ORDER only, not Jira dependency
links (only Dependencies create Jira links)
- Already-synced stories with manifest keys satisfy dependencies
  without recreation
- Cycle detection applies only to unsatisfied creation needs, not
  between already-keyed stories
- Failed prerequisite → dependents blocked (no write/checkpoint)
- Blocked stories remain eligible for next explicit /sync

Helper reference updated with new subcommand documentation.

14 new tests: linear chain, cross-epic prerequisite, existing key
reuse, cycle detection, missing target, deterministic order, DOCS
contributing-only ordering, changed DOCS with missing new dep,
mapped-key cycle distinction, multi-ref list, CLI subcommand, and
3 instruction contracts (preflight before epic writes, 6a planned
order, Step 3 plan call).

…stop, test identity

Helper (plan_creation_order):
- _get_required_refs: use item file field for path resolution (not
index scan); report unreadable/missing sources as errors instead of
silently returning empty refs (fail-closed, not fail-open).
- Contributing stories extracted only for DOCS-typed stories (checked
via title prefix), not every story with such a line.
- Classify each dependency target as available (has key), satisfiable
(in approved creation plan), or unavailable (removed with no key,
nonexistent). Unavailable targets produce errors and block dependents.
- Cycle detection via DFS reachability: a node is a cycle member only
if it can reach itself. Downstream-blocked nodes reported separately
with "blocked" diagnostic, not falsely labeled as cycle participants.
- Changed/reopened stories with refs satisfiable by creation plan are
NOT blocked at planning time (runtime guard checks after creation).

sync.md:
- Step 3 preflight: invalid plan (errors non-empty) is a hard stop,
not an acknowledgement bypass. Blocked-only plans can proceed for
  eligible stories after approval.
- Restored epic summary in dry-run and reopened status marker.
- Step 6a: reload current manifest before each dependent's guard.

Tests (20 planner tests replacing 14):
- All fixtures use actual file identities matching _make_dev/docs_story.
- New: reversed cross-epic, self-cycle, cycle-downstream distinction,
removed-no-key unavailable, changed DOCS satisfiable vs unsatisfiable,
deterministic shuffled input, DOCS contrib-only ordering verified
through actual prepare() output (no Jira dep links).
- CLI success with all expected JSON keys.
- 3 instruction contracts with marker bounds.

…ability check

Address four proven consumer gaps in sync_helper.py:

1. Epic ref extraction: _get_required_refs now extracts Epic refs
(Epic N, Epics N,M) alongside Story refs, and recognizes native
Jira keys (PROJ-123) as externally available references. Invalid
local Epic refs (e.g. Epic 999) are diagnosed before writes.

2. CLI exit code: plan-creation-order now returns EXIT_ERROR (1) when
the plan contains errors, instead of unconditionally returning 0.
check-ref-availability also returns nonzero when stories are blocked.

3. Unavailable ref owner exclusion: Stories with unresolvable refs
(nonexistent targets, removed/no-key sources, unreadable files)
are excluded from the ordered list along with their transitive
dependents. Distinguishes real cycles from downstream blocks and
   preflight-blocked stories consistently.

4. Runtime availability check: New check_ref_availability() helper
and check-ref-availability CLI subcommand. Reloads manifest fresh,
checks story+epic refs and owning epic key, handles native Jira
keys as always-available. Never mutates manifest or source files.

34 new tests (314 total, 0 failures) covering epic ref extraction,
native Jira key handling, DOCS Contributing ordering with real
detect_changes items, CLI exit codes via subprocess, unavailable
ref exclusion with transitive blocking, check_ref_availability
ready/blocked/after-recording/no-mutation scenarios, and unreadable
source error handling.

…preflight changed refs

Complete consumer integration for the reference-availability guard:

1. sync.md Steps 6a/6b now explicitly call `check-ref-availability`
CLI on the current story and fresh on-disk manifest BEFORE calling
`prepare`. Nonzero exit blocks/skips that story with precise reason
while continuing independent eligible work. Step 3 preflight
   description covers changed/reopened invalid refs.

2. Shared `extract_required_refs()` top-level function used by both
   planner `_get_required_refs` and runtime `check_ref_availability`.
Same DOCS-only Contributing rule, existing Story/Epic list grammar,
and explicit native Jira keys. Ambiguous same-index files produce
a structured blocked result instead of silently picking candidates[0].

3. Changed/reopened stories with invalid refs (Story 9.99, Epic 999)
now append to errors — CLI returns nonzero at structural preflight
BEFORE unrelated Step 5 epic writes. Valid planned new targets
   remain acceptable before creation.

4. Helper reference (sync-helper-usage.md) documents check-ref-availability
subcommand with bash recipe, JSON output shape, exit semantics,
   and invariants.

12 new tests (326 total, 0 failures): changed/reopened invalid refs
with CLI exit codes, sync.md consumer contracts for 6a/6b guard
wiring, DOCS Contributing with mapped parents + PREP verification,
fresh manifest missing→record→ready flow, failed-parent blocking
with independent ready, ambiguous file detection, portable mocked
unreadable source.

…h, fatal-vs-blocked guard

Address five inline findings and three outside-diff test findings
from automated review:

1. (4458710764, 4458710805) Document plan-creation-order and
check-ref-availability in module docstring and helper reference
table. Clarify that structured invalid/blocked results exit 1 WITH
JSON, while fatal I/O/manifest errors also exit 1 but with stderr
   only and no JSON.

2. (4458710793) Use re.fullmatch for qualified ID validation in
   check_ref_availability — trailing garbage like "story-1.01abc"
is now rejected. Filename regexes (trailing slug expected) unchanged.
Regression tests added for trailing garbage, newlines, valid QID,
   and CLI behavior.

3. (4458710819) Remove unreachable "blocked but no errors" partial-
proceed branch from Step 3; pin the invariant that every blocked
item has at least one root-cause error entry. Preflight stop
before any writes preserved; Step 8 runtime reporting unchanged.

4. (4458710829) Fail-closed result interpretation in Steps 6a and 6b:
three-way distinction between ready (proceed), structured blocked
(skip story, continue independent work), and helper failure
(stop sync, report stderr). No Jira write or checkpoint on blocked
or fatal. Step 8 records per-story missing refs.

5. Outside-diff tests: 6b ordering assertion proves check-ref before
prepare; 6a guard scoped to guard paragraph; behavioral contracts
   for no-prepare/no-checkpoint/Step 8 reporting; fatal-vs-blocked
CLI distinction with corrupted manifest test.

339 tests, 0 failures.

Co-Authored-By: Claude Opus 4.6 <noreply@anthropic.com>
… scoped tests

1. Three-way guard interpretation is now a complete fail-closed
partition with explicit catch-all: ready (exit 0 + valid JSON +
QID in ready only), blocked (exit 1 + valid JSON + QID in blocked
with missing/reason), catch-all failure (every other outcome
including exit 0 invalid JSON, missing/wrong QID, QID in both
lists, malformed fields, unexpected codes). Catch-all stops sync
and reports stderr; no skip or continue.

2. Bash snippet uses `|| REF_EXIT=$?` idiom so expected exit 1 is
captured under errexit without aborting independent work.

3. Step 6b guard explicitly prohibits prepare, render, update, reopen,
and checkpoint on blocked or failed stories (not just checkpoint).

4. Tests: guard-scoped assertions with existence/order bounds before
slicing; 6b ordering proves check before prepare; catch-all covers
exit 0 invalid JSON and conflicting QID; blocked CLI asserts
matching QID/missing-refs/empty-ready; valid QID fixture asserts
actual ready entry and empty blocked; planner negative case pins
   blocked→errors invariant; Step 8 per-story missing/reason contract;
Step 3 no unreachable blocked-success-path wording.

346 tests, 0 failures.

Sync review follow-up published

Published d10f106130f5c305cd9dc5aeace04a652d9953dd and rebased onto current main (70405a68d64650695fce1ad5510ca179854ebc3d). The prior branch changes were preserved; the branch is no longer behind main.

This addresses the five new inline findings and the three outside-diff test findings from the review:

  • Complete qualified-ID matching and precise command/output exit contracts.
  • Strict preflight guidance without the unreachable blocked-without-errors success path.
  • Fail-closed ready/blocked/helper-failure handling in both write paths, with safe capture of the expected blocked exit status.
  • Tests that require the Step 6b check before the actual preparation instruction, scope field assertions to the Step 6a guard, pin no-write/no-checkpoint behavior, and check Step 8's per-story missing-reference/reason reporting.

Validation: 346 local helper/unit/contract tests passed. Repository pre-review, Markdown lint, version validation, the exact CI-pinned validator, CI-configured offline link checks and whitespace checks passed. The preservation range-diff matched all prior branch commits. These tests do not claim live Jira integration; no live Jira writes were performed.

At the latest check on this revision, all ten reported GitHub CI checks passed with no failures or pending checks. Review is still required. The separate decomposition gate-policy changes are not part of this update.

Update the stale "filenames are locked" note in Step 1.

This line says the presence of sync-manifest.json "means" filenames are locked. Line 25 and the Identity stability paragraph now fix filenames, numbers, and paths from creation, whether or not /sync has run. An agent that reads Step 1 first can conclude that files are unlocked before the first sync. Reword the line so the manifest only signals that Jira issues exist.

Proposed wording
-- `.artifacts/design/{issue-key}/sync-manifest.json` (if exists — means
- epics/stories have been synced to Jira and filenames are locked)
+- `.artifacts/design/{issue-key}/sync-manifest.json` (if exists — means
+ epics/stories have been synced to Jira; filenames are fixed from
+ creation regardless)

Minor · Functional Correctness · Quick win

Reconcile the "stories per epic (confirm)" rule with cross-epic story creation.

This rule is outside the diff, but the change makes it stale. Step 6a now creates new stories across all epics in one dependency-first pass. Step 3 asks for one approval of that whole plan. Line 39 still says to process "stories per epic (confirm)". An agent can pause for a per-epic confirmation that the new flow does not define, or skip a confirmation that the rule requires. Reword the rule to match Step 3 and Step 6, for example: "Process epics first, then stories in the approved creation order."

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.

Write helper stderr to a temp file, not to ref_check_err.txt in the working directory.

2>ref_check_err.txt creates an untracked file in the project root on every guard call. The file persists after the run. A later git add -A can commit it. Use mktemp, and reference the variable in the catch-all branch on line 573. Delete the file at the end of the run.

600-608: Existing finding still applies: classify every [DOCS] input, not only unmarked ones.

Action 2 checks only "unmarked" items, so a stale [Assumption] or [Open: §9.N] marker can stay in place. An item that the design describes but that depends on an unresolved question also gets conflicting guidance: one sentence says leave it unmarked, another says mark it [Open: §9.N]. Classify each item with explicit precedence: unresolved question, then design/PRD-supported, then [Assumption].

612-614: Existing finding still applies: define the terminal state when the 2-round cap is reached.

The text says re-review happens only if the cap "has not been reached". It does not say what happens when the second round changes artifacts. Step 10 sets FLAG only for remaining CRITICAL or HIGH findings, so changed but unreviewed artifacts can be presented as validated. Add a cap-exhausted branch. It must set FLAG, report the changes that were not re-reviewed, and ask the user how to proceed.

… clarity

R1: Step 4 DOCS quality failure halts entire sync before Step 5 —
no Jira writes, fresh approval required after /revise correction.
R2: Self-references (own qid, native key, resolved alias) BLOCK
the source in both planner and runtime guard — not silently
satisfied. Covers Dependencies and DOCS Contributing stories.
Native key extraction added for Contributing section.
R3: Per-guard mktemp stderr capture replaces fixed ref_check_err.txt
    in sync 6a/6b and helper reference. Cleanup in every branch;
fatal path reads text before removal.
R4: Remove two Step 3 preview remnants: blocked-stories placeholder
and ({B} blocked) suffix on create total.
R5: Revise manifest note clarifies partial mappings (not "all synced").
Batch rule updated for cross-epic creation batch. Helper reference
corrects old per-epic order promise to dependency-first.

Tests: 403 total (57 new) — self-ref unit/planner/runtime/CLI/DOCS,
quality gate consumer, preview cleanup, guard recipe, approval/order,
Bash temp capture, Contributing native key extraction.

… approval refs

A: Self-ref detection uses canonical QID (story-{N}.{NN:02d}) for
   own-key lookup — non-canonical CLI inputs (story-1.1) now find
their mapped key. Integration tests for canonical/non-canonical/
resolved-alias paths with real manifest fixtures.
B: Self-ref blocked entries use invalid_refs (not missing) with
accurate reason; legitimate missing-ref cases retain their data.
Planner new-story self-ref gets specific reason (not generic
   "unavailable prerequisite"). Renamed test_mapped_cycle_preserved
   to test_unmapped_new_cycle_detected; added mapped-peer positive
control (distinct keys, A→B→A accepted at runtime).
C: Temp capture does cat+rm immediately after helper call (before
branch interpretation) so cleanup cannot be forgotten. Aligned
   6a, 6b and helper-reference recipes. Real documented-recipe
tests with actual helper, real fixtures, errexit, no-clobber
   and allocation-failure proof.
D: Step 6a cross-references Step 5 confirmation covering the
ordered creation batch. Quality gate trace tests verify all
   approval flow cross-references.

Tests: 423 total (77 new vs d10f106 baseline of 346).

Quality-gate and reference-safety follow-up

Published f524bbb349f2e575a6be4a770114cc43caa5d248.

  • A DOCS quality failure in the existing Step 4 validation set now halts the entire proposed sync before Jira writes. Correction requires a fresh preflight, preview and approval; runtime prerequisite failures retain their separate independent-work handling.
  • Planning and runtime checks block self-references even when the issue already has a key, with canonical identity handling and accurate invalid-versus-missing diagnostics. Valid peer references remain supported.
  • The helper reference and both write-path guards share fail-closed status/output handling and temporary diagnostic capture with inline cleanup.
  • Both remaining blocked-preview entries are removed, and the earlier partial-cleanup reply has been corrected.

The outside-diff notes from the review are also addressed: manifest presence describes potentially partial mappings/checkpoints, not the start of identity immutability; creation-order guidance uses the feature-wide plan rather than numeric/per-epic assumptions; explicit plan, epic-operation and cross-epic new-story-batch confirmations are retained, with existing per-epic update/close handling.

Validation: 423 local tests passed, along with repository pre-review, Markdown lint, version checks, CI-configured offline links, whitespace checks and the validator built from the exact CI-pinned source. A fresh full workflow review found no introduced blocking issues in the scoped changes. Evidence consists of helper/CLI, shell and instruction-contract checks; no live Jira writes were performed.

CI is not yet complete: at the latest check on this commit, ten checks passed, none failed, and automated review was still running. The separately held Step 9 gate-policy changes are not included.

Preserve identities on repeat decomposition.

The controller recommends rerunning /decompose from scratch, but Step 1 does not load the existing epic and story artifacts. A new plan can reuse an existing or retired number and leave conflicting files that block /sync.

On a rerun, load and reconcile the existing artifacts, including removed files, before planning. Preserve their identities and removal markers. Clarify the controller’s “from scratch” guidance to match this behavior.

600-608: Check provenance classification for marked inputs.

The check covers only unmarked items, but it also says to fix incorrect markers. A resolved [Open: §9.N] marker can therefore remain stale. An item supported by the design but dependent on an unresolved question also matches two different rules. Check all inputs and the precedence between those rules.

612-614: Check the terminal state at the review cap.

If the second fix round changes artifacts, this condition prevents another review. Step 10 does not define a gate for those unreviewed changes unless a CRITICAL or HIGH finding remains. Check how the workflow reports cap exhaustion and avoids presenting the changed artifacts as reviewed.

The omitted invalid_refs field does not establish an incorrect report: the required reason includes the invalid IDs, and no keys are missing. The documented guard also distinguishes fatal errors from blocked JSON.

The examples in the comment do not use the required dependency format. The inspected contract does not require sync to accept or reject those formats. The regex behavior alone does not establish a defect for supported input.

Recognize Markdown prefixes on contributing-story lines.

A line such as - Contributing stories: Story 1.02 meets the stated reference criterion but is ignored by extract_required_refs. If Dependencies does not also list that story, the plan can order the DOCS story first and the runtime guard can report it ready. Rendering can then produce [unresolved].

Include unavailable reference IDs in blocked[].missing.

When a new story has an unavailable reference, plan_creation_order omits that reference from deps_graph. Its missing list therefore contains a placeholder or other blocked dependencies, but not the unavailable reference. Step 3 requires missing reference IDs in this field.

Record each unavailable ref_qid when detected and include it in missing alongside blocked graph dependencies. Add tests for both cases.

If the runtime cannot resume the original reviewer, the replacement may miss invalid IC or TC references introduced by a fix. The review protocol requires the testplan for these checks.

**Preflight check.** If `errors` is non-empty, the plan is invalid
— **stop**. Errors cover new stories with missing targets, cycles,
unreadable source files, AND changed/reopened stories whose edited
Dependencies or Contributing stories reference nonexistent or
unavailable items. Every `blocked` item has at least one root-cause
entry in `errors`, so a non-empty `errors` list is the single gate.
Present the diagnostics and recommend `/revise` to fix the
decomposition before re-running `/sync`. Do not proceed to any Jira
writes (including epic creation in Step 5) with an invalid plan. Do
not silently create extra issues to satisfy references or proceed
with `[unresolved]` placeholders.

Define how to handle a fatal plan-creation-order failure.

Exit 1 can mean either a plan with errors or a fatal failure with no JSON. Step 3 does not say how to handle the latter, so the agent can proceed without a valid preflight plan.

Handle structured errors as specified. If the output is missing or invalid, or the exit status is unexpected, stop and report stderr before the preview or Jira writes.

### Step 4: Validate `[DOCS]` Stories Before Sync

Before proceeding to Jira operations, validate every `[DOCS]` story
that is **new**, **changed**, or **reopened** in the change plan
against all seven criteria in `decompose.md`. Also include any
`[DOCS]` story whose own hash is unchanged but whose contributor set
changed: a listed contributor was modified, removed, renamed, or
reopened, or a new/changed/reopened `[DEV]`/`[UI]` story's user-facing
surface falls within the `[DOCS]` story's documented scope. Determine
this from the change plan's full item list (new, changed, removed,
reopened), not only from existing references.

If any `[DOCS]` story fails validation, **stop the entire sync** —
do not proceed to Step 5 or any Jira writes (epic creates, story
creates, updates, reopens, closures), success checkpoints, or
content checkpoints for this plan. Present the findings, recommend
`/revise` to correct the source, and require a fresh
preflight/preview/approval cycle after correction.
Sync does not edit story source files.
Do not reuse a stale approval from before the correction.
