Clear Codex approval only after the idle composer settles.

If a scrape stays negative for 400 ms, this condition clears latched approval without checking for the idle composer. It also applies to Cursor. A prolonged partial redraw or another non-approval screen can therefore clear the approval latch before the required 1.5-second idle-composer interval. Track that interval for Codex, and keep Cursor’s existing clearing rule.

Review comment at @src/pty/shared.rs at line 203:
Update the approval-clearing condition in the shared PTY logic so the Codex path
requires the 1.5-second idle-composer interval before clearing latched approval,
rather than relying only on approval_scrape_negative_since. Preserve Cursor’s
existing clearing rule and ensure non-approval redraws cannot clear Codex
approval prematurely.
