Problem

With Codex's /voice mode on, an answered approval can stay latched in hcom indefinitely. Every idle-time delivery attempt is then held with Gate blocked: approval, and messages pile up unread until someone wakes the session by hand.

How it was observed

hcom 0.7.30 on macOS (arm64), a Codex agent with /voice on but muted and a custom status line. An approval was answered normally. hcom then held the approval latch for almost two hours. Between two log checkpoints 46 minutes apart, hcom.log recorded 2,204 Gate blocked: approval lines for that agent. 17 messages queued, and only a manual hcom term inject woke the session. The screen at the time showed Codex's normal idle composer (› Ask Codex to do anything and ? for shortcuts) with no dialog.

Root cause

Since 73bbaa3 ("Keep Codex approval blocked until its dialog clears", in 0.7.30), the Codex approval latch clears only through the screen. The latch drops when the scrape is negative and output has been quiet for APPROVAL_SCRAPE_CLEAR_MS (400 ms). With /voice on, Codex's footer redraws its mic and codex level meters continuously, so output is never quiet for 400 ms and the latch never clears.

Fix

Codex draws its idle composer (empty, with the ? for shortcuts hint) only when no approval or other modal holds the bottom pane.

  • ScreenTracker::is_codex_idle_composer detects that composer. It deliberately does not use is_ready(), because a dialog's › option rows or › history rows also satisfy it.
  • update_delivery_state records when the idle composer was first seen (ScreenState::codex_composer_since).
  • Once the composer has stood for CODEX_COMPOSER_CLEAR_MS (1.5 s), a negative scrape clears the latch the same way quiet output would.

A positive scrape still latches immediately. Cursor and the other scrape-latched tools are unchanged. The only behavior change is a second way to clear a Codex latch that is already negative on screen.

Tests

  • codex_idle_composer_on_an_idle_voice_screen: a real idle /voice screen (text anonymized) is an idle composer and not an approval.
  • codex_idle_composer_is_not_an_approval_dialog_or_a_draft: an approval dialog under › history rows, and a composer with a draft, are not.
  • codex_composer_settles_only_after_a_steady_stretch: settle timing.
  • codex_approval_clears_on_its_idle_composer_while_voice_meters_churn: drives update_delivery_state through dialog, then idle composer with churning meters. The latch holds until the composer has stood for CODEX_COMPOSER_CLEAR_MS, then clears while output is still churning.

Results

Local run on this branch (on main 669a067), macOS arm64, rustc 1.99.0 (the repo pins 1.97.1), cargo test --bin hcom with the test names as filters. cargo fmt --check is clean. Clippy and the full suite were not run on this branch.

  • With the fix: all 4 tests pass.
  • With only the new clearing term disabled (|| codex_composer_settled(...) turned into || false), codex_approval_clears_on_its_idle_composer_while_voice_meters_churn fails at "the idle composer clears the latch". The three detector and timing tests still pass, as expected.

…ven while /voice keeps output churning

Since 73bbaa3 (in 0.7.30) a Codex approval clears only through the screen:
the latch drops when the scrape is negative and output has been quiet for
APPROVAL_SCRAPE_CLEAR_MS. With Codex's /voice on, its footer meters redraw
continuously, so that quiet never comes. Observed in a live session: an
answered approval stayed latched for almost two hours; every idle-time
delivery attempt logged "Gate blocked: approval" (2,204 times in 46
minutes), 17 messages piled up, and only a manual `hcom term inject` woke
the session.

Codex draws its idle composer (empty, with the `? for shortcuts` hint) only
when no approval or other modal holds the bottom pane.
ScreenTracker::is_codex_idle_composer detects it (not is_ready(), which a
dialog's `›` option rows or history also satisfy); once it has stood for
CODEX_COMPOSER_CLEAR_MS (1.5 s), a negative scrape clears the latch as quiet
output would. A positive scrape still latches at once; Cursor and other
tools are unchanged.

Tests: the idle /voice screen (a real capture, text anonymized) is an idle
composer; an approval dialog with history `›` rows and a draft are not; the
composer settle timing; and update_delivery_state clears the latch on a
churning idle screen (fails without this change).

📝 Walkthrough

Approval scraping now tracks how long a negative result persists. Latched approval clears after the configured interval even during ongoing screen redraws. The PTY loop limits its poll timeout while approval remains latched.

Changes: Approval clearing during redraws

ScreenState records when scraping becomes negative. Approval-clearing logic checks whether the negative interval reaches 400 ms.

Delivery-state updates clear latched approval after a sustained negative interval or stable output. The PTY loop caps its poll timeout while approval remains latched. A regression test covers partial redraws, renewed dialogs, and sustained negative scraping.

This change is meant to clear a stuck Codex approval state once the idle composer is visibly back. As implemented, approval clears after any 400 ms stretch without a detected dialog, and the same rule now applies to Cursor. During long partial redraws, hcom could treat a pending approval as finished and deliver messages into an active approval prompt. Restrict clearing to the settled Codex idle composer and keep Cursor's existing rule before merging.

…ng an idle composer

is_codex_idle_composer accepted an empty input scrape plus a `? for
shortcuts` line anywhere on screen. With no prompt row drawn, the input
scrape falls back to the ready pattern, which any `› ` inside a line
satisfies (a modal breadcrumb such as `Settings › Model`), and a hint line
quoted in history above that modal then passed as the composer's hint. An
unrecognized modal could thus start codex_composer_since and clear the
approval latch after CODEX_COMPOSER_CLEAR_MS.

Require the bottom-most prompt row and the hint below it. Also test that a
draft or dialog interrupting the idle composer restarts the full wait.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Replace the Codex idle-composer detector with a tool-agnostic rule: a
latched Codex/Cursor approval clears once the scrape has been negative
for APPROVAL_SCRAPE_CLEAR_MS, not only after output goes quiet. A TUI
that never stops redrawing (Codex /voice meters, animated status lines)
held answered approvals forever under the quiet-output rule.

Also cap the Unix poll timeout while an approval is latched. Once the
dialog left and the tool went quiet, the latch was only re-evaluated on
the next 10s poll timeout, so delivery waited up to 10s after every
approval (measured live: 10s -> 0.6s). Windows already re-evaluates on
its ~120ms quiet timeout.

🎯 Functional Correctness | 🟠 Major | 🏗️ Heavy lift

Clear Codex approval only after the idle composer settles.

If a scrape stays negative for 400 ms, this condition clears latched approval without checking for the idle composer. It also applies to Cursor. A prolonged partial redraw or another non-approval screen can therefore clear the approval latch before the required 1.5-second idle-composer interval. Track that interval for Codex, and keep Cursor’s existing clearing rule.
