This PR makes the task-history delete path obey the same lock discipline as every writer: deleteMany() resolves each task id's file through the canonical lock key - so a symlink alias and its referent cannot delete and write in parallel - takes the per-path file lock before unlinking, and reports per-id failures honestly: failed ids stay in the cache and the store never reports a deletion it did not perform.

This is the last writer in the chain to join the lock discipline. It is the ninth of nine PRs for epic #1375 (chain tracked on #1991), so merging it completes the series.

Closes: #2024 (this unit's tracking issue).
Closes: #1375 (epic - the last unit of the chain; the epic closes when this lands).
Refs: #1991 (chain tracking).

Description

What this PR changes:

  1. TaskHistoryStore.deleteMany() resolves each id's file through the canonical key every other writer uses and takes the per-path file lock before unlinking.
  2. Per-id failure accounting: a failed delete is recorded per id, the batch continues, failed ids stay in the cache, and the write-through is skipped when the whole batch failed.
  3. Consistency ports this unit carries: the approved outside-workspace identity binding in guardedWrite (approvedCanonicalTarget, captured before the approval - the guard only compares against it, never against a lookup done at publish time), the workspace-root resolution fix (an unresolvable root is dropped from the allow-list instead of vetoing every write), and failed-batch assertions rewritten to state the contract instead of comparing a live map with itself.

Why the diff looks bigger than the change - per carried file, and why this unit requires it: the chain is non-cumulative: this branch is based on main, where none of the predecessors have landed yet, and the unit's own tests (the new TaskHistoryStore.deleteSemantics.spec.ts and the ClineProvider delete specs) compile and run against the whole src tree. Every carried file below is byte-identical to the owning unit's head content, is required for this branch to compile and for its own tests to execute, and disappears from this PR's diff when the predecessor merges (merge order: U1 -> U2 -> U3 -> U4 -> U5 -> U8 -> U6 -> U7 -> U9, all predecessors of this unit):

Carried files Owning unit Why this branch must carry them
TaskHistoryStore.ts, its new spec, task-persistence/index.ts, ClineProvider.ts + the two ClineProvider specs this unit the unit's own delta: the delete path's lock discipline and honest per-id accounting.
guardedWrite.ts + guardedWrite.spec.ts, the tool-wiring files (ApplyDiffTool, ApplyPatchTool, EditFileTool, EditTool, SearchReplaceTool, WriteToFileTool + specs) U7 (#1918), U6 (#1915) deleteMany locks through the same canonical key guardedWrite takes - the unit's contract is stated against that guard's semantics, and the tree does not compile without it on main.
observationRegistry.ts + spec, ReadFileTool + spec, Task.ts + dispose test U4 (#1913) read-scope recording imported by the carried guard and tool wiring.
DiffViewProvider.ts + spec, indentation-reader + specs U8 (#1916) the guarded save path the carried guard calls.
safeWriteText.ts + specs U5 (#1914) the publish primitive the carried editor path calls.
safeWriteJson.ts + specs U3 (#1912) the durability/lock-key sibling the delete path's contract mirrors (and the store's merge writer).
eslint-suppressions.json mechanical per-file count entries for the carried files; counts only decrease.

This unit's own delta is 308 a+d / 23 changed executable lines, all in TaskHistoryStore.ts and its spec.

Review focus: src/core/task-persistence/TaskHistoryStore.ts (deleteMany) and src/core/task-persistence/__tests__/TaskHistoryStore.deleteSemantics.spec.ts.

Test Procedure

From the repository root, with dependencies installed:

pnpm --dir src test -- core/task-persistence
pnpm --dir src exec vitest run --globals core/tools/__tests__/guardedWrite.spec.ts
pnpm --dir src exec eslint . --ext=ts --max-warnings=0

Expected: the delete-path contract (every id reported, entries and files kept when the batch failed, the canonical key used for the lock) plus the reconciliation and concurrency suites.
Environment: Windows 11, Node 20, unit layer only.

Pre-Submission Checklist

  • Scope is one gate scope and matches the unit.
  • No .changeset or CHANGELOG changes (AGENTS.md).
  • src/eslint-suppressions.json unchanged - no suppression count increased.
  • New code lints clean (--max-warnings=0) rather than relying on suppressions.
  • Tests added at the lowest layer that would have caught each finding.
  • Every ported test re-measured in this unit's harness, with a negative control.
  • Visual Snapshot item considered: not applicable (no rendered surface).

Visual Snapshots

Not applicable - the delete-path locking is extension-host internal.

Videos (interaction / animation only)

Not applicable.

Documentation Updates

No user-facing documentation change: the delete path's locking is internal, there is no new setting or surface, and no model-facing text changed. No .changeset and no CHANGELOG edit (AGENTS.md).

Additional Notes

  • Budget: own delta 308 a+d / 23 changed executable lines - inside both caps. The mutation gate reports the GitHub diff's changed-line count (1407 against the 500 cap) because the unlanded predecessors inflate it; the row clears on rebase after the predecessors merge.
  • Negative controls measured in this harness: identity comparison dropped -> 2 failed; baseline taken from the post-approval lookup with the caller capture dropped -> 1 failed; missing capture accepted -> 1 failed; failure path evicting the cache and mtime maps -> 3 failed. All restores byte-identical.
  • taskFileMtimes is populated only by the reconcile path, so an un-reconciled store legitimately has an empty map there; the failed-batch test seeds it before asserting presence.
  • Content source of record: the superseded umbrella PR feat(tools): track read completeness and guard the diff-view save path (S4b, #1375) #1833 (closed), replayed onto the current main tip. Split plan of record: comments 5993969784 / 5994039786 / 5994053776 on [Tracking] guarded-write chain (epic 1375): rules, port sources, chain-level rows #1991.
  • Merge order for the chain: U1 -> U2 -> U3 -> U4 -> U5 -> U8 -> U6 -> U7 -> U9. This unit's merge-order predecessor is U7 (feat(tools): route the remaining write tools through the guard (U7, #1375) #1918).

Verification at this head

  • vitest core/task-persistence: 197 passed (11 files).
  • vitest core/tools + integrations/editor: 916 passed, 5 skipped, 2 failed - both failures donor-junction artifacts of the local harness (DEFAULT_WRITE_DELAY_MS resolves from a stale junctioned dist; CI test and compile are green at this head).
  • ESLint (the CI command, full src) exit 0; suppressions unchanged.
  • CI at this head: all required checks green; mutation-diff red on the budget-cap count described above (no surviving mutants in this unit's own lines).

Stryker directive removal (equivalence notes)

Per AGENTS.md the two Stryker suppression comments in the guarded save
path were removed instead of kept; their equivalence claims:

  • writeKind default StringLiteral ("update" -> ""): an empty kind
dispatches exactly like "update" inside guardedWrite - only "edit"
and "create" branch distinctly - and the default-parameter line is
not part of this unit's changed-line selector, so the mutant is
    equivalent / out of scope.
  • saveTask operand of the adoption-probe guard (LogicalOperator):
GuardRejectedError is only thrown by guardedWrite, which is reached
only after the !saveTask check above has already thrown. Whenever
the operand is evaluated saveTask is provably non-null, so dropping
the operand cannot change which branch is taken.

mutation-diff remains advisory for stacked units; maintainers decide any
narrow gate-config exclusion.

Re-review responses (error-fix re-review wave)

  • Dangling leaf link (Major Security): the canonicalization walk now
lstats the lexical leaf when realpath reports ENOENT and refuses
symbolic-link leaves (and unresolvable lstat failures) before authorizing
the write - the publish resolves the name again, so an unchecked referent
could receive the bytes outside the authorized container. Genuinely missing
leaves keep the existing create path. Regression test asserts the guard
rejects before the publish is reached.

Dangling-leaf refusal on this branch (3a5ebbb)

The leaf-link refusal described in the previous round's section now
lands on this branch: the guarded write lstats the lexical leaf before
publishing and refuses a symbolic link that does not resolve, so the
publish can never follow a referent the check did not authorize. The
commit is byte-identical to the copy that first landed on the U7 branch;
negative control: removing the check reddens exactly the dangling-symlink
test. 67/67 green at this head.

Out of Scope Changes disposition (timing, not necessity). The row
lists predecessor-unit implementation (observation tracking, read
completeness, indentation clipping, DiffViewProvider teardown) that U9
does not require. Every PR in this chain targets main, so each diff
carries the open predecessor units until they merge; the declared merge
order places them all before this PR, after which this diff reduces to
the U9 deletion changes plus the minimal canonical-lock and
approved-identity support the issue requires. Nothing listed is
implemented here for the first time:

Listed item Owning PR(s) in this chain (merge before this one)
observation tracking / read completeness 1910-1913 (U1-U4)
indentation clipping 1913 (U4)
DiffViewProvider teardown 1916 (U8)
guarded-write publish / canonical identity 1914 (U5), 1916 (U8), 1918 (U7)

Security Boundaries disposition (already-registered family). The row
repeats the TOCTOU ancestor-swap claim already dispositioned and
registered: the fix lands once at the publish-primitive owner and copies

  • including this unit's guarded deletion path - reconcile at rebase
    (record 6106959976 on issue 1408; tracking 1991).

📝 Summary

Summary by CodeRabbit

  • New Features

    • File reads now distinguish clipped lines from omitted lines.
    • File edits and writes check for changes since a file was read, with additional safeguards for partial reads.
    • JSON writes can be restricted to a specified directory.
  • Bug Fixes

    • File updates are published atomically while preserving existing permissions.
    • Writes through symlink aliases use consistent locking and update the resolved file.
    • Failed task-history deletions leave affected items intact while batch deletion continues for other items.
    • Rejected file edits no longer leave unintended changes in the editor buffer.

The PR adds task-scoped file observations and guarded publication, atomic text and JSON writing, and explicit task-history deletion failures. File tools and diff saves pass write intent and outside-workspace approval data to guarded writes. Task-history deletion retains entries when removal fails and reports failed IDs.

Changes: Observed file versions and guarded writes

Layer / File(s) Summary
Record observations and read completeness
src/core/task/Task.ts, src/core/task/observationRegistry.ts, src/core/tools/ReadFileTool.ts, src/integrations/misc/indentation-reader.ts, related tests
Tasks track observed file versions and completeness. Stable reads record observations. Read results report clipped lines and distinguish complete content from partial, truncated, or lossily decoded content.
Guard and publish file-tool writes
src/core/tools/guardedWrite.ts, src/core/tools/*Tool.ts, src/integrations/editor/DiffViewProvider.ts, related tests
Guarded writes serialize by path, check observed versions, and enforce completeness and workspace rules. File tools pass create or edit kinds. DiffViewProvider publishes through the guard and coordinates cleanup.

Atomic text and JSON publishing

Layer / File(s) Summary
Stage and commit text or byte content
src/services/file-safety/safeWriteText.ts, src/services/file-safety/__tests__/*
safeWriteText resolves publish targets, validates staging paths, preserves target modes, and performs backup-aware atomic publication with durability handling.
Confine and publish JSON at resolved targets
src/utils/safeWriteJson.ts, src/utils/__tests__/safeWriteJson*
safeWriteJson adds optional path confinement and uses resolved targets for locking, merge reads, and publication. It delegates backup and commit handling to safeWriteText.

Task-history deletion

Layer / File(s) Summary
Report failed task-history removals
src/core/task-persistence/TaskHistoryStore.ts, src/core/task-persistence/index.ts, src/core/webview/ClineProvider.ts, related tests
Task-history deletion retains items when file removal fails and reports failed IDs. Batch deletion continues across IDs. ClineProvider cleans artifacts for successfully deleted tasks and updates state.

Security Architecture Review

Security architecture risk: 🟡 Moderate · up to e787e

Version checks and canonical locking improve write safety, but atomic replacement can weaken existing Windows file permissions. Permissions are restored only after publication, and restoration failures are ignored. In directories accessible to other accounts, a previously restricted file could become readable or writable by those accounts.

Retained concerns

  • Medium · security · inferred: New atomic text publication does not preserve Windows access restrictions throughout the transition. The replacement file is renamed into place before the original DACL is restored. Failed DACL capture skips restoration, and failed restoration is swallowed. If staging inherits broader permissions than the original target, other local or shared-directory principals can gain read or write access during this window; interruption or restoration failure can leave that exposure persistent despite a successful return. The merge-base direct-save path wrote the existing file without replacing its security descriptor.

Security review details

Security Blast Radius

  • inferred — The permission-drift concern reaches existing Windows files published through the shared text primitive, including approved direct tool edits. Its independently attackable scope is the affected files accessible to another principal under the replacement DACL; no remote, cross-tenant, or privilege-escalation reachability was established.

Security Findings and Attack Paths

  • inferred — For a target whose explicit DACL is narrower than its directory's inherited permissions, replacement can expose content before restoration. A principal newly permitted by that DACL can read or modify the file without controlling the tool request. Failed restoration can leave the broader access in place; the tests explicitly expect publication to succeed despite capture or restoration failure.

Trust Boundaries and Controls

  • observed — The owning task's observation registry supplies publication authority. The guard rejects absent edit authority, checks cancellation before publication, and compares versions under a canonical advisory lock. Its documented atomicity guarantee applies to writers participating in that lock protocol, not arbitrary external filesystem writers.

Resilience and Maintainability Implications

  • inferred — Without a prior task observation, ApplyDiff can derive content from one read while the preview records a newer token. The guard can then accept earlier-derived content against that newer token. Existing observations prevent this substitution, and unobserved direct edits reject. The same inter-read overwrite exposure existed in the merge-base unguarded save flow, so it is a remaining limitation rather than an introduced concern.

Hardening Proposals

  • proposed — Make Windows permission preservation a pre-commit requirement: restrict staging before writing sensitive bytes, apply and verify the intended DACL before publication, and abort without replacing the original when that guarantee cannot be established.
  • proposed — Bind edit publication to the stat-matched read used to derive its content, rather than permitting a later preview read to supply that identity.

Full details: Persistence Integrity

Explanation

No explicit persistence-integrity failure was introduced. TaskHistoryStore.removeTaskFile awaits lock resolution, lock acquisition, and unlinking, and deleteMany retains failed IDs, continues the batch, writes through only after a successful deletion, and reports partial failure. ClineProvider.deleteTaskWithId awaits cleanup and preserves the batch error. safeWriteJson awaits staging and safeWriteText; publication uses atomic rename, flushed staging, backup handling, and explicit post-commit durability reporting. Changed save callers await guarded publication.

Explanation

The broad diff is explicitly identified as carried predecessor content. The description lists ownership, rationale, merge order, and the focused U9 delta, which aligns the additional files with the stacked-PR workflow.

Full details: Description check

Explanation

The description includes linked issues, implementation details, review focus, test procedure, environment details, checklist status, documentation impact, and additional notes. It also reports known local test and mutation-diff failures with context.

Full details: Security Boundaries

Explanation

The changed guarded-write path still permits an allowlist bypass. guardedWrite.ts:550-572 resolves and validates a canonical target, then replaceIfVersion and createIfAbsent pass that string to safeWriteText at lines 207 and 290. safeWriteText.ts:271-274 resolves the path again, and line 530 renames through the current ancestor directories. If an attacker replaces a validated workspace ancestor with a symlink before this second resolution, the publish can write outside the workspace. The same race affects an approved outside-workspace target, despite approvedCanonicalTarget. fileLock.ts:18-35 locks a pathname and does not pin directory identity. WriteToFileTool.ts:104-156 supplies this approved-write path.

Resolution

Remove the check-then-resolve gap. Bind validation to the exact filesystem target used for publication. Use directory-handle/no-follow operations or another atomic mechanism that prevents target and ancestor replacement between validation and rename. Apply this protection to ordinary workspace writes and approved outside-workspace writes.

Full details: Regression Evidence

Explanation

The prior regression remains unresolved. safeWriteJson.ts:206-210 adds the under-lock confineTo check after resolvePublishTarget runs, but the changed tests only cover pre-lock rejection (safeWriteJson.test.ts:667-751), a symlink escape before locking (:753-780), and lock-key resolution failure with release (safeWriteJson.lockKey.spec.ts:118-152). No test changes the target or symlink referent after lock acquisition and verifies ConfinedPathEscapeError from the under-lock branch.

Resolution

Add a utility-layer safeWriteJson test that starts with an in-scope target, acquires the lock, changes the target or symlink referent outside confineTo, and asserts ConfinedPathEscapeError. Also assert lock release and no temporary file, backup, rename, or outside write. Keep the existing pre-lock and resolution-failure tests.

Full details: Lifecycle Resource Cleanup

Explanation

A changed cancellation path can leak a file watcher. DiffViewProvider.saveChanges() now returns an empty save result at src/integrations/editor/DiffViewProvider.ts:722-728 when teardown starts during guardedWrite. The changed callers do not inspect this result; for example, EditTool.ts:238-249 immediately calls trackFileContext, and the same pattern remains in WriteToFileTool.ts:189-202, EditFileTool.ts:464-475, SearchReplaceTool.ts:234-245, ApplyDiffTool.ts:273-284, and ApplyPatchTool.ts:271-281/577-586. If Task.disposeOnce() disposes fileContextTracker at Task.ts:3597-3600 and starts revertChanges() at Task.ts:3603-3607 while the publish is pending, saveChanges() returns the cancellation result, then the caller invokes trackFileContext. FileContextTracker.trackFileContext() and setupFileWatcher() have no disposed guard (FileContextTracker.ts:48-95), so a new watcher is created after disposal and is not cleaned up. The callers can also continue with post-save state and queued-message work after cancellation.

Resolution

Return and check an explicit completed/cancelled save result in every save caller. Stop before trackFileContext, edit-state updates, success reporting, reset, and queued-message processing when the save did not complete. Add a disposed guard to FileContextTracker.trackFileContext() and setupFileWatcher() so disposal is final. Add a race test that disposes the task while guardedWrite is pending and verifies that no watcher or post-save work is created.

Review comments at @src/core/tools/ApplyPatchTool.ts:
- Around line 109-114: Update the observation handling in the ApplyPatchTool
flow so its internal read never grants full-file authority: set completeness to
true only when a prior observation is complete and its version matches
preReadToken; otherwise record it as false. Keep targeted edit writes allowed,
and update the no-prior-observation test to expect a partial observation and
reject a subsequent full-file write.

Review comments at @src/services/file-safety/__tests__/safeWriteText.spec.ts:
- Line 1052: Update the test assertion around `fs.rmdir` to verify it was called
with the staging directory created by `fsSync.mkdirSync`, rather than accepting
any call; use the directory from the first `mkdirSync` call as the expected
argument.

Review comments at @src/services/file-safety/safeWriteText.ts:
- Around line 421-429: In the safeWriteText flow, track whether the
tempPath-to-targetPath rename committed; after that point, prevent the outer
catch from restoring backupPath over the new content. On
PostCommitDurabilityError, release the backup when applicable, and add a
backup-enabled regression test confirming a failed directory fsync does not
trigger the rollback rename.

Confirmed — this is the same defect as the other three fsync threads, not four separate ones.

The rollback in the catch ran whenever backup mode had renamed target -> backup, including when the failure happened after the commit rename had already published. PostCommitDurabilityError tells the caller the content is at the target path, but the catch had already renamed the backup back over that target, so the error message and the file disagreed.

Fixed in the unit that owns the publish path, #1910, commit 58f803ddb: a committed flag is set immediately after the commit rename and the rollback is skipped once it is set, so only a pre-commit failure can restore the backup.

Regression test at the lowest layer that would have failed (commit rename succeeds, post-commit directory open fails, backup mode on): fails without the guard, passes with it. This unit carries the same copy of safeWriteText.ts, so it inherits the fix once #1910 merges first in the chain order U1 -> U2 -> U3 -> U4 -> U5 -> U8 -> U6 -> U7 -> U9.

Resolving as handled.

Not applicable at this head. Line 109 already reads const complete = prior?.complete === true && prior.version === preReadToken, so an internal read grants full-file authority only when a prior observation was complete and its version matches; otherwise it records false. No change needed.

Not applicable at this head. This unit does not remove || this.isPaused; it adds it. The diff is - if (this.userMessageContent.length > 0) -> + if (this.userMessageContent.length > 0 || this.isPaused), so a paused task does not fall through to the noToolsUsed() retry.

CodeRabbit thread 4226780789 (Minor) on PR Zoo-Code-Org#1917.

safeWriteText now keeps the previous-content copy when the post-commit
parent-directory fsync fails, and reports its location through
PostCommitDurabilityError.backupPath, so the caller owns that file.
safeWriteJson always passes backup:true but never handled the error: the
generic catch logged "Operation failed" and rethrew, leaving a hidden
.safeWriteText.bak_* copy beside the target for every affected write, and
telling callers such as TaskHistoryStore that a write had failed when the new
JSON was already committed at the target.

Catch PostCommitDurabilityError around the safeWriteText call: release the
reported backup best-effort, log the durability caveat, and return, because
the content really is at the target. Every other error keeps the existing
failure path.

Test: new utils/__tests__/safeWriteJson.postCommitDurability.spec.ts drives a
parent-directory open failure through safeWriteJson against a real temp
directory, and asserts the caller's result, that the failure actually fired,
and that no backup or staging residue is left; a second test pins that a
pre-commit failure still rejects. Negative controls: dropping the backup
release turns the first test red, and swallowing every error turns the second
red (1 failed | 1 passed each).
… a teardown it does not own

Port of `0974ad534` (U8, Zoo-Code-Org#1916) to U9. Same defect, open on four units at their current heads -
Zoo-Code-Org#1915 (U6), Zoo-Code-Org#1916 (U8), Zoo-Code-Org#1917 (U9, this PR), Zoo-Code-Org#1918 (U7) - and DiffViewProvider.ts already
carries four distinct blobs across those heads (82b5857 / 44049b5 / 854569b /
15f032a). Authored once in the unit that owns the save-gate teardown and ported in the
declared merge order U1 U2 U3 U4 U5 U8 U6 U7 U9, so the blob table on
#41 traces every copy back to `0974ad534`.

Task.disposeOnce() can reach revertChanges() while saveChanges() owns its post-publish
teardown. runTeardown() made the caller a waiter and returned false, revertChanges() then
skipped its finalization, and the save's own pass closes the views and restores the tabs but
never resets - the tool caller that owns the provider lifecycle may never come back after a
disposal. The provider was left with isEditing true and activeDiffEditor retained.

- runTeardown() records that a cancellation is waiting on the pass that owns the session.
- saveChanges() reports no completed save when a cancellation landed during its post-publish
pass, instead of running diagnostics and the EOL/patch tail against provider state
(newContent, relPath) that the finalization clears.
- revertChanges() closes the session after the owning pass returned, when that pass did not.

Port adaptations for this unit (this branch's runTeardown takes a second `finalize` argument,
which U8's does not):
- The finalization decision is a recorded flag, `teardownPassResets`, set inside
revertChanges()'s own finalize step and cleared when a pass starts - not U8's
`isEditing || activeDiffEditor` state check. On this branch the rejected save's discard pass
also passes a finalize (it restores the preview tabs but does not reset), so "has a finalize
step" is not the same question as "closes the session", and a state check would let a waiter
reset a session the owner had already closed.
- No `ownedTeardown` result check exists in this unit's saveChanges(), so the source commit's
`!ownedTeardown` block was not ported; only the cancellation bail-out was added there.

Verification on this unit: red first with the production hunks absent and the tests ported ->
4 failed | 142 passed. Green -> 146 passed. Negative control: removing the waiter finalization
-> 4 failed | 142 passed, production file restored byte-exactly, post-restore run 146 passed.
tsc --noEmit 50 errors, identical to this branch's baseline and 0 in the touched files; eslint
--max-warnings=0 clean on both files; src/eslint-suppressions.json untouched.

…ed, not as a completed deletion

Pre-merge row (Persistence Integrity, error) on this PR: `removeTaskFile` wrapped three different
operations in one `try` and read any ENOENT out of it as "the file is gone". The lock-key
resolution, the lock acquisition and the unlink all report ENOENT for different things, so a
failure that left the file on disk evicted the cache entry and wrote the store through - the next
reconciliation then contradicted the deletion the caller was told about.

- The three stages are separated. A lock key that cannot be computed, and a lock that cannot be
taken, are reported with a reason naming the stage; only the unlink's own result can prove the
  file is gone.
- The unlink's outcome is captured inside the locked operation, because `withFileLock` rethrows the
operation's error unchanged: a lock failure and an unlink failure otherwise arrive at the same
place and cannot be told apart by their error code.
- A lock that could not be taken at all is settled by asking the named path directly (lstat, which
does not follow a symlink). proper-lockfile creates `<key>.lock` with mkdir, and that reports
ENOENT both when the directory that would hold the file is gone - nothing to delete - and when
the key is an alias whose referent is missing while the named file is still right there. The
lock's errno says nothing about the file, so it cannot be the evidence.
- The cache entry and the write-through are preserved on every failure, and `deleteMany` still
attempts the rest of the batch.

Tests: two new cases - a lock key that cannot be resolved (a propagated, non-ENOENT realpath
failure: `canonicalDirKey` walks up to the nearest existing ancestor for a missing directory, so
"not there" is a key it can still compute) and an ENOENT from lock acquisition. Both assert the
item stays in the cache, no write-through happens, and the reported reason names the stage.
Red first: 2 failed | 15 passed. Green: 17 passed in the spec, 195 across `core/task-persistence`.
Negative controls, one-to-one: treating a resolution failure as deleted -> exactly the resolution
test fails; tolerating a lock-stage ENOENT -> exactly the lock test fails; removing the lstat
evidence step -> the two pre-existing "missing file is a completed deletion" cases fail, which is
what keeps the idempotent path honest; the whole method back to the original single-catch shape ->
exactly the two new tests fail. Mutants restored byte-exactly and re-run at 17 passed.
`tsc --noEmit` stays at this unit's 50-error baseline with no error in either touched file; eslint
`--max-warnings=0` clean on both; `src/eslint-suppressions.json` untouched.
Two artifacts of merging org main a101c61, both of the kind that only exist in the merged tree:

1. src/core/tools/__tests__/readFileTool.spec.ts carried the same import twice - "import type { Task } from "../../task/Task"" at line 21 and again at line 26, one from each side of the merge. oxc reports it as [PARSE_ERROR] Identifier 'Task' has already been declared, which kills the whole file before a single test runs: vitest then says "Tests no tests" and CI's unit-test job is red with no failing test to point at. The second occurrence is removed; the first, in the type-import block, is the one the file's own ordering keeps.

2. This branch carries the hasClippedLines field (5 uses in src/integrations/misc/indentation-reader.ts) and main added src/integrations/misc/__tests__/indentation-reader-unicode.spec.ts, which pins the whole reader result with toEqual - a second-class merge conflict git cannot see. The spec's expectations learn the field, matching production: true for the two clipped-line cases, false for empty input and for both reader-error results. The spec's intent is untouched. Contract change to an existing test, stated explicitly: it pinned the pre-merge shape. numstat 6/0. The negative control was measured on the U6 branch (forcing the production flag to false turns exactly the two clipped-line tests red).

src/eslint-suppressions.json is re-pruned after the merge, as both sides' violation counts move: the only real change is core/tools/__tests__/readFileTool.spec.ts @typescript-eslint/no-explicit-any 96 -> 94 (a decrease, from the duplicate import removal and main's own edits to that file); the rest of the 1731/1731 numstat is the file's own re-serialization. Verified by comparing the parsed files leaf by leaf: one changed leaf, zero increases. eslint . --ext=ts --max-warnings=0 is exit 0 on the merged tree after this (it exits 2 with the stale counts, in both directions).
…helpers, dead code and test hygiene

Six of the seven open threads on this PR, each with the disposition the code justified rather than the row text assumed:

1. safeWriteJson reused a private copy of the errno-code test (_scopeErrorCode) and a third inline copy of the same check (isAbsent). errorCode is now exported from safeWriteText and used for both, so the question of whether an error is ENOENT has one answer in the tree. _resolveScopeRoot is deliberately NOT folded into canonicalDirKey: canonicalDirKey canonicalizes the PARENT of a file and re-joins its name, while the scope root is a directory canonicalized as itself, and at the filesystem root it falls back to the lexical spelling rather than throwing. Folding one into the other would change which spelling decides a scope.

2. ApplyPatchTool's move path computed moveCanonicalTarget only when the destination was outside every workspace root, and returned early on that same condition a few lines later. The value was therefore always undefined at the publish, and the second isPathOutsideWorkspace call at the publish was always false. Checked before deleting, because dead code and a missed wiring need different treatment: the early return is a deliberate policy (a move outside the workspace is refused with a tool error), so the capture was dead, not missing. It was also not harmless - canonicalizeForApproval can throw, so a destination whose name could not be resolved surfaced an exception where the user should have got the tool error. The capture is gone, the publish passes false and undefined, and the comment states the guarantee the code now relies on.

3. safeWriteJson.test.ts carried two tests whose titles duplicated earlier ones and whose scenarios were the same ordering checks written without symlinks - the comment claimed they existed to run on every lane, but the earlier pair is not symlink-based either (the symlink case is a separate test.skipIf on win32). Both duplicates removed. Note the it( count for this file goes DOWN by two: this is a duplicate removal, not lost coverage - the surviving pair asserts the same ordering, and the row asked for it.

4. TaskHistoryStore.deleteSemantics used mockRejectedValue where one rejection was meant; mockClear does not reset implementations, so the rejection outlived the test and later tests were decided by it. Now mockRejectedValueOnce.

5. writeToFileTool.spec restored the realpath and outside-workspace doubles after the assertions, so a failing assertion left realpath rejecting and every later test in the file failed for the wrong reason. Both restores moved into a finally, matching the test above it.

6. safeWriteText.integration.spec was titled for the commit rename but passed backup:true, so the run stopped one step earlier at the backup copy hitting the same EISDIR and the commit rename never ran. The backup flag is gone: the failure now happens where the title says it happens.

Measured: 95 passed, 9 skipped across the six affected specs (safeWriteJson, safeWriteJson.lockKey, applyPatchTool.execute, writeToFileTool, TaskHistoryStore.deleteSemantics, safeWriteText.integration). tsc --noEmit with a local paths override: 0 errors. eslint . --ext=ts --max-warnings=0 exit 0, no suppression-count change.

Coverage gap recorded rather than papered over: no test in this repo drives a move whose destination is outside every workspace root, so the failure path this change removes is verified by reading the control flow (the early return on the same condition, and the throw the capture could raise before the tool error), not by a test. A move-path test is worth its own row.
The compile job's Check formatting step runs 'prettier --check .' and lists 17 files on this PR's merge ref (job 114119333176, log read with the ANSI codes stripped - the [warn] lines are the file list, and 'Code style issues found in 17 files' is the count). Every one of them is inside this PR's own diff, so formatting them stays inside the boundary of what this PR may touch. The previous formatting commit covered only eslint-suppressions.json because the log excerpt that was passed along named that one file; the gate checks the full list, not the excerpt.

Formatting only, and verified as such rather than asserted: prettier --check now passes on all 17; tsc --noEmit with the local paths override reports 0 errors; eslint . --ext=ts --max-warnings=0 exits 0; eslint-suppressions.json is untouched.

Five tests fail in this worktree (DiffViewProvider saveChanges default write delay x2, ClineProvider blanket auto-deny x3). They were classified rather than waved at: the same two spec files were run with the formatting stashed and unstashed, and the failure set is identical before and after - same five names, same counts (5 failed / 347 passed both ways). The unit-test jobs are green at this head on CI, so these are local artifacts (the write-delay pair is the known DEFAULT_WRITE_DELAY_MS junction difference), not something this commit introduced or hides.
…n give

_verdictAfterLockFailure exists because an ENOENT from proper-lockfile is a report about the lock,
not about the task file: mkdir reports ENOENT both when the directory that would hold the lock is
gone and when the key is an alias whose referent is missing. So the named path is asked directly,
and that question has three answers. Only one of them had a test.

The absent case was already reached by an existing case, and now has one that arrives through the
lock failure itself: the file is removed for real, the lock rejects with ENOENT, and the item is
evicted and written through without it. The uncheckable case had nothing: with the check reporting
EACCES, the item has to stay and the report has to carry both facts - which lock failed, and that
the file could not be checked to settle it - with the check's own error as the cause. Reporting only
the lock would read as a plain lock timeout and send the next operator after a peer that never
existed. The third answer, the file is there, was already pinned.

Negative controls, one per branch, measured in this harness against the production line:
- make the ENOENT answer stop settling anything: 2 failed (the new case, and the pre-existing case
  that shares the branch).
- report only the lock failure: 1 failed, exactly the new uncheckable case.
- that second control run against the spec as it stood before this commit reddens nothing in 17
tests, which is the evidence that the branch really was uncovered rather than covered elsewhere.

Baseline: 19 passed, up from 17; tsc unchanged at this branch's 124 error lines with 0 in the touched
file; eslint --max-warnings=0 clean; src/eslint-suppressions.json untouched.
Zoo-Code-Org#2038 turned @typescript-eslint/no-unused-vars back on in src and Zoo-Code-Org#799
added pnpm format:check to Code QA. Replaying this unit onto that main
exposed unused bindings left by main's dead-code removal (ReadFileTool /
SafeWriteTextOptions / lockfile imports, four write-path destructurings
no assertion reads, an unused realpath callback parameter), a duplicate
Task import, and the path import main pruned from DiffViewProvider.spec
that this unit's tests still need. Formatting brought to
prettier-canonical under the repo config.

Conflict notes: the file-safety rollback commits (committed guard,
RollbackFailureError message) were replayed onto main's untouched
file-safety core; Task.ts kept main's runState/runRejections fields
while applying this unit's isPaused removal; eslint-suppressions.json
took main's pruned baseline and the unit's earned reductions.

No behavior change: 21 unit specs 709 passed / 9 skipped, eslint clean,
prettier clean, tsc adds nothing beyond the pre-existing local
donor-package drift.

Pre-merge checks failed. Please resolve the failing checks before merging.

…path

AGENTS.md bans suppression comments in source and the mutation gate
fails any PR that introduces them. The two directives here were
equivalence claims, not fixes; the equivalence now lives in the PR
description:

- the writeKind default StringLiteral ("update" -> "") is not in the
changed-line selector for this unit's delta, and an empty kind
dispatches exactly like "update" inside guardedWrite (only "edit" and
  "create" branch distinctly);
- the saveTask operand of the adoption-probe guard is provably non-null
whenever evaluated (guardedWrite is reached only after the !saveTask
check above has thrown), so dropping the operand cannot change which
  branch is taken.

Comment-only change: no behavior delta.

Pre-merge checks failed. Please resolve the failing checks before merging.

Security Boundaries (port of the same fix landed on the tool-wiring
unit, whose guardedWrite this unit carries unchanged): verifyTarget
checked containment against the CANONICAL target, then guardedWrite
handed the caller's alias to safeWriteText, which resolves it again
before renaming - a symlink swapped in after the check moved the bytes
to a file the containment decision never covered. verifyTarget now
resolves the path ONCE, checks containment (or the approved identity)
against that resolution, and RETURNS it; createIfAbsent and
replaceIfVersion bind the existence check, the version comparison, the
publish and the post-publish token read to the returned canonical
target, so the alias is never resolved again after the decision.
assertCanonicalInsideWorkspace accepts the pre-resolved target so the
decision and the bound publish are the same resolution. Regression
tests pin the canonical publish and the bound version comparison; NC:
publishing to the alias reddens exactly the canonical tests.

Also names the write kind explicitly in WriteToFileTool's approval path
(same carried code) instead of relying on the saveChanges parameter
default, pinned by the full-workflow test.

66/66 guardedWrite.spec, 24 passed/5 skipped writeToFileTool.spec,
eslint/prettier clean, tsc adds nothing beyond the known local donor
drift.
main's tool-call scoping (Zoo-Code-Org#2015) made ToolCallbacks.toolCallId required;
the unit's own spec callbacks name it with the repo's test convention so
check-types passes on the merge ref with either BaseTool shape.

realpathNearest checked only ancestor components: realpath reports ENOENT
for a dangling leaf exactly as for a missing file, and the ancestor walk
then authorized the lexical path. safeWriteText resolves the name again at
publish time, so a link swapped in between verifyTarget() and the publish
could send the bytes to a referent the guard never checked. lstat the leaf
on the ENOENT path and refuse symbolic links (and unresolvable lstat
failures) before authorizing.

Disposition update for the 05:51Z walkthrough row (comment 5994218600, head 3a5ebbb04)

The Security Boundaries ERROR row ("the changed guarded-write path still permits an allowlist bypass — guardedWrite.ts:550-572 resolves and validates a canonical target, then replaceIfVersion/createIfAbsent pass that string to …") verbatim-repeats the check-then-resolve TOCTOU family registered in this PR body this round: removing the gap requires binding validation to the exact filesystem identity through publication — the directory-handle/no-follow publish family that lands once at the publish-primitive owner unit with copies reconciled at rebase (precedent record 6106959976, issue #1408). This unit's own delta (the dangling-leaf lstat check ported at 3a5ebbb04, 67/67 green) is unchanged. Evidence grade: registered-family repeat; no new concrete claim versus the prior wave's row text.

No code change this round; heads unchanged (3a5ebbb04).

Disposition update for the 08:21Z walkthrough rows (comment 5994218600, head 3a5ebbb04)

Security Boundaries (check-then-resolve allowlist bypass) - the row verbatim-repeats the registered check-then-resolve family: safeWriteText re-resolves the validated path before its rename, and removing the gap requires directory-handle/no-follow publication - the family that lands once at the publish-primitive owner unit with copies reconciled at rebase (precedent record 6106959976, issue #1408). This unit's own delta is unchanged. Evidence grade: registered-family repeat; no new concrete claim versus the prior wave's row text.

Regression Evidence (under-lock confineTo rejection test) - the flagged code (safeWriteJson.ts:206-210) and the cited specs are carrier content on this unit (scope registration in this PR body): the under-lock confinement check and its utility-layer test belong to the safeWriteJson owner unit, where the check was authored. The row itself notes "the prior regression remains unresolved" - the disposition is unchanged from the previous wave. Evidence grade: carrier content, owner-unit disposition.

No code change this round; heads unchanged (3a5ebbb04).

Security Boundaries — disposition and sequencing (transparency note)

The blocked finding on this PR belongs to a family of check-to-rename TOCTOU issues: a target is validated by path, then safeWriteText re-resolves the path and commits with fs.rename(path, path), so an ancestor replaced by a link inside that window redirects the publish.

The series already carries the reference mitigation in #1914 (expectedResolvedPath, expectedAncestorIdentities, AncestorReplacedError, and a fail-closed step-2b before the commit rename). #1912/#1913/#1917 are non-cumulative branches and each carries an older generation of safeWriteText.ts, so that mechanism is absent here (0 hits).

Residual-window ownership is already registered: #2148 (fd-relative publication, opened from #1914's checklist), #1414 (cross-process window), #1408 comment 6106959976 (registration rule) and tracking #1991. No new issue is needed; this PR will inherit the mitigation instead of growing a third implementation, because two competing fixes for the same window would be unreviewable and unsafe to merge.

Verification on the rebase will be: rg expectedResolvedPath|expectedAncestorIdentities is non-zero in this branch's safeWriteText.ts/guardedWrite.ts, the ancestor-swap integration test passes, and a failed post-commit re-check leaves no file outside the scope.

This PR's action: declared merge order puts this branch last, so it inherits the pinning on rebase. Note that its guardedWrite is a different generation than #1914's (expectedResolvedPath/expectedAncestorIdentities are absent, and neither branch is an ancestor of the other), so the rebase reconciliation must be a checked step, not an assumption: after rebase, the grep above must be non-zero, the ancestor-swap test must run in this branch, and guardedWrite must pass the resolved pin pairs into safeWriteText.

Security Boundaries — disposition with cross-PR evidence (sequencing, not a duplicate fix)

This PR's blocked finding is the same check-to-rename TOCTOU family as #1912/#1913/#1914: the target is validated by path, safeWriteText re-resolves and commits with fs.rename(path, path), so an ancestor swapped inside that window redirects the publish.

Verified facts (from the triage of this series):

Declared merge order puts this branch last, so the plan is: rebase after #1914/#1912 land, then prove the reconciliation instead of assuming it.

Rebase verification (will be checked in this PR before requesting re-review):

  1. rg -c 'expectedResolvedPath|expectedAncestorIdentities' src/services/file-safety/safeWriteText.ts src/core/tools/guardedWrite.ts is non-zero in this branch.
  2. This branch's guardedWrite passes the resolved pin pairs into safeWriteText (same call shape as feat(tools): guarded write core under the shared lock (U5, #1375) #1914).
  3. The ancestor-swap integration test exists and passes in this branch (swap after the check, fail closed, no file outside the scope, lock released).
  4. git diff upstream/main --stat still matches this PR's declared unit (no inherited hunks from feat(tools): guarded write core under the shared lock (U5, #1375) #1914).

I am not duplicating the mitigation here: two competing fixes for the same window would be unreviewable and unsafe to merge.
