Summary

Every new issue starts as needs-triage, only a maintainer takes the label off, and contributors are told not to work on an issue that still carries it.

Fixes #1036

Changes

  • tools/triage_gate.py + .github/workflows/triage-gate.yml. What it does is a function of the issue's state (labels, label history, reopenings), never of which event woke the run, so a replayed, delayed or cancelled run reaches the same answer:
    • an opened issue gets needs-triage however it was created (the issue forms add it; the API, gh issue create, apps and workflows do not), unless the history shows a maintainer already removed it;
    • an issue reopened by a non-maintainer goes back to triage, with one comment, unless a maintainer removed the label since;
    • the label removed by a non-maintainer is put back, with one comment, unless a maintainer removed it since;
    • an hourly sweep job gives every open issue created since TRIAGE_SWEEP_SINCE that lacks the label the same decision (a failed API call, a disabled workflow, an issue created with a workflow's own GITHUB_TOKEN);
    • the script never removes the label. Apps and bots are not maintainers and are handled like any contributor. "Maintainer" is the login list in MAINTAINERS.md, shared with assign_on_comment.py and issue_dependencies.py (a test pins all three); FORMER_MAINTAINER_LOGINS keeps a retired maintainer's past removals valid.
  • Same trust posture as assign-on-comment.yml: contents: read + issues: write, the checkout action pinned to a full SHA and one file of the default branch, no ${{ }} in a run: line, issue title and body never read, gh limited to the issues endpoints. Runs on one issue are queued per issue; a pending run may be replaced because the state decides, not the event.
  • nightly.yml creates the nightly-failure issue with needs-triage.
  • Documentation: Issue triage in CONTRIBUTING.md, Triage in MAINTAINERS.md (including the secret / personal-data exception to "an issue is never deleted"), GOVERNANCE.md, the issue forms, the PR template, docs/standards/code-review.md (merge precondition 6), CLAUDE.md / AGENTS.md, and the CHANGELOG. Test-module counts (132 → 133) and the 48 → 72 hour reply target in the user manual are brought in line.

Rollout: sweep of the existing open issues (acceptance criterion of #1036)

Read-only sweep on 2026-10-20 of repos/HodeTech/ForgeLM/issues?state=open (pull requests excluded):

Count
Open issues 865
With needs-triage 1 (#1042, filed after the review program)
Without it 864
of those: a severity or urgency label and a wave / phase: 16 / deferred label 810
of those: epics 39
of those: Phase 16 roadmap items #888–#902 (classified by the roadmap, no severity or wave by design) 15

All 864 were opened by the maintainer; none by an outside contributor or an app. No open issue is untriaged yet lacks the label, so none was labelled. The 15 [Triage] issues that carried the label were triaged in the same session (#878 and the closed ones are recorded on their threads). Issues created before TRIAGE_SWEEP_SINCE are not swept; MAINTAINERS.md says so.

Type

  • Bug fix
  • New feature
  • Documentation
  • Refactoring / code quality
  • Test coverage
  • CI / infrastructure

Testing

  • pytest tests/ passes (6190 passed, 98 skipped); tests/test_triage_gate.py has 203 cases
  • ruff check . passes
  • ruff format --check . passes
  • New tests added for changed code. Mutating each decision rule, the workflow if:, the checkout SHA and cone mode, the gh endpoint guard, the bot-suffix rule, the page limit, the event sort, the second look before writing and the nightly label array makes a test fail.
  • python -m forgelm --config config_template.yaml --dry-run works
  • The full documented gauntlet ran on this branch rebased onto fa182e2: exit 0.

Before merging / after merging

  • Before: TRIAGE_SWEEP_SINCE in triage-gate.yml is 2026-10-21T00:00:00Z. If this merges later, set it to the merge date, so issues the maintainer filed by hand in between are not labelled.
  • After: the workflow takes effect on development only. Smoke test with a throw-away issue: open it with gh issue create (expect needs-triage within seconds); remove the label as a non-maintainer, if there is a second account (expect it back with a comment); then close it.
  • Tools that create issues through the API will also get needs-triage; remove it with the maintainer token once the issue is classified. The gate skips an issue whose history already shows a maintainer's removal.

Decisions taken in review

  • Edits after triage do not bring the label back; triage is a point in time. Documented in CONTRIBUTING.md and MAINTAINERS.md.
  • "A maintainer will not review it" is a maintainer practice, not an automated check; code-review.md says so. A required PR check was left out deliberately.
  • An already-open nightly-failure issue that fails again only gets a comment and no new label: it is the same, already-triaged issue.

Checklist

  • My code follows the project's style (ruff formatted) and docs/standards/coding.md
  • I've updated documentation if needed (no Turkish mirror exists for the root governance files; the user-manual line is changed in both languages)
  • I've added tests for new functionality
  • No new dependencies added
  • Config template (config_template.yaml) not affected
  • CHANGELOG entry under [Unreleased]
  • The issue I link has been triaged (it does not carry needs-triage), or this PR links no issue

Summary by Sourcery

Enforce maintainer-led issue triage by keeping needs-triage on every untriaged issue and preventing contributors from starting work until it is reviewed.

New Features:

  • Add a triage gate that labels newly created issues as needs-triage regardless of how they were opened.
  • Restore triage status when contributors reopen issues or remove the triage label, while allowing only maintainers to complete triage.
  • Add an hourly sweep to catch recent issues missed by event-based processing and label nightly failure issues at creation.

Bug Fixes:

  • Prevent missed or bypassed triage labeling for API-created, workflow-created, delayed, replayed, or failed issue events.

Enhancements:

  • Define and synchronize maintainer-controlled triage rules across issue tooling, workflows, and repository guidance.
  • Harden the triage automation with least-privilege permissions, restricted API access, pinned actions, per-issue concurrency, and safe handling of issue data.
  • Expand documentation and contributor guidance to require triage before work begins on linked issues.

CI:

  • Add the Triage gate GitHub Actions workflow for issue events, scheduled sweeps, and manual runs.

Documentation:

  • Document the triage process, maintainer responsibilities, contributor restrictions, and related review and governance requirements across project guidance and templates.

Tests:

  • Add comprehensive tests covering triage decisions, history and reopening behavior, sweep recovery, failure handling, workflow security properties, maintainer synchronization, and documentation requirements.

Chores:

  • Update the changelog, architecture test counts, and user-manual triage response target.

… a maintainer completes triage

The Triage gate workflow (tools/triage_gate.py) keeps needs-triage on every issue until a
maintainer removes it: it labels an issue however it was created, sends an issue reopened by
a non-maintainer back to triage, and puts the label back, with one comment, when anyone but a
maintainer removes it. A maintainer who removed the label before the workflow started is not
overruled. The nightly-failure issue is created with the label, because events caused by
GITHUB_TOKEN start no other workflow.

CONTRIBUTING.md (Issue triage), MAINTAINERS.md (Triage), GOVERNANCE.md, the issue forms, the
pull-request template, the review standard and the agent guides tell contributors not to
work on, or open a pull request for, an issue that still carries the label.

Fixes #1036

Co-Authored-By: Claude Sonnet 5.5 <noreply@anthropic.com>
tools/issue_dependencies.py also keeps a MAINTAINER_LOGINS list and also adds needs-triage
(for an issue whose blocker was closed without being completed). MAINTAINERS.md now names all
three lists, the test pins their parity, and CONTRIBUTING.md's Issue triage section lists that
second way an issue returns to triage.

Reviewer's Guide

This PR implements a maintainer-controlled issue triage gate: a pinned, least-privilege workflow invokes a defensive Python tool to add or restore needs-triage across issue lifecycle events, while extensive tests and repository-wide documentation establish and verify the process.

File-Level Changes

Change Details Files
Added a defensive triage-gate implementation that enforces maintainer-only completion of issue triage.
  • Adds needs-triage on issue creation unless history proves a maintainer already removed it.
  • Restores the label and posts a single notice when a non-maintainer reopens an issue or removes the label.
  • Uses paginated label history, bounded retries, API response validation, and explicit exit codes while never removing labels.
  • Uses fixed REST endpoints through a shell-free gh api wrapper and avoids reading or logging issue content.
tools/triage_gate.py
Introduced a least-privilege workflow to run the triage gate on relevant issue events.
  • Triggers only for opened, reopened, and unlabeled issue events, ignoring removal of other labels.
  • Checks out only the gate script from the default branch at a pinned commit.
  • Grants only contents: read and issues: write, with no concurrency cancellation and no shell interpolation of event data.
.github/workflows/triage-gate.yml
Expanded automated coverage around triage decisions, workflow security, and repository consistency.
  • Adds unit tests for all decision rules, event-history cases, API failures, retries, malformed payloads, logging safety, and idempotence.
  • Pins workflow triggers, permissions, checkout configuration, secret usage, and absence of concurrency.
  • Verifies maintainer and label definitions stay aligned across the triage, assignment, dependency, documentation, and issue-form configurations.
tests/test_triage_gate.py
Ensured workflow-created nightly failure issues enter the same triage queue.
  • Creates nightly failure issues with needs-triage because GITHUB_TOKEN-originated events do not trigger the gate workflow.
.github/workflows/nightly.yml
Documented the maintainer-controlled triage process and contributor expectations throughout the project.
  • Adds the triage workflow and label behavior to contributor and maintainer guidance.
  • Adds governance, code-review, PR-template, issue-form, agent-guide, and changelog updates instructing contributors not to work on untriaged issues.

The new triage automation works for issue events. The hourly repair sweep rechecks every issue already triaged since the cutoff date, so its GitHub API usage grows over time. Once more than 500 such issues are open, the sweep stops working altogether. Nothing breaks at merge time, but the sweep should be bounded soon so it keeps repairing missed labels and does not consume the shared API quota.

Review of #1041:

- GitHub App logins end in "[bot]" and were rejected as invalid, so an App opening, reopening or
un-labelling an issue made the gate exit 2. The login shape now allows the suffix; tests use
  real App logins.
- The decision is a function of the issue's state (its labels, label history and reopenings),
not of the event that woke the run: a replayed, delayed or cancelled run reaches the same
answer, a maintainer's removal after a contributor's reopening is no longer overruled, and a
second look right before the write lets a maintainer's removal in the window win. Events are
ordered by (created_at, id), not by arrival, and a history of exactly 1000 events is read in
full; a longer one fails closed for a non-maintainer and is reported by the sweep.
- An hourly sweep gives every open issue created since TRIAGE_SWEEP_SINCE that lacks the label
the same decision, which repairs a failed API call, a disabled workflow and issues that a
workflow creates with its own GITHUB_TOKEN.
- Runs on one issue are queued per issue (no cancel-in-progress); the job timeout is 10 minutes
and a gh call 30 seconds. A former maintainer's removals stay valid (FORMER_MAINTAINER_LOGINS).
- gh calls are restricted to the issues endpoints of the repository; the repository name, the
action type and the sweep date are validated.
- Docs: an exposed secret is rotated and an issue holding a live secret or personal data is
deleted (the "never deleted" rule had no exception), triage is a point in time, the "will not
be reviewed" rule is a maintainer practice listed in the review standard, label table rows for
enhancement/duplicate/invalid/wontfix, the 72-hour reply target everywhere, test-module counts.
- Tests pin the workflow's trigger, `if`, concurrency, checkout SHA and cone mode, the gh
wrapper's refusals without starting gh, 999/1000/1001-event histories, out-of-order events and
  the nightly label array.

Comment thread tools/triage_gate.py Resolved
…ging every open issue

untriaged_candidates paged the list of open issues newest-first and counted issues that already
carry needs-triage against its page budget; past the budget it raised and handled none of the
candidates it had found, and since labelled issues keep their place in the list, a bulk import of
more than 500 issues would have stalled every later sweep.

The candidates now come from the search API (open, an issue, no needs-triage, created since
TRIAGE_SWEEP_SINCE), so the server does the filtering and every page entry is a candidate. A run
handles at most 500 and says so; handling an issue removes it from the query, so the next run
continues with the rest and no checkpoint is needed. The filters are checked again on what the
(possibly lagging) index returns. gh may use the search endpoint for GET only.
