Summary

compute TotalTokenCount from input and output token counts
share the usage mapping between streaming and non-streaming MAF responses
add regression coverage for completed-turn usage mapping

Testing

dotnet test tests/CodexSdk.MAF.Tests/CodexSdk.MAF.Tests.csproj --no-restore
dotnet test codexsdk.slnx --configuration Release --no-build --no-restore
dotnet build codexsdk.slnx --configuration Release --no-restore

NuGet packaging and publishing were not performed.
