Release: professional READMEs + auto-changeset for every package → master - #28

The auto-changeset step hard-coded db and odoo, so changes to obsidian/ (including Dependabot bumps merged to master) never got a changeset and were never released. Derive the package dirs from pnpm-workspace.yaml via `pnpm -r exec pwd` so new packages are picked up automatically.
- Root README: package overview with npm badges, Claude Code quick start, shared design principles, layout, development and release flow. - db / odoo / obsidian: one structure across packages (quick start for Claude Code, Claude Desktop/Cursor and VS Code, configuration, tools, safety, troubleshooting), English + Vietnamese. - db docs now match the code: defaultAlias makes databaseAlias optional, defaultSchema, db_list_tables limit/offset/namePattern. - obsidian ships README.vi.md. - CONTRIBUTING / SECURITY list obsidian, replace the dead docs/ADDING_A_NEW_MCP.md link with the actual steps, and describe npm Trusted Publishing instead of an NPM_TOKEN secret.
