Summary

What:

  • Add queue-serialized reconciliation that opens confirmed failures and resolves issues from authoritative final results, rejecting older requests and late refresh notifications.
  • Resume pending passes before accepting newer results so retries finish partial issue and history writes; leave publishing and consumer registration disabled.

Why:

  • Keep build polling independent while giving operational issue membership and lifecycle history one ordered writer.
  • Use durable progress to recover interrupted work without cross-table transactions or provider-build ordering assumptions.

Test Plan

  • Add unit tests.
  • Run controller tests with race detection.

Revert Plan

  • Revert this PR. The controller remains unwired; existing validation and progress storage are unaffected.
