Add reviewed memory correction path

Capability

Adds a reviewed durable-memory correction path. Explicit Correction: prompts now supersede the matching same-scope current fact, preserve the old fact as history, expose provenance through memory search, and require review for ambiguous corrections.

Proof

Boundary notes

• No provider keys, .env, generated DBs, personal memory, or unrestricted local paths are committed.
• Proof identities are synthetic: tenant acme, project family, user rohan.
• S13Code still calls the gateway seam for LLM work; the extension is inside scoped memory/runtime contracts.
