Stop repeated delegate status polling

What changed

• stop a foreground turn after three identical pending delegate_status results
• rely on the existing completion watcher for the eventual worker result
• report sanitized per-tool counts at the hard step limit
• add regression coverage from the observed 98-poll production loop
• update the collected-test claims from 3219 to 3222

Verification

• tests/test_agent_loop.py: 37 passed twice
• tests/test_site_claims.py: 11 passed, 1 skipped twice
• full source + macOS Python suite: 3321 passed, 2 skipped
• red team: 263/263
• public hygiene: passed
• OSV: 0 findings across three Python locks and Swift resolution

This changes no permissions, dependencies, provider routes, release tag, installer pointer, or deployed service.
