fix: verify the OpenAI domain and serve a large site icon - #558

Conversation
The OpenAI plugin portal reads a public token from /.well-known/openai-apps-challenge. The token now ships as a Worker var. Claude shows a connector icon from Google's favicon service, which kept a retired skilld mark because the site declared no large icon. The site now serves /apple-touch-icon.png, and the MCP server lists its icons in serverInfo.icons.

📚 Description
The OpenAI plugin portal verifies that we own skilld.dev before it connects the MCP server. It reads a token at
/.well-known/openai-apps-challenge. The token is public, so it ships as theNUXT_OPENAI_APPS_CHALLENGEvar inwrangler.jsoncthrough the deploy workflow. The Claude connector showed a retired skilld mark. Claude takes connector icons from Google's favicon service, and Google kept serving an old icon because the site declared no large one. This adds
/apple-touch-icon.png(the current 512 px mark) with its<link>, and lists the server's icons in the MCPserverInfo.icons. The Claude listing already uses a custom icon URL. The chat tile follows when Google refreshes. The runbook records where each icon comes from.
