fix(artifact-delivery): keep known sweep failures through a spent quota - #483

A retryable failure replaced the recorded tag, so the next check that saw the known failure again alarmed as new.
The registry indexes 5,949 Skills, not about 2,100, so 25 checks per run took two and a half days per cycle.

📚 Description
Two fixes to the
sweep-skill-runstask from #481, which merged before they landed.A retryable failure, such as a spent GitHub quota, replaced the recorded failure tag. The next check that saw the known failure again then alarmed as a new failure. A retryable result now keeps the known failure and its first-seen time.
The registry indexes 5,949 Skills, not about 2,100, so 25 checks per run took about two and a half days per cycle. The task now starts 40 checks per run, so a cycle takes about 37 hours.
