Adds dw init, the side-based d6 initiative procedure: defaults to Party vs Enemies, takes any number of named sides, rerolls ties within the tied group until broken (shown as roll chains), and -r/--rounds N rolls several rounds at once. Stateless, deterministic in tests via an injectable die. DB-108. Note: touches the same README/cli.ts/completions lines as the dw turn PR; whichever merges second gets rebased.
