Problem

Copilot ACP holds session/prompt while an attached asynchronous shell runs. T3 keeps showing foreground work after Copilot has answered, potentially indefinitely for a dev server. Root work that resumes after background completion can also go unseen outside a run.

Change

Use Copilot's root foreground events to settle on idle and open a continuation on a wake. Child events do not settle their parent. Match prompt acceptance against the first text block T3 actually sent, including instruction wrappers and attachment notices. Buffer ambiguous output until acceptance or the RPC response, and defer its continuation until acceptance. If the pending prompt is rejected while the runtime remains usable, preserve that buffered wake in a continuation. A buffered wake that holds only state changes queues no continuation. RPC fallback leaves the existing queued wake in its continuation and projects the later user reply into the user run. Replay uses the newest known native work state, while keeping a completed wake's result separate from a later user cancellation.

Keep foreground settlement separate from pending RPC ownership. A later prompt can dispatch while the old RPC is held, and the old result cannot settle or clear the newer prompt. Stop uses session/close only when advertised, otherwise session/cancel, then requests runtime replacement. Stop also cancels and retires carried background subagents after the prompt RPC has returned. Either can affect Copilot's background work. Other ACP flavors retain their existing serialization and Stop behavior.

Scope and approval

Part of #27301 and #29211, Bug A. Maintainer triage for this substantial fix is pending. A triage request is posted; this PR is submitted for consideration before that triage and claims no maintainer approval. PR #29235 covers the separate replay leak, Bug B. PR #32476 handles dead-runtime task cleanup separately. These changes touch some of the same adapter code and may require conflict resolution when integrated.

This changes the shared server adapter used by web, desktop, mobile, local and remote connections. It does not change client contracts or UI.

Verification

  • The final replay and Stop cases failed against reviewed commit 21d7c19288be for premature acceptance, aborted idle, and unsupported close. The image-only and unmatched-message cases failed against e18d6f24cf76 because idle offered a spurious continuation. The corrected cases pass.
  • Mutation checks prove the dispatch guard and late-error ordering: bypassing the guard completes a run before dispatch; restoring the old late-error ordering leaves runtime count at 1 instead of 2. Both fail through assertions. The late-error test now awaits a failure-callback completion hook that runs on both orderings. The runtime fixture covers cancellation with interruptPromptOnCancel: false and older-response ownership. A rejected-prompt regression checks that the user run remains failed and its buffered wake appears in a completed continuation, while empty or state-only buffers and runtimes scheduled for replacement offer none. A pending Stop close rejects with -32800 before quarantine and offers no continuation.
  • vp run check:changed passed on 7dd739db92ec with 152 tests in 3 files, affected package typechecks, format, lint, knip and GraphQL.
  • In a standalone Linux session on pinned Copilot CLI 1.0.107, the same shell PID and start time were live before prompt 2, after RPC 1 returned cancelled, and at prompt 2 foreground idle. SIGTERM was then sent to that captured PID; the CLI exited 0. The probe did not capture the shell exit status. The native session history identifies the selected model as GPT-6 Luna.
  • A second standalone probe used one GPT-6 Luna request with text blocks produced by T3's real instruction builders. With T3 MCP instructions off and no image block, Copilot's root user.message contained both complete blocks and its RPC returned end_turn. A separate image-only request used the real attachment-path builder and a valid PNG between the text blocks. Its user.message also contained both complete text blocks and the RPC returned end_turn. Both used GPT-6 Luna with T3 MCP instructions off. No T3 app or live T3 state was used.

The follow-up regressions reproduced five assertion failures on the previous implementation: queued wake output attributed to an unmatched user turn, an old buffered idle settling a newer wake, Stop skipping background work after RPC completion, and stale pending-work state after rejected or lost-session prompts. They extend the existing acceptance, Stop and failure tables. The failure tests await the prompt fiber through its existing failure hook before reading pending work.

Independent Claude Opus 5.5 High reviews identified Stop, acceptance, stale-response, reply-attribution and replay-order issues. All blocking findings were corrected. The follow-up at 4dcea59268af found no blocking issues. Its fresh-state correction is included, and the remaining prompt-matching ambiguity is disclosed below. The two attribution/order regressions failed by assertion at 8ce7bbac8114 and now pass. The acceptance table checks that the pre-existing wake and the user reply appear only in their own runs, including steering-style acceptance. Mutation checks also prove that retaining native foreground state and preserving a completed wake's terminal result are necessary.

Not verified: manual T3 app use, live Stop, Windows, or subagent-completion wake delivery. If Copilot rewrites the prompt text, ambiguous output waits for the prompt request to return unless a queued continuation already owns it. Output buffered before the user turn starts stays with the queued continuation. With an unmatched echo, wake output arriving after the user turn starts can appear in that user run. If that request fails while the runtime remains usable, buffered output appears in a background continuation instead of the failed run. A partial lifecycle feed that omits both idle signals can leave a continuation open. Copilot separately reports that a new prompt can cancel background subagents in github/copilot-cli#4555.

Implemented by GPT-6.1 Terra High and GPT-6.1 Sol High through Codex in T3 Code. Reviewed by Claude Opus 5.5 High through Claude Code in T3 Code.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — Copilot will settle foreground turns and accept subsequent prompts while background work remains active. This changes the default execution policy; a human must weigh background-task continuity and Stop expectations.

Walkthrough

The changes add foreground-event handling for Copilot ACP v1 sessions. The adapter tracks prompt acceptance and settlement, buffers foreground updates, and adjusts continuation, overlapping-prompt, prompt-failure, and Stop behavior.

Changes: Copilot foreground events

Layer / File(s) Summary
Foreground event contract and routing
packages/provider-acp/src/server/adapter.ts, packages/provider-acp-registry/src/server/adapter.ts, packages/provider-acp-registry/src/server/copilotAcp.ts
The adapter adds foreground-work notifications and enables foreground-event tracking for opted-in ACP v1 sessions. The Copilot extension advertises foreground events and reports qualifying user messages, turn starts, and idle states.
Overlapping prompts and foreground settlement
packages/provider-acp/src/server/AcpSessionRuntime.ts, packages/provider-acp/src/server/adapter.ts, apps/server/src/orchestration-v2/Adapters/AcpAdapterV2.test.ts, apps/server/src/provider/acp/AcpJsonRpcConnection.test.ts
The runtime supports configured overlapping prompts while keeping dispatch serialized. The adapter buffers foreground updates, matches acceptance text, settles turns against idle events, and handles buffered wake output. Tests cover acceptance, settlement, and cancellation of an overlapping prompt.
Prompt failures, Stop, and runtime replacement
packages/provider-acp/src/server/adapter.ts, apps/server/src/orchestration-v2/Adapters/AcpAdapterV2.test.ts
The adapter updates prompt-failure continuation and restart handling, pending-background-work reporting, and Stop behavior. Tests cover session-close and cancel-only paths, prompt failures, and stale foreground callbacks during rollback replacement.

Merge Risk: Minimal · up to 7dd73

The change makes Copilot sessions settle foreground work correctly while background tasks run. The previously reported problem, where a failed prompt left the session showing pending background work, is fixed in the current code. No actionable merge-blocking risk remains from this review. Live Copilot CLI behavior was not verified, which is normal rollout uncertainty.

Explanation

The title "fix(acp): settle Copilot foreground work while background tasks run" matches the main change. The changeset adds Copilot foreground-event tracking in the ACP adapter, settles foreground work on idle, and handles prompt acceptance and Stop behavior while background tasks run. It is a conventional commit title in plain language and stays within one sentence.

Full details: Description check

Explanation

The description follows the repository template closely. It has the Problem, Change, Scope and approval, and Verification sections, and each section has substantive content. The Problem section states the Copilot ACP issue and its effect on T3. The Change section explains the fix, including why the child events, buffering, Stop behavior, and non-Copilot flavors are handled as described. The Scope and approval section links the related issue (#27301) and a triage request, states that maintainer approval is pending, and explains that the PR claims no approval. It also names related PRs (#29235, #32476) and the shared-adapter impact. The Verification section lists the focused tests, mutation checks, the check:changed run, a standalone Copilot CLI probe, and an explicit list of what was not verified. The author also discloses the known limitations, such as rewritten prompt text and missing idle signals. The template's agent-attribution note is present: the author names the models that implemented and reviewed the change. The description is detailed and mostly complete. Some items are dense and could be clearer. The Change section does not separate the independent fixes the template asks authors to split, and the Scope section admits that substantial triage is still pending. Those points are open risks for maintainers, but they do not make the description incomplete.

The three reported correctness findings and the CodeRabbit buffer finding are addressed in 7dd739d. Each thread has a reply with the regression evidence. Independent Claude Opus 5.5 follow-up review found no blocking issues at 4dcea59; its small fresh-state correction is included in the final head, which passed the required changed checks with 152 tests and affected typechecks.

The execution-policy and maintainer-triage decisions remain pending. The description links the triage request and claims no maintainer approval. It also discloses the remaining attribution ambiguity when Copilot rewrites a prompt echo, along with the manual/live testing limits.
