perf(server): Windows process launches no longer block the RPC thread - #32260
Fixes#25245. On Windows, child_process.spawn() runs CreateProcess on the calling thread. Defender, endpoint security, a busy disk or a bloated kernel token table can hold that call for 0.5 to 10 s, and the server launches git, gh, netstat and provider CLIs on its RPC thread. Each slow launch freezes RPC and heartbeats for that long; past 15 s the client drops to "Connecting".
Change
On Windows the shared Effect ChildProcessSpawner now launches through apps/server/src/process/SpawnWorker.ts, a pool of worker threads:
A worker owns each child process. The server thread gets a stand-in handle that relays stdin, stdout, stderr, extra descriptors, exit, close, kill and ref/unref over postMessage, with backpressure.
A worker blocked inside CreateProcess takes no new launch. Launches go to an idle worker, up to 4; one idle worker stays warm (about 12 MB each).
Effect's Windows taskkill (process-tree kill on interrupt or timeout) goes through the same launcher instead of execFile on the server thread. That is the one change to the Effect patch.
If a worker dies, its children's process trees are killed through another worker, their output ends so readers finish, and the exit reports the kill once taskkill has returned. If taskkill fails, the direct child is killed, the same fallback Effect's own kill uses.
It plugs into NodeChildProcessSpawner.layerWithSpawn, a small hook this PR adds to the Effect patch. #30209 adds the same hook for its Linux launcher, so whichever lands second drops its copy of the patch and both platforms share it.
Because it sits in the shared spawner, it covers every launch that goes through Effect: processRunner, GitVcsDriverCore, VcsProcess, Codex, the ACP providers, OpenCode, the port scanner. #32234 targets the same issue with a worker per command, wired into processRunner and GitVcsDriverCore only, so provider launches through the spawner stay on the RPC thread there. Maintainers should pick one; I'd close whichever doesn't land.
Not covered: launches that call node:child_process directly (the Claude CLI in ClaudeAdapterV2.ts, speech, document text) and node-pty. macOS and Linux keep the stock spawner; #30209 covers Linux.
Scope and approval
A focused fix for the triaged bug #25245, no behavior or setting changes. It is the Windows half of the direction #30209 takes for Linux.
Verification
Windows 11, Node 24.13, Git 2.55, this PC.
Is CreateProcess safe to move off-thread? One launch of a freshly written 400 MB git.exe (Defender scans it inside CreateProcess): from the server thread it blocked 651 ms and froze the thread 646 ms; from a worker it blocked 556 ms and froze the server thread 2 ms. No process-wide lock.
The real spawner under load (chart above): 60 git --version runs, 8 at a time, through ChildProcessSpawner with the stock layer vs this one. With fresh large copies so every launch is scanned, the longest server-thread freeze went from 1,122 / 500 / 391 ms to 6 / 7 / 18 ms, and total time from 2,606 / 1,411 / 2,192 ms to 1,541 / 1,334 / 1,463 ms. With healthy launches: 29 ms to 6 ms, total 318 to 372 ms.
Which thread launches each command in the running server (found in review): a preload that logs isMainThread for every child_process.spawn, 45 s of t3 --auto-bootstrap-project-from-cwd on this repo. Before the last commit all 11 launches ran on the server thread, git included: NodeHttpServer.layer also outputs NodeServices, so Node's own spawner replaced the worker one for every layer above the HTTP server. After it, 10 of 11 run on a worker: git, codex --version, codex app-server, claude --version, npm view and taskkill. The one left is the Claude CLI, which calls node:child_process directly.
before after
11 MAIN 10 WORKER
1 MAIN claude.exe (direct node:child_process)
Then vp run dev with the same preload, seeded with real threads: a Replay turn with approval, the diff panel listing this branch against origin/main, and a terminal running a local server all worked. The server made 897 launches; 896 ran on a worker, and the one on the server thread was the Claude CLI. An earlier 2-minute A/B in the dev app came out within noise; it predates the wiring fix, so its launches never left the server thread, and I've dropped it.
A launcher thread dying (raised in review), measured on Windows 11 with a harness that drives the real layer and counts every launcher thread's errors and exits. The grandchild is started detached, so it leaves libuv's kill-on-close job the way a git or cmd grandchild would, and only a tree kill can end it.
Check
Result
275 hostile commands (missing executable, NUL bytes in command and arguments, a directory as the executable, missing cwd, shell syntax error, stdin to an exited child, kill after exit twice, stdout dropped mid-flood, interrupt during launch, stderr flood with stdout closed), then 300 launches at once
launcher thread errors 0, exits the pool did not ask for 0
Launcher thread terminated while its child runs a grandchild, 50 times
exit reported 50 / 50 (as a failure), child left running 0, grandchild left running 0
taskkill /T /F against our own running child with a grandchild, 200 times
failed 0, nothing left running
taskkill cannot start (PATH without System32), the only way it failed here
main's Effect spawner and this PR both end the child and leave the grandchild, identically
So a launcher thread does not die from anything a command does; when one is killed anyway, the whole tree ends before the exit is reported; and the remaining case needs taskkill itself to be missing, where this PR behaves exactly like main.
Tests:vp test run apps/server/src/process/SpawnWorker.test.ts, 4 passed on Windows. They run on every platform, since the worker layer works anywhere. "runs children on a launcher thread" checks that the server thread holds no ProcessWrap while a child runs; it fails against the stock spawner. vp run check:changed passes.
Not checked: a packaged desktop build (the SQLite reader worker already runs inline worker source the same way in packaged builds), and macOS.
Built with Claude Opus 5.5 in Claude Code (T3 Code), reviewed by GPT-6 Astra.
Node's spawn() runs CreateProcess on the calling thread. On Windows, antivirus
scans and a busy disk can hold that call for seconds, and every git, gh and
provider launch froze RPC and heartbeats for that long (pingdotgg#25245).
On Windows the shared Effect spawner now launches through a small pool of worker
threads. Each worker owns its children; the server thread gets stand-in handles
that relay stdio, exit and kill. Effect's taskkill goes through the same path.
…ocess trees
If a launcher worker exits, readers of its children's output now finish instead
of waiting forever, the whole process tree is killed through another worker, and
writes fail with EPIPE. Errors inside the worker no longer take it down.
Macroscope's review found this PR requires human approval — Windows process launches move off the RPC thread into workers. The new file-level static-analysis suppression requires human review, and the taskkill-failure discussion leaves an explicit decision about potentially unowned descendants.

Full details: Description check
Explanation
The description follows the repository template closely. It has all four required sections: Problem, Change, Scope and approval, and Verification. The Problem section states the bug (#25245, Windows CreateProcess blocking the RPC thread) with environment and symptoms. The Change section explains the worker-pool design, the Effect patch hook, and the taskkill routing. The Scope and approval section links the triaged issue #25245 and explains why the change is a focused fix of an obvious bug, with a note on the related Linux work in #30209. The Verification section is detailed: it names the Windows 11 environment, reports before/after timings, lists the hostile-input and grandchild-kill checks with counts, gives the exact focused test command with 4 passing, and states what was not checked (packaged build, macOS). The template's agent-attribution note is also present at the end. The author also explicitly lists what is excluded from scope. The description is thorough and specific, not vague.

Coordinate signals with the host’s child lifecycle.
On Linux, the host can reap a child before the server receives its exit event. If the PID is reused in that window, an Effect kill or scope cleanup can signal an unrelated process.
Route kill decisions through the host or use another lifecycle-safe mechanism. Effect also sends group signals directly, so changing HostedChild.kill alone is not sufficient. Preserve cleanup of surviving group members.

Not changing this here: SpawnHost.ts is the Linux launcher from #30209, which this PR stacks on. A change to its kill path belongs in that PR. The Windows launcher this PR adds sends kill to the worker that owns the child, which calls ChildProcess.kill on its live process handle, so it does not signal by a possibly reused PID.
The reason will be displayed to describe this comment to others. Learn more.
@SunkenInTime Thanks for clarifying. This finding concerns the Linux launcher in #30209, not the Windows launcher under review here. Following the repository’s stacked-PR review guidance, I won’t request a fix in this PR.
Would you like me to open a follow-up issue for the Linux PID-reuse concern?

[review] Worker16 changed-head review — Windows shared process-spawner delta
Reviewed commit:b1f00f33e39dace23d97c99f0cadec46046f8cb0 Target at review:main@d5fc29fe5cb62db68bfbf824cfabe0ae94ac6ac4 Reviewer: Wladefant, GPT-6, separate nonauthor session. The PR author is SunkenInTime; this is a separate same-family technical review permitted by the temporary #1899 policy. It is not a distinct GitHub-principal or opposite-family approval.
Scope: only the two-commit Windows delta after 462ad4839f0811f153e4e8d473be3f8cc353faf1: SpawnWorker.ts, SpawnWorker.test.ts, the Effect spawner patch, lockfile delta, and the Windows layer-selection hunk. The six-commit Linux helper stack remains with parent PR #30209 and its author; this review does not assess or take custody of that stack.
P2 — worker failure reports child exit before process-tree termination is known
In apps/server/src/process/SpawnWorker.ts, SpawnWorker.fail() calls this.pool.killTree(child.pid) and immediately follows it with child.exit(null, "SIGKILL") and child.abandon(). On Windows, killTree() only starts taskkill /T /F, attaches an error handler that discards the error, and returns without waiting for the taskkill process to exit or checking its exit code.
Therefore a launcher-thread failure can emit the child's terminal SIGKILL result and release the caller's process scope while the original child/descendants are still alive. If taskkill fails (for example, access denied or executable/creation failure), the failure is silently discarded and the original tree has no remaining owner. This contradicts the comment at line 390 ("end the whole tree before reporting the exit") and the PR's stated worker-failure contract.
Requested correction: retain cleanup ownership until the tree termination result is observed; do not publish the terminal child exit or discard the child record before taskkill succeeds (or an equivalent OS-owned process-tree handle confirms termination). Surface a failed cleanup as an explicit failure/owned unresolved state rather than reporting SIGKILL. Add a deterministic worker-failure regression that holds taskkill, proves the child remains cleanup-owned and the exit is not falsely finalized, then releases taskkill; include a negative taskkill-failure case proving no silent orphan/false-success result.
The checked-in SpawnWorker.test.ts has four generic tests but no Windows worker-thread failure/descendant-cleanup regression. The existing SpawnHost.test.ts launcher-death case exercises the Linux helper-process path, not this Windows WorkerThread path.
Validation and remaining gates
Exact-head normal PR workflow #42322470424 completed SUCCESS. Test Server 1 executed src/process/SpawnWorker.test.ts: 4/4 passed on Linux. This does not execute the Windows taskkill path.
The PR body reports Windows 11 measurements and four tests; those are author-reported, not independently observed machine logs in this review.
PR review inventory had Macroscope/CodeRabbit and author entries, no prior separate nonauthor review.
The full PR still carries the six-commit Linux helper stack. The existing Macroscope out-of-scope check is failed; parent #30209 remains OPEN with its own author. Maintainers must resolve the stacked-target/scope disposition rather than treating this Windows-delta review as approval of the Linux stack.
T3 CONTRIBUTING's maintainer-triage/approval requirements for substantial fixes remain applicable; the linked #25245 issue currently has no triage label in the fetched issue record.
Disposition: CHANGES_REQUESTED for the P2 process-tree cleanup ownership defect. No merge, branch write, CI dispatch, Macroscope approval, Windows host acceptance, packaged desktop acceptance, or deployment is claimed.
The reason will be displayed to describe this comment to others. Learn more.
[review] P2 — this reports exit before process-tree termination is confirmed.killTree() is fire-and-forget on Windows: it starts taskkill, suppresses its error, and does not observe its exit code. fail() immediately emits child.exit(null, "SIGKILL") and abandons the streams, so callers can see the process as dead while the child/descendants remain alive; a failed taskkill silently orphans them. Keep the child under cleanup ownership and defer terminal exit/release until taskkill success (or an equivalent OS process-tree handle confirms termination); surface failure explicitly. Add a deterministic worker-failure test that holds taskkill, verifies the child is not finalized, then releases it, plus a negative taskkill-failure case.

Note
Written by claude-opus-5-5 on behalf of Maria
Fixed in 9139b35. When the launcher thread dies, fail() now reports the child's exit and ends its streams only after taskkill /T /F exits. If taskkill fails to start or exits nonzero, it falls back to process.kill(pid, "SIGKILL") on the direct child before reporting, so a failure is no longer swallowed. I did not add the held-taskkill test: worker death and the taskkill path aren't reachable from the test layer without exposing pool internals, and that would grow this PR.

Not adding this. Producing a real EMFILE inside the launcher thread means exhausting the test process's descriptor limit, which would make the shared test runner flaky, and the worker source is an inline string with no seam to stub spawn. The guard is a one-character optional chain on a value Node documents as unset in that case.

@maria-rcks Understood. I accept the test stability trade-off and won’t pursue the test. The worker’s EMFILE and ENFILE paths remain untested.

[review] Worker16 changed-head review — Windows process-worker cleanup and launch-error handling
Reviewed exact current head: 93556fe.
Target at review: main@33f85846be8bc4bc592f0e621d6f0313e5abd4a4.
Author: SunkenInTime. Reviewer: GPT-6, separate nonauthor session. The PR body reports Claude Opus 5.5 as implementer; no self-review or distinct GitHub-principal approval is claimed.
P2 remains — failed taskkill still reports the entire process tree as terminated
The previous review's timing concern is partly addressed: SpawnWorker.fail() now waits for the taskkill child to emit error/exit before calling finish(). However, SpawnWorkerPool.killTree() still calls finish(false) both when taskkill emits error and when it exits nonzero. finish(false) then attempts process.kill(pid, "SIGKILL") on only the direct child, silently ignores that fallback's error, and unconditionally calls done(). The caller's finish callback consequently publishes child.exit(null, "SIGKILL") and child.abandon(), dropping the only remaining ownership record even though descendant termination was not established.
A direct-child TerminateProcess/SIGKILL fallback does not prove the Windows process tree was killed. If taskkill cannot start or fails (for example access denied or executable creation failure), descendants can remain alive while the spawner reports a terminal SIGKILL and releases the process scope. This is the same user-visible lifecycle invariant from the prior P2, not an unchanged line-level objection: the new callback delays the report but still finalizes it on the failure path.
Required: distinguish successful tree termination from failed/unverified cleanup. Keep the child/tree cleanup owned or surface an explicit unresolved cleanup failure; do not report whole-tree SIGKILL merely because the direct-child fallback was attempted. Add deterministic tests for taskkill spawn error and nonzero exit, including a descendant that remains alive until cleanup is actually confirmed.
P3 — the EMFILE/ENFILE guard has no original-scenario regression
The new optional chain at SpawnWorker.ts:92 correctly avoids dereferencing undefined child.stdio on a failed Node spawn. The current SpawnWorker.test.ts still has only four tests; its launch-error test covers a missing executable, not EMFILE/ENFILE with child.stdio unset. Add a focused regression proving that descriptor-exhaustion error is delivered as that command's failure, an already-running sibling remains alive, and a subsequent launch still succeeds. This is a coverage gap, not a claim that the one-line guard is wrong.
Exact-head validation and scope
Normal exact-head PR workflow #42343291421 completed SUCCESS at 93556fe. I read Test Server 1's log: it explicitly ran src/process/SpawnWorker.test.ts, 4/4 passed on Linux. The other Test Server shards, Test, Test Web, Release Smoke, Build and Typecheck, Rust, Lint and Check jobs completed successfully; Mobile Native Static Analysis was skipped. These results do not execute the Windows taskkill failure path or an EMFILE/ENFILE fixture. The PR author's Windows 11 measurements remain author-reported, not independently reproduced.
The PR still includes the parent #30209 Linux helper stack; the current automated out-of-scope review has flagged that stack. Maintainers must resolve the stacked-target/scope disposition separately. This review does not approve the Linux helper stack or the full PR.
Disposition: CHANGES_REQUESTED for the remaining P2. No source/branch write, CI dispatch, merge, packaged-desktop acceptance or Windows host acceptance was performed.

P2: finish(false) falls back to killing only the direct child, ignores failure, then done() publishes SIGKILL and abandons the child record. A failed/nonzero taskkill /T /F does not establish descendant termination. Keep cleanup ownership or report an explicit unresolved cleanup failure; add deterministic taskkill-error and nonzero-exit descendant regressions.

No change for the taskkill fallback. When taskkill fails, this matches what Effect's own Windows kill does on main today: terminateProcessGroup catches the failed taskkill, kills the direct child with childProcess.kill(), then reports the exit. Holding the exit open instead would leave the caller's exitCode and its scope finalizer waiting forever on a process nobody can signal.
This path only runs if a launcher thread dies, and the thread doesn't die on its own: spawn, write and kill errors are caught and returned per command (0ac0733 closes the last gap). No EMFILE test either, since a real EMFILE inside the launcher means exhausting the test runner's descriptors.
The Linux launcher from #30209 is no longer in this PR (a9549c1). The diff is now the Windows launcher, its tests, the Effect hook and the server wiring.
The reason will be displayed to describe this comment to others. Learn more.
P3 coverage gap: this optional chain handles child.stdio === undefined, but the current test suite only covers a missing executable. Add an EMFILE/ENFILE spawn-error regression proving an active sibling survives and the pool accepts a subsequent launch.
Correction — PR #32260 target branch advanced after the initial review readback
Review #5810584909 remains anchored to the verified source HEAD 93556fef0b20cf5b830e239bd7931301b1af75d2. Its body recorded the PR metadata's base_sha=33f85846be8bc4bc592f0e621d6f0313e5abd4a4; that was the PR's recorded base, not the current main tip at the later readback.
Fresh refs/heads/main read: 257e4073c1c51dcae291ecc7ca482dc66900f961 (2026-11-07T06:38:24Z). PR metadata still records base 33f85846..., so the candidate is 10 commits behind current main. Exact compare 33f85846... → 257e4073... is 10 ahead / 0 behind; its 76 changed paths contain neither apps/server/src/process/SpawnWorker.ts nor apps/server/src/process/SpawnWorker.test.ts. The source paths reviewed at head 93556fef... are therefore unchanged by this observed base-only delta.
Correct target for any current qualification/landing handoff:main@257e4073c1c51dcae291ecc7ca482dc66900f961, after the original author/landing owner performs the normal supported base synchronization and expected-head readback. No rebase, branch write, CI dispatch or merge was performed here. Base-only synchronization does not itself require a new full review under the repository's current delta policy; a changed SpawnWorker delta would require the related delta check.
Precise chronology correction — current main had advanced before review submission
One further precision: refs/heads/main commit 257e4073c1c51dcae291ecc7ca482dc66900f961 was committed at 2026-11-07T06:38:24Z, while review #5810584909 was submitted at 06:38:48Z. Thus the review's Target at review: main@33f85846... line was already stale at submission; it reflected the PR metadata's recorded base, not the branch tip. The correct target at review was main@257e4073....
The source finding remains valid for the exact reviewed PR head 93556fef...: the 10-commit base→current-main compare has no changes to SpawnWorker.ts or SpawnWorker.test.ts. This correction does not approve the PR, synchronize its branch, or change the CHANGES_REQUESTED findings. Original author/landing owner retains the base-sync decision and expected-head write.

Test the spawner selected by the server composition.
The launcher-thread test provides SpawnWorker.layer directly, so it cannot detect a regression to the stock spawner in the server composition. Extend that test to resolve the spawner through the server’s platform-services composition on Windows. Keep the direct worker-layer coverage on other platforms.

Done in fc010c9. The launcher-thread test now goes through SpawnWorker.layerServer, the layer server.ts provides, with the platform set to win32 and then linux: on win32 the server thread holds no ProcessWrap, on linux it does. It fails if the Windows selection falls back to the stock spawner.
@SunkenInTime Thanks for adding the platform-selection test.
The test provides SpawnWorker.layerServer directly at Line 43 of SpawnWorker.test.ts. It does not exercise layerPlatformServices, where the server also merges NodeServices.layer. The test could still pass if the server selected the stock spawner.
Please test through the same platform-services composition that the server uses. I inspected the code but did not run the tests.

Agreed that the selector test does not prove the composition, and your outside-diff comment showed why: NodeHttpServer.layer also outputs NodeServices, so the stock spawner won for every layer above the HTTP server. fc986f5 fixes that. A test through layerPlatformServices alone would still have passed against that bug, and covering it means building the whole server layer, which no test here does today. The proof for the wiring is a runtime log of the thread behind every child_process.spawn: 896 of 897 launches in a vp run dev session ran on a worker, and the one left is the Claude CLI, which calls node:child_process directly (details in the PR description).

@SunkenInTime Thanks for the clarification. You’re right: testing layerPlatformServices alone would not cover the NodeHttpServer.layer override.
The reported dev-session log gives evidence for the launches observed. It does not provide an automated regression test for the full server composition. I won’t insist on that test here.

Provide the worker spawner before constructing ProcessRunner.
On Windows, ProcessRunner.layer captures the CLI’s stock spawner before layerPlatformServices installs the worker spawner. Git launches through VcsProcess, including source-control discovery, can still block the RPC thread during slow process creation.

Worker16 changed-head review for the Windows process-launcher candidate.

Candidate and custody: T3 Code PR #32260, author SunkenInTime, author-owned fork branch SunkenInTime/t3code:t3code/windows-spawn-off-main, exact current branch/PR HEAD fc010c9 → recorded base main@33f85846be8bc4bc592f0e621d6f0313e5abd4a4. OPEN / UNMERGED. Current upstream main is 25bf84a; this candidate is 13 commits ahead / 37 behind that current main. This is a review-only delta; no branch/path custody or source write was taken.
Changed-head delta since the previous Worker16 review: 0ac073 removed the Linux helper stack and wired the Windows-only layer into server platform services; 0caa12 added relay-stream destruction forwarding and a stdin-EOF regression; fc010c changed the launcher-thread test to exercise SpawnWorker.layerServer with explicit win32/linux platform selection. The earlier stdin-destroy finding is resolved by the new destroy protocol and regression. The earlier missing production-layer-selection test is also materially addressed by the fc010c test. I am not repeating either as an open finding.
P2 — failed taskkill still drops process-tree ownership after killing only the direct child
At SpawnWorker.ts:459-461, taskkill error/nonzero calls finish(false), which only invokes process.kill(pid, "SIGKILL") and then unconditionally calls done(). On Windows that terminates the direct process, not its descendants. SpawnWorker.fail() then synthesizes child.exit(null, "SIGKILL"), abandons the streams and clears the worker's child map. A child that created a grandchild can therefore leave that descendant running and no longer tracked, even though the fail path/comment and PR description say the process tree is ended before reporting exit.
I read the author's response that this matches Effect's existing direct-child fallback. That explains the chosen fallback but does not establish that this new worker-failure path has terminated descendants or retained ownership of unresolved cleanup. Please either provide a supported fallback that verifies tree termination, or represent the failed tree cleanup as uncertain and retain/report the outstanding ownership rather than treating it as a completed tree kill. Add a deterministic regression for taskkill spawn failure/nonzero with a child plus grandchild; this does not require exhausting real runner file descriptors.
Current verification and limits
PR workflow #42403696871 was still running when checked. Build and Typecheck, Lint, Rust and Release Smoke had completed successfully; Test Server shards, Test Web and the Test job were still in progress. The preceding head 0caa12's run #42403318875 completed successfully and its Test Server 8 log showed SpawnWorker.test.ts 4/4 PASS, including the stdin-EOF case; that does not validate the new fc010c layer-selection test. The author reports Windows 11 host measurements, but I did not independently operate a Windows host or packaged desktop in this run. Packaged desktop and current-main composition remain NOT RUN here.
The competing Windows approach #32234 and Linux companion #30209 remain separate candidates; maintainers must select/combine them without overlapping source ownership. No merge, base synchronization, CI dispatch, host/service restart, install, deployment or device action was performed.
Disposition: REQUEST_CHANGES at exact HEAD fc010c9. This is a separate nonauthor GitHub-principal review under the temporary #1899 same-family allowance, not an approval or landing authorization.

P2 — this fallback kills only the direct child, then reports cleanup complete. If taskkill errors or exits nonzero, process.kill(pid, SIGKILL) does not terminate Windows descendants. The caller then invokes done(), so SpawnWorker.fail() emits a synthetic SIGKILL, abandons streams and clears the child map. A child that started a grandchild can leave it running and unowned. Please add a deterministic taskkill-failure/nonzero regression with a child+grandchild and either a supported tree-termination fallback or explicit retained/uncertain cleanup ownership; do not report the whole tree ended when only the direct child was attempted.

Measured on Windows 11 instead of argued. The harness drives the real layer and counts every launcher thread error and exit the pool did not request. The grandchild is started detached, leaving libuv's kill-on-close job like a git or cmd grandchild, so only a tree kill can end it.
Check
A launcher thread does not die from anything a command does. When one is killed anyway, the whole tree ends before the exit is reported, and the exit is a failure, not a clean result. The remaining case needs taskkill itself to be unable to start, and there this PR leaves exactly what main's Effect spawner leaves. The same table is in the PR description.
NodeHttpServer.layer also outputs NodeServices, so Node's own spawner

the outermost ProcessRunner was built before the server's spawner.
Claude Opus 5.5 via Claude Code
Confirmed, and it went further than ProcessRunner. Logging the thread of every child_process.spawn in a running server showed all launches on the server thread: NodeHttpServer.layer also outputs NodeServices, so Node's own spawner replaced the worker spawner for every layer above the HTTP server, and the outermost ProcessRunner was built from the CLI's spawner. Fixed in fc986f5: one layerSpawner follows both NodeServices outputs and is provided to the outer ProcessRunner. The same run now shows 10 of 11 launches on a worker; the remaining one is the Claude CLI, which uses node:child_process directly.

Worker16 changed-head technical review — COMMENTED / NOT APPROVED
Candidate: T3 Code PR #32260, original author SunkenInTime, author-owned branch t3code/windows-spawn-off-main, exact HEAD 6877d5e0a15525a016521e13f01b633e7fb33178 → recorded PR base main@33f85846be8bc4bc592f0e621d6f0313e5abd4a4. Current upstream main is 25bf84ab07f53f141c20239c63e383823a9d5ea0; compare is 17 ahead / 37 behind current main. OPEN / READY / UNMERGED. Author retains all six changed paths and branch. This is a separate nonauthor same-family review under the temporary #1899 allowance, not a distinct-principal or maintainer approval.
Corrections to the previous Worker16 review
The taskkill-fallback P2 is withdrawn as a regression finding. The source still has a real limitation: if Windows taskkill /T /F cannot start, killTree can only fall back to terminating the direct PID, so a detached grandchild can remain. I checked the pinned Effect 4.1.2 baseline: its NodeChildProcessSpawner also delegates process-tree cleanup to taskkill and ignores cleanup errors on scope exit. The author's new Windows 11 harness reports the same grandchild-left-running result for baseline and this PR when taskkill cannot start (PATH without System32), while the normal taskkill path succeeded 200/200. This is an inherited best-effort limitation, not a new regression introduced by this candidate; I do not keep it as a blocking P2. The limitation should remain explicit.
The CodeRabbit ProcessRunner wiring finding is resolved on the current head. Commit fc986f5c78e900743649e5e2413939c8467efe5c adds layerSpawner as the outer provider after ProcessRunner.layer, so the worker spawner is available while the composed layer is built. I traced the pinned Effect 4.1.2 Layer.provide semantics (dependency layer built first, its context provided to the composed layer) and the updated server composition. The author also reports a running-server trace with 896/897 observed launches on workers, the remaining launch being Claude's direct node:child_process call. I do not uphold the earlier concern that ProcessRunner necessarily captures the stock spawner.
Current implementation and evidence
The bounded source now has:
a Windows-only worker-backed shared ChildProcessSpawner;
relay handling for stdin/stdout/stderr/additional descriptors, backpressure and destroy;
server-layer wiring that follows both NodeServices and NodeHttpServer spawner outputs;
worker-error/exit cleanup that attempts process-tree termination before reporting a failed child exit.
Exact-head CI: workflow #42405848342, completed SUCCESS. The committed SpawnWorker.test.ts has four tests covering launcher-thread selection, stdio/env/cwd/args/additional descriptors, launch errors/exit codes, and explicit kill/scope interruption. The PR description reports 4/4 on Windows.
The author additionally reports a Windows 11 harness: 275 hostile commands plus 300 concurrent launches with zero unexpected launcher errors/exits; 50 worker-termination runs with child and detached grandchild both gone; and 200 successful taskkill /T /F runs. I did not execute that host harness myself.
Nonblocking qualification note: the committed four-test suite does not itself terminate a launcher thread while a child and detached grandchild are active. The author’s repeated Windows harness is useful evidence, but a reproducible Windows regression for that worker-failure branch would make the recovery guarantee durable in CI. I recommend adding it when practical; I am not retaining a P2 solely for this test gap given the reported repeated native evidence.
Dependencies and remaining gates
#32260 competes with T3 PR #32234 for the same Windows RPC-spawn issue; maintainers must choose one implementation, not merge both.
The shared Effect patch overlaps Linux counterpart #30209; whichever lands second must reconcile/drop its duplicate patch as described in the PR.
Candidate is 37 commits behind current main; original author/maintainers own any base-forward work.
No packaged desktop build, macOS acceptance, or Windows user-session service acceptance is claimed here.
Disposition: prior taskkill P2 withdrawn as non-regression; ProcessRunner wiring concern resolved; no new P1/P2 demonstrated in the bounded current-head review. COMMENTED, not APPROVED. No source, branch, service, host, or device was changed.
Worker16 review-metadata correction (not a new review or source finding): the PR's own attribution identifies Claude Opus 5.5 as implementer and GPT-6 Astra as an earlier reviewer. This session's reviewer is GPT-6, so this review is opposite-family relative to the stated implementer; it does not rely on the temporary same-family exception. The earlier reviewer and this reviewer are same-family, and neither is a distinct-principal/maintainer approval.

Worker16 changed-head review of the additive environment-snapshot delta at exact HEAD 2004169cb9a4b865b15561316b8e371fe2d732fe → recorded PR base 33f85846be8bc4bc592f0e621d6f0313e5abd4a4 (current main 25bf84ab07f53f141c20239c63e383823a9d5ea0; candidate 18 ahead / 37 behind). Disposition: COMMENTED / NOT APPROVED. This review is limited to the one new commit and two changed paths; it does not repeat the previous six-file review.
The source change fills options.env with a snapshot of the server process environment only when the Effect spawner supplied no explicit environment. This addresses the worker-thread semantic difference: a worker's own process.env is captured when that worker starts, while a later child launch from the server should inherit the server's environment at the time spawn is called. The new regression starts/reuses the worker pool, sets SPAWN_WORKER_LATE afterward, launches another child with no explicit env, and asserts the child receives the new value. The existing explicit env + extendEnv path remains intact. I found no demonstrated P1/P2 defect in this two-file delta.
Exact-head validation read: normal automatic Actions #42409365200 completed SUCCESS. In Test Server 8, the log explicitly ran src/process/SpawnWorker.test.ts: 4 tests passed / 0 failed at this exact head. This is CI fixture evidence, not a packaged desktop build or host acceptance.
Prior finding disposition preserved: the earlier taskkill-failure/direct-child fallback remains a documented best-effort limitation inherited from the pinned Effect 4.1.2 baseline; this new commit does not change it, and I do not re-open it as a candidate regression. The author-reported Windows 11 stress/child-grandchild harness is not independently executed by this reviewer.
The PR remains 37 commits behind current main and competes with #32234 for the shared Effect spawner hook; maintainer selection and current-main composition are still needed. Original author retains all source paths and branch. No source custody, CI dispatch, approval, merge, host/device action or installation is taken.

Problem
Fixes #25245. On Windows,
child_process.spawn()runs CreateProcess on the calling thread. Defender, endpoint security, a busy disk or a bloated kernel token table can hold that call for 0.5 to 10 s, and the server launches git, gh, netstat and provider CLIs on its RPC thread. Each slow launch freezes RPC and heartbeats for that long; past 15 s the client drops to "Connecting".Change
On Windows the shared Effect
ChildProcessSpawnernow launches throughapps/server/src/process/SpawnWorker.ts, a pool of worker threads:postMessage, with backpressure.taskkill(process-tree kill on interrupt or timeout) goes through the same launcher instead ofexecFileon the server thread. That is the one change to the Effect patch. It plugs into
NodeChildProcessSpawner.layerWithSpawn, a small hook this PR adds to the Effect patch. #30209 adds the same hook for its Linux launcher, so whichever lands second drops its copy of the patch and both platforms share it.Because it sits in the shared spawner, it covers every launch that goes through Effect:
processRunner,GitVcsDriverCore,VcsProcess, Codex, the ACP providers, OpenCode, the port scanner. #32234 targets the same issue with a worker per command, wired intoprocessRunnerandGitVcsDriverCoreonly, so provider launches through the spawner stay on the RPC thread there. Maintainers should pick one; I'd close whichever doesn't land. Not covered: launches that call
node:child_processdirectly (the Claude CLI inClaudeAdapterV2.ts, speech, document text) and node-pty. macOS and Linux keep the stock spawner; #30209 covers Linux.Scope and approval
A focused fix for the triaged bug #25245, no behavior or setting changes. It is the Windows half of the direction #30209 takes for Linux.

With healthy launches: 29 ms to 6 ms, total 318 to 372 ms.Which thread launches each command in the running server (found in review): a preload that logs
isMainThreadfor everychild_process.spawn, 45 s oft3 --auto-bootstrap-project-from-cwdon this repo. Before the last commit all 11 launches ran on the server thread, git included:NodeHttpServer.layeralso outputsNodeServices, so Node's own spawner replaced the worker one for every layer above the HTTP server. After it, 10 of 11 run on a worker: git, codex --version,codex app-server,claude --version,npm viewand taskkill. The one left is the Claude CLI, which callsnode:child_processdirectly.Then
vp run devwith the same preload, seeded with real threads: a Replay turn with approval, the diff panel listing this branch againstorigin/main, and a terminal running a local server all worked. The server made 897 launches; 896 ran on a worker, and the one on the server thread was the Claude CLI. An earlier 2-minute A/B in the dev app came out within noise; it predates the wiring fix, so its launches never left the server thread, and I've dropped it.A launcher thread dying (raised in review), measured on Windows 11 with a harness that drives the real layer and counts every launcher thread's errors and exits. The grandchild is started
detached, so it leaves libuv's kill-on-close job the way a git or cmd grandchild would, and only a tree kill can end it.taskkill /T /Fagainst our own running child with a grandchild, 200 timestaskkillcannot start (PATH without System32), the only way it failed heremain's Effect spawner and this PR both end the child and leave the grandchild, identicallySo a launcher thread does not die from anything a command does; when one is killed anyway, the whole tree ends before the exit is reported; and the remaining case needs
taskkillitself to be missing, where this PR behaves exactly likemain.Tests:
vp test run apps/server/src/process/SpawnWorker.test.ts, 4 passed on Windows. They run on every platform, since the worker layer works anywhere. "runs children on a launcher thread" checks that the server thread holds noProcessWrapwhile a child runs; it fails against the stock spawner.vp run check:changedpasses. Not checked: a packaged desktop build (the SQLite reader worker already runs inline worker source the same way in packaged builds), and macOS.
