fix(server): poll provider usage only while visible - #31032
Excessive usage polling was hitting provider rate limits, especially when the same account was used across multiple computers. Background health checks and overlapping refreshes requested quotas even without a visible usage view, multiplying requests and sometimes replacing valid readings with errors.
Change
Quota reads now follow visible web/desktop and mobile usage surfaces at the existing five-minute cadence. Health checks omit them, and periodic reads stop when the surface is hidden, the app is backgrounded or the connection closes. Model fallback reads quotas on demand; widgets refresh on connect/resume. Hubs retain a startup discovery read.
Overlapping refreshes share an account-scoped cache within each T3 server. Manual refreshes respect cooldowns, failed probes retain the last good reading, and reset credits require explicit redemption.
We chose this demand-driven approach because it removes unnecessary requests and reuses existing provider refresh and cache paths. Clients connected to the same server share results; independent hosts keep separate caches. This avoids introducing a cross-machine coordination service.
Scope and approval
The triaged bug covers hub pacing and retained readings. This PR also changes native providers and clients; maintainer approval for that broader scope is pending.
Verification
Published head: 68c32c9eb1.
Current-head CI: build/typecheck, lint and all scheduled test jobs passed; six check runs were skipped. CodeRabbit reported no actionable code findings. Codex review failed twice with a sampling error.
Local check:changed: 83 tests and all 34 affected package typechecks passed, along with fast checks. Regressions cover credential changes, cooldown expiry and fallback eligibility.
Grok KISS audit addressed: unused pending-refresh mode removed; fallback reads stop at the first usable candidate. Other proposals have source/test-backed replies.
Limits
Older clients retain legacy polling. Claude SDK failures without response headers use a five-minute cache lifetime. Quota-driven selection waits at most five seconds. Current web/desktop and mobile interaction is unverified; earlier browser automation timed out.

Usage-limit readings now include stale-state and refresh-deadline metadata. Provider refreshes can target selected instances and drivers. Web and mobile clients coordinate refreshes by account identity and deadlines. New-thread, draft-send, project-launch, and text-generation flows refresh usage before resolving model selections.

Provider reads use polling controls, account refresh keys, and usage-client caching. Registry and RPC paths support filtered usage refreshes and report server time. Usage sources retain cached accounts after failures and support provider filtering.
Clients coordinate refreshes by account resources and connection generation, schedule checks from deadlines, and refresh usage in visible usage views. Usage displays add stale-reading notices.
New-thread, draft-send, project-launch, and text-generation paths refresh usage before resolving model selections. Bounded waits and pending state are added where applicable.

Remove the unreachable usage-recovery branch in refreshOnInterval.
refreshOnInterval has one call site, at line 380. That call runs inside the loop forked at line 390 under Effect.provideService(UsagePolling, false). No code on that path sets UsagePolling to true, and the function is not exported. The guard at line 295 therefore always takes its early return. Lines 301-310 never run.
shouldRefreshOnInterval still checks needsUsageRecovery at line 319. For a probe-failed snapshot that needs no provider refresh, each interval tick can wake the loop, check host demand, and take the refresh semaphore before refreshOnInterval returns without work. Removing that clause stops these wakeups. Usage recovery continues through refreshUsage, which re-probes a probe-failed reading after the five-minute deadline at line 165 has passed or when a reset is requested.

Initial inline findings are addressed in 7745561 and f7c1571, with a reply and resolution on each thread. The suggested Retry-After ceiling was declined with reasoning: an arbitrary shorter fallback would retry before the upstream cooldown expires.
The top-level CodeRabbit maintainability finding is also fixed: the unreachable periodic usage-recovery branch and its no-op wakeups are removed. Usage recovery remains demand-driven through refreshUsage.
Validation: 332 changed-file tests, affected-package type checks, lint/knip/GraphQL, four browser visibility tests, and React compiler checks pass. The final import-order format correction passed its targeted check and commit hooks. Linux CI still needs to exercise the Unix CLI account-switch/reset fixtures skipped on Windows; native mobile interaction and full live-client automation remain unverified as described in the PR.
The three f7c1571 findings are fixed in 934fba4, with replies and resolved threads. The regressions failed before the fix. Required changed-file checks now pass, including 333 tests and type checks for all 36 affected packages; the five browser hook tests cover visibility and a delayed initial read. New-head CI is pending.

Honor Retry-After for retryable 5xx usage responses.
When a usage refresh receives a CLIProxy 5xx response with a Retry-After header, this branch returns the error without setting the account cooldown. The five-minute usage cache can then retry the request before the upstream delay expires. Apply the same 5xx and Retry-After rule used by makeUsageHttpClient.

Scope the Claude capability cache by the current account before reading it.
resolveCapabilities keys the cache only by includeUsage, then checks for an account change after the cache read. If Claude switches accounts while the entry is valid, the health check or usage refresh can return the prior account's email, workspaceId, and usage. lastAccount is then updated with that same cached value, so the invalidation branch does not run.
Use the current Claude account identity in the cache key before Cache.get. Keep includeUsage as a separate cache dimension. This preserves the five-minute cache while preventing one account from reusing another account's capabilities.

Do not cache the stale-account fallback.
When a probe fails, the fallback reuses the previous account's email and capabilities. It can match the current workspace ID, including when both IDs are absent. This PR gives that fallback the full CAPABILITIES_PROBE_TTL. Until the TTL expires, the cache keeps the previous account's identity, and the account-change guard in resolveCapabilities cannot detect the change. The merge-base code retried fallbacks with zero TTL.

Mark retained accounts stale when a whole-source read fails.
When api.readAccounts fails, the branch at UsageLimitSources.ts:92-98 returns previously successful accounts from lastGoodSources. It does not set their stale flag. The shared usage contract shows the stale notice only when account.limits.stale is set, so the dashboard shows the old percentages without a "last reading" notice. The only notice is the source error, such as Accounts: token expired. The PR states that successful readings are retained with a stale notice. This path does not provide that notice.
The retained accounts can be marked stale in this branch while the source error is kept. Accounts that are already stale remain stale.

The two 934fba4 review findings are fixed in f9b4f40, with replies and resolved threads. Pooled composer demand now refreshes all native accounts of the rendered driver and matching hub accounts. Native cached responses preserve their original sample times through the quota readers.
The regressions failed before the fixes. Required changed-file checks pass: 358 tests, type checks for 36 affected packages, lint/knip/format/GraphQL, and compiler checks. The focused cache/registry suites pass 58 tests; native reader suites pass 38 tests with two platform skips. Latest-head CI is pending.
Hub usage now applies the account cooldown to 5xx responses carrying Retry-After, using the existing case-insensitive header lookup and delay parser. The cooldown regression covers both 429 and 503; the 503 row failed before this fix.
Claude reads the existing local account identity before cache lookup and keys entries by identity plus includeUsage. Account changes clear the previous fallback and both capability entries. Unrelated config writes do not change the identity. The account-switch fixture no longer explicitly invalidates the cache; cross-platform config tests cover same-organization account changes. The Unix CLI fixture awaits Linux CI.
Kept the five-minute failed-probe cache intentionally. Restoring zero TTL would violate the requested failed-probe backoff and permit repeated SDK probes during an outage. The identity fix above clears the old fallback before lookup when the local account changes; same-account fallback omits usage. This addresses the identity risk without increasing polling after failures.
Whole-source failures retain windows and sample timestamps but mark usageLimits.stale, the actual contract field. The stale marker is saved in the retained source state so a later refresh of another provider cannot erase it. The regression covers both the source failure and subsequent filtered refresh.
Validation: focused suites passed 47 tests with four platform skips before the additional identity assertions. The final vp run check:changed passed format, lint, both knip checks, GraphQL generation, type checks for 36 affected packages, tests across 42 files, and React compiler checks. No new live-client interaction was claimed.
The previously reported workflow runs have settled: relay, EAS and web previews were skipped; Mobile Fingerprint Check succeeded. CI and review for this new commit are pending.

@ElliotDrel I re-evaluated all four findings against 0e899b9772. Findings 1, 2, and 4 are addressed. I withdraw the zero-TTL recommendation in finding 3.
Hub Retry-After cooldown — addressed. apps/server/src/usage/cliproxyApi.ts, Line 278–289, uses a case-insensitive header lookup and applies the account cooldown to both 429 responses and 5xx responses with Retry-After. The parameterized regression covers 429 and 503. It checks that no new upstream usage request occurs five minutes into a ten-minute cooldown.
Claude account-scoped cache — addressed. apps/server/src/provider/Drivers/ClaudeDriver.ts, Line 225–247, reads the local account identity before Cache.get. The cache key includes that identity and includeUsage. An identity change clears both cache entries and the previous fallback. The tests cover same-organization identity changes, unrelated config writes, and an organization switch without explicit cache invalidation.
Five-minute failed-probe TTL — retain it.
My zero-TTL recommendation addressed the identity risk through the wrong mechanism. The identity check now clears the old fallback before lookup. For the same identity, the fallback removes usage data. Retaining the five-minute TTL preserves failed-probe backoff and prevents repeated SDK probes during an outage. The regression also checks that a repeated fallback read does not start another probe.
Whole-source stale state — addressed. apps/server/src/usage/UsageLimitSources.ts, Line 92–108, sets the correct field, usageLimits.stale, and saves that state in lastGoodSources. It preserves the existing windows and sample timestamps. The regression checks that a later refresh for another provider does not remove the retained stale marker. My original suggested patch used the wrong field name, limits.
Verification scope: I inspected the source and regression assertions. I did not run repository tests. Current build, lint, and test jobs remain pending. Wait for latest-head Linux CI to confirm the Unix fixtures. Native mobile interaction and full live-client automation remain unverified.

Learning: In apps/server/src/provider/Drivers/ClaudeDriver.ts, Claude workspace identity comes from oauthAccount.organizationUuid in .claude.json through the organization-ID reader in apps/server/src/provider/claudeResetCredits.ts. It can become available after an earlier probe lacked it, independently of a successful usage read. In apps/server/src/provider/CodexProvider.ts, Codex workspace identity comes from accountId in the usage response itself, so a failed usage read does not supply a new workspace ID.

Elliot asked for all-provider, demand-driven usage, with the smallest design that still isolates accounts, keeps the last good sample, and honors a Retry-After T3 can actually see. That core is here and should stay: health probes skip quotas, a visible surface opts in, usageHttpClient caches by account, and managedProvider.usageRefreshAfter skips another CLI or SDK probe inside the server deadline. Hub readings have one owner in UsageLimitSources. I am not treating the all-provider scope as something to revert, and I am not treating the PR body's note that maintainer acceptance is still outstanding as an approval.
Must-fix
MCP launch is the one preflight with no deadline. Composer and text generation give up after 5 seconds and continue with the snapshots they have. handlers.ts awaits refreshUsageForModelSelection until the probe returns. A cold or stuck provider read now blocks thread creation.
Delete
afterPending. Main 4c3a9d5818 (#32448) removed the only caller, mobile refreshAfterEnable, when the Cursor Keychain path went away. No production call passes true.
The second client scheduler. useLiveRefresh and the mobile 30-second floor both exist to stop refreshUsageLimits from immediately retrying a deadline that is already past. One floor inside refreshUsageLimits covers web and mobile.
retainedUsageLimits. It is a private second copy of a sample so a health-only marker change can hide the bars and restore them. OpenCode's health hash covers every stored key, while the quota hash is the selected key. Hash the selected key, then the private copy has nothing to repair.
Simplify
Text generation, once the selected model is exhausted, quota-checks every eligible provider and then still picks the first match in driver order. Walk that order and stop at the first usable candidate. refreshNewThreadUsage and refreshUsageForModelSelection already stop after one fallback.
Optional
serverTime plus absolute refreshAfter is a clock sync so a skewed client can honor the server epoch. The server gate already refuses an early provider call. A client can schedule from its own completion time, capped at 5 minutes. Dropping the field loses skew alignment, not account isolation or Retry-After.
Driver tests that pin a real quirk should stay, including the Claude local-account regression on this head. The afterPending case and a second copy of the generic ownership matrix can go when the code they pin goes. I did not re-file the existing Codex threads on native Codex ownership or synthesized 429 expiry.

Addressed the first-principles review: removed the unused pending-refresh mode and changed text generation to stop quota reads at the first usable fallback. Published head: e0d829f311689d575ab42b9cf2c0c0f8139f3f68.
The other inline proposals have source/test-backed replies and are resolved. MCP already has a five-second service bound. OpenCode can use a fallback credential after 401/403, so its first key cannot replace owned-sample reconciliation. The client schedulers preserve visible periodic refresh, immediate overdue rechecks and remount coordination.
Keeping optional clock normalization: scheduling five minutes from a cached read can postpone the server's already-near deadline, and a skewed client can otherwise repeatedly arrive just before expiry. The existing remote-client and skew cases verify this timing. The server gate bounds provider calls but does not schedule the client's next useful read. Kept adapter identity tests and managed ownership-policy tests because they exercise different boundaries; removed only the obsolete afterPending assertions.
Validation passed 93 tests, seven compiler checks and all 34 affected typechecks, plus formatting, lint, dependency checks and GraphQL generation. Existing MCP deadline and OpenCode fallback-key tests also passed. New-head CI/reviews are pending. Broader scope approval and current client-interaction evidence remain outstanding.

Please review latest head a5ed779d68; the summary still covers e0d829f311. The two native Codex ownership findings are fixed with regression coverage. Broader-scope approval and current live-client verification remain explicitly pending in the description.

Please review current head 8eed9b4584, including the hub retry-deadline fix. The completed e0d829f review predates that fix and the native Codex ownership fixes. The description continues to identify broader-scope approval and current live-client verification as pending; neither is claimed complete.

@coderabbitai The summary's all-providers-hidden periodic-refresh item does not match current source. UsagePage.tsx enables the Limits timer only when PROVIDER_ORDER.some((provider) => !hiddenProviders.has(provider)); useLiveRefresh.ts returns without scheduling when disabled. No source change is needed for that item. Please clear it from the pending-risk summary in the already-requested current-head review.
@ElliotDrel You’re right. I withdraw the all-providers-hidden periodic-refresh item for 8eed9b4584.
UsagePage.tsx, Line 386–390, disables automatic Limits refresh when every provider is hidden. useLiveRefresh.ts, Line 114 and Line 245–252, prevents scheduling when disabled and clears the previous timer. No source change is needed for this item.
I’ll request a summary update with that correction. Earlier regeneration attempts left the summary unchanged, so removal is not yet confirmed. Broader-scope maintainer approval and current live-client interaction remain unverified.
Action performed
Summary regeneration completed, but the summary was unchanged.
Please review current head 68c32c9eb1, which fixes the two latest Codex findings. Local changed-file checks passed; broader-scope approval and current live-client verification remain pending.

@juliusmarminge Requesting your review and a scope decision on 68c32c9eb1, following your hub-polling triage. This also changes native-provider and client polling, so broader-scope approval is still needed.
Build/typecheck, lint and scheduled tests passed; all review threads are resolved, and CodeRabbit reports no actionable code findings. The KISS pass removed the unused pending-refresh path and stopped fallback probes at the first usable candidate. Latest local checks passed 83 tests and 34 package typechecks.
Current web/desktop and mobile interaction remains unverified; the description labels the screenshots as historical. Codex's latest review failed twice with a sampling error, so there is no completed latest-head Codex review. These gaps remain explicit; this is a request for maintainer review, not a claim that merge approval is complete.

Problem
Published head:
68c32c9eb1.check:changed: 83 tests and all 34 affected package typechecks passed, along with fast checks. Regressions cover credential changes, cooldown expiry and fallback eligibility.Limits
Older clients retain legacy polling. Claude SDK failures without response headers use a five-minute cache lifetime. Quota-driven selection waits at most five seconds. Current web/desktop and mobile interaction is unverified; earlier browser automation timed out.
