fix(server): a policy that manages observability endpoints is refused instead of ignored - #30973
A managed policy can set or lock observability.otlpTracesUrl, otlpMetricsUrl or otlpLogsUrl. t3 policy check accepts it, the server lists the path in lockedSettings, and the policy looks enforced, but the server never uses the policy value. resolveServerConfig (apps/server/src/cli/config.ts) reads the endpoints straight from settings.json through parsePersistedServerObservabilitySettings before ManagedPolicy loads. Those config values then drive the OTLP tracer, metrics and logger (observability/Observability.ts, serverLogger.ts) and the browser trace proxy (http.ts). Even with that ordering changed, T3CODE_OTLP_*_URL and OTEL_* variables outrank the setting (resolveSignalEndpoint in packages/shared/src/otelEnvironment.ts), and the desktop process reads the same file for its own exporter.
Repro through the real startup path: a policy locking observability.otlpTracesUrl to "" and observability.otlpLogsUrl to https://otel.example.com/v1/logs, plus a user settings.json with otlpTracesUrl: "https://user-collector.example/v1/traces":
Traces still go to the user's collector, and the organization's logs collector is never used.
Change
makeManagedSettings now refuses any observability key, with a reason that says why. This follows the existing rule for policies the server cannot enforce: the server refuses to start rather than look managed when it isn't. That rule already covers keys like projectSettingsOverrides, compactIdleThreads, and a storageCleanup.worktree* lock without worktreeCleanup. t3 policy check reports the same reason. The managed-policy guide gets a bullet under "A few settings need care".
Actually enforcing OTLP endpoints from a policy would mean changing environment-variable precedence, the server's startup order, and the desktop's own exporter. That is a feature, not this fix.
Scope and approval
This is a small fix for an obvious bug. The policy docs promise that T3 Code refuses to start rather than run with a policy it cannot fully enforce, and this key was silently unenforced. The change adds one guard next to the existing refusals, one test case, and one docs bullet. It has no UI change.
Verification
Extended refuses files it cannot fully enforce in apps/server/src/policy/ManagedPolicy.test.ts with a policy that locks observability.otlpTracesUrl.
Before the fix: FAIL src/policy/ManagedPolicy.test.ts > ManagedPolicy.load > refuses files it cannot fully enforce. The policy loads, with lockedSettings: ['observability.otlpTracesUrl'].
After the fix: ManagedPolicy.test.ts, managedLimits.test.ts and cli/policy.test.ts all pass (19 tests).
vp lint and vp fmt --check on apps/server/src/policy and docs/user, plus tsc --noEmit in apps/server, are clean.
Not checked: a real desktop start with a deployed policy. The refusal goes through the same ManagedPolicyFileError path as the other refused keys.
Proved with a test that fails on main and passes with the fix, plus a run through the real server-config path showing the locked URL was ignored.

Macroscope's review found this PR approvable — Policies attempting to manage observability endpoints now fail explicitly instead of appearing enforced. The restriction is narrowly scoped and documented; affected administrators must remove the unsupported entry before startup.

Walkthrough
Managed policy validation now rejects settings headed by observability. The rejection reason identifies startup sources that policy cannot override. The test covers this rejection, and the user documentation describes the restriction.

Summary
Reject and document observability settings apps/server/src/policy/managedSettings.ts, apps/server/src/policy/ManagedPolicy.test.ts, docs/user/managed-policy.md
The validator rejects policy settings headed by observability. The test checks the rejection reason, and the documentation identifies environment variables, the settings file, and desktop startup input as sources for OTLP endpoints.

ran node apps/server/src/bin.ts policy check /tmp/policy-observability.json with a locked observability.otlpTracesUrl; the running PR code refused it with the expected ManagedPolicyFileError and the reason that the value is resolved from environment, settings, and desktop bootstrap input.

I did not restart the server with an installed managed policy or exercise the desktop-only bootstrap input. The CLI validator and the shared startup policy loader path were exercised; the desktop shell itself was not available on this Linux VM.
