When Codex reports a terminal responseStreamDisconnected, responseStreamConnectionFailed, or httpConnectionFailed with no HTTP response status, T3 files it as provider_error. The same network failure is a transport_error while Codex is retrying it. Both a non-retrying error notification and a completion-only error lose the transport classification.
Change
Classify those structured connection errors without an HTTP status as transport_error in both terminal paths. Preserve existing usage-limit precedence; a completion-only HTTP 503 control still yields provider_error. Explicit completion evidence determines the terminal class; earlier notification details are reused only when the classes agree. Missing completion metadata, including retry exhaustion without an HTTP status, retains the notification's richer evidence. Do not change retries, continuation, quota classification or provider selection.
Scope and approval
This is a small focused obvious-bug correction to terminal failure categories. No new contract or setting is needed. The terminal event already reaches all clients and delegated runs.
Related open #30376 fixes HTTP 429 usage-limit classification in these variants, and #26532 adjusts exhausted-429 wording. This patch handles connection failures with no HTTP status; it does not duplicate either change. Merged #25611 already handles explicit capacity refusals and needs no replacement PR.
Verification
Extend the existing Codex terminal-evidence replay table with disconnection notifications, completion-only disconnections, both other connection variants, and an explicit HTTP 503 control. Four network cases fail on upstream main with provider_error, while the HTTP control passes. Seven additional regression cases reproduce stale notification-class reuse before the correction: both HTTP-status directions for all three connection variants and a retrying-notification case. The expanded existing HTTP-status table also checks that matching-class notification details survive. With the correction, all cases pass through the real Codex app-server replay adapter.

Implemented with GPT-6.1 Sol through the Codex harness in T3 Code.
Macroscope's review found this PR approvable — Codex terminal connection failures without an HTTP status retain the transport-error category instead of becoming provider errors. Usage-limit handling, retries and terminal lifecycle behavior remain unchanged, limiting the change’s risk.

Some terminal connection failures may still be reported as provider errors. The remaining classification gaps are bounded and can be addressed with owner awareness.

Require class equality before reusing the notification failure.
When a matching notification and completion have different httpStatusCode values, the current condition can emit the notification's stale transport_error or provider_error class instead of the later completion's status-sensitive class. This misclassifies the terminal event, although the inspected queue-hold path treats both classes the same.

Confirmed the class-reuse finding with replay tests: seven cases fail before the correction, covering both HTTP-status directions for all three connection variants and a retrying-notification case. Explicit completion evidence now prevents reuse of a differently classified notification failure. I preserved richer notification evidence when the completion omits error metadata or retry-exhaustion HTTP status; unconditional class equality would regress the existing 429-to-missing-status cases. A matching-class control still retains the detailed notification message.

cut an established upstream connection after forwarding real response bytes. codex 0.164.0 reports structured no-status retries, then generic other terminal errors. main and the original head lost the transport class. the follow-up retains it only when the raw terminal text exactly matches the no-status retry details; different messages and http-status errors keep their own classification. newer terminal status also takes precedence over older retry evidence, while statusless retry exhaustion preserves matching transport evidence.

merged current main (67eebaca23) with a merge commit. two rows from #25341 needed reconciling with this PR:
an other completion no longer counts as class evidence, so a matching ChatGPT sharing notification keeps usage_limit (chatgpt-workspace-limit).
sharing-code-mention now expects transport_error: a statusless terminal disconnect is transport, and prose naming a sharing code still does not make it a usage limit.
also fixed two CodeRabbit findings: blank retry details fall back to the native message, and a non-retrying transport notification keeps its class when the completion repeats it as other. one row each in the existing table. check:changed through blacksmith passed format, lint, knip, typecheck, and 169 tests. the live codex run above was not repeated on this head.
