fix(server): a failed Antigravity checkpoint no longer bricks the thread - #30443
When an Antigravity context-summarization checkpoint fails once (for example, expired Agent Platform credentials, invalid_rapt), the runtime stores the failed step and replays it on every later prompt to that native session: agent executor error: context summarization: checkpoint step 841 failed: ... CORTEX_STEP_TYPE_CHECKPOINT .... T3 classified this as an ordinary, reusable failure and kept resuming the same native session, so the thread failed on every message even after the credentials were fixed. Fresh threads worked.
Change
AntigravityTurnOutcome recognises the context summarization: checkpoint step <n> failed error step and reports the native thread as broken. A checkpoint that failed on quota keeps its usage_limit class and reset time, so usage-limit recovery still resumes it. Any other checkpoint failure reads "Antigravity could not summarize this conversation, so its saved session can no longer continue. The next message starts a fresh Antigravity session with this thread's context."
The shared ACP adapter carries that disposition from the outcome check to the turn's terminal event (provider thread status error), and resumeThread refuses a provider thread in error. The turn start's existing resume-failure recovery then opens a fresh native session with this thread's context handoff.
The error status now survives the paths that reset it before the next turn: a queued message starting, a session restart after a model selection change, a cross-instance resume, and local provider commands (sign-out, empty compaction).
This uses the same adapter plumbing as https://git.cafe/pingdotgg/t3code/pulls/28889 (a different Antigravity error), byte-identical in adapter.ts, so whichever lands second rebases trivially.
Red then green on the focused tests: AntigravityTurnOutcome.test.ts, AcpAdapterV2.test.ts, Orchestrator.control-reads.test.ts, ProviderTurnStartService.test.ts.
With the four implementation files reverted to main (b183048), the behaviour tests fail:
marks the native thread broken when a failed checkpoint step is replayed: expected undefined to equal 'broken'
marks the thread broken when the outcome check reports unusable native history: expected 'reusable' to equal 'broken'
keeps a broken native thread in error when a queued message starts: expected 'not_loaded' to equal 'error'
signs out the existing native provider before opening the newly selected provider with status error: expected 'idle' to be 'error'
the quota-wrapped checkpoint case, against the first version of this fix: expected 'provider_error' to equal 'usage_limit'
vp run check:changed: format, lint, knip, typecheck (14 packages), and the 6 changed test files pass.
The new regex /\bcontext summarization: checkpoint step \d+ failed\b/ is linear (one \d+ between literals, payload capped at 16 KiB). Adversarial inputs of about 16 KB ran in 0.01 to 0.03 ms.
Server-only change: the only visible difference is the failure message text, in the existing error row.
Not checked: a live Antigravity runtime (none available). The fix assumes the runtime ends the replayed failure as end_turn plus a type-17 error step, which matches the reporter's step table and the runtime 1.4.5 behaviour handled in https://git.cafe/pingdotgg/t3code/pulls/28629.

…lure is resumed forever
These tests reproduce #30414. A turn that ends with Antigravity's replayed
"context summarization: checkpoint step N failed" error must mark the native
thread broken, so the ACP adapter finalizes it as broken (provider thread
status "error") and refuses to resume it, letting the next turn start a fresh
native session with the thread's context.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Antigravity stores a failed context-summarization checkpoint step and
replays it on every later prompt to that native session, so one transient
auth failure left the thread failing forever: the outcome check reported a
generic reusable failure and T3 resumed the same native session each turn.
The outcome check now classifies "context summarization: checkpoint step N
failed" as a broken native thread. The ACP adapter carries that disposition
through finalizeTurn (terminal threadDisposition "broken", provider thread
status "error") and resumeThread refuses an "error" thread, so the turn start
recovers in a fresh native session with this thread's context handoff. The
"error" status now survives a queued run's dispatch, a session-restarting
selection change, and a provider sign-out command until that recovery
replaces the row.

Macroscope's review found this PR approvable — Failed Antigravity checkpoints now trigger fresh-session recovery instead of repeatedly resuming unusable history. Recovery reuses the existing context-handoff path and preserves the application thread identity.

Antigravity prompt outcomes now distinguish reusable threads from broken threads. The ACP adapter records broken threads as errors and rejects attempts to resume them. Orchestration preserves error status during queued-run startup, steering restarts, and provider sign-out.
Antigravity prompt outcomes now pair failures with a thread disposition. Failed context-summarization checkpoint replay produces a broken disposition; other failures remain reusable. Tests cover both outcomes.
Apply disposition in ACP adapter packages/provider-acp/src/server/adapter.ts, apps/server/src/orchestration-v2/Adapters/AcpAdapterV2.test.ts
The adapter passes the disposition into turn finalization, marks broken threads as errors, and rejects resuming error threads. Tests cover reusable and broken outcomes.
Preserve error status during orchestration apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/orchestration-v2/Orchestrator.control-reads.test.ts, apps/server/src/orchestration-v2/ProviderTurnStartService.ts, apps/server/src/orchestration-v2/ProviderTurnStartService.test.ts
Queued-run startup, steering restart, and provider sign-out retain error status. Tests cover queued-run startup and sign-out with error threads.

Explanation
The title names the main change: a failed Antigravity checkpoint no longer leaves the thread unusable. The changeset marks the native thread as broken on a replayed checkpoint failure, and the fresh-session recovery follows from that. The title is specific and follows the conventional commit form.
Full details: Description check
Explanation
The description follows the template. It has Problem, Change, Scope and approval, and Verification sections. The Problem section gives the failure, the replay behavior, and the observed symptom. The Change section lists the fix in each affected component and explains why the adapter and orchestrator changes are needed. The Verification section gives focused test results, the failing assertions observed before the fix, the lint and typecheck results, and what was not checked. The Scope and approval section links issue #30414, but it does not show a maintainer approval comment, and the template asks for one. The template also asks for the model and harness used, and the description ends with that attribution. The approval link is the only gap that matters, and the rest of the description is mostly complete, so the check passes.
Full details: Out of Scope Changes check
Explanation
The changes in AntigravityTurnOutcome.ts, adapter.ts, Orchestrator.ts, ProviderTurnStartService.ts, and their tests support the recovery flow for issue #30414. The preserved error status ensures that the existing fresh-session recovery can run. No unrelated change is demonstrated.
Full details: Linked Issues check
Explanation
Issue #30414 requires the failed context-summarization checkpoint to be retried with recovered credentials so the turn proceeds in the original native session. The implementation detects the replayed checkpoint in AntigravityTurnOutcome.ts, marks the provider thread as error, and makes resumeThread reject it in adapter.ts. Existing recovery then starts a fresh native session with context handoff. This prevents the permanent failure but does not implement the issue's stated checkpoint retry requirement.
Resolution
Implement a retry or re-summarization path for the failed checkpoint in the original native session, or confirm an updated requirement that accepts fresh-session recovery instead of checkpoint retry.

Keep error on direct cross-instance starts.
When a direct cross-instance start reuses an existing provider thread whose status is error, the status is overwritten to active at this line. The native reference is kept. Turn start then passes the row to ACP resumeThread, which refuses only when the status is error. The broken native history can then be resumed instead of triggering the fresh-session fallback. The queued-start and steering-restart paths already preserve error. This path should do the same.

Keep error on direct cross-instance starts (Orchestrator.ts:7034): valid. The start path for provider switches, forks and merge-backs overwrote the provider thread's status with active. A broken thread switched to another Antigravity instance (cross-instance resume keeps its native ref), or back to an instance whose root thread is broken, would then resume the poisoned session. Fixed in b6aceef as suggested. Focused tests (Orchestrator.control-reads, ProviderTurnStartService, ProviderSwitchService, AcpAdapterV2): 156 passed. vp run check:changed passes.
Linked Issues check (retry the checkpoint instead of starting fresh): T3 cannot do that. The failed checkpoint step lives in the Antigravity runtime's own conversation store, and the runtime replays it on every prompt to that session (the reporter's step table shows the replay in a freshly started runtime with valid credentials). ACP exposes no way to retry or re-summarize a stored step. A fresh native session carrying this thread's context is the recovery available from T3's side, and it is the "re-summarize" path the issue asks for.

Major · Do not finalize an unobservable checkpoint replay as… · AntigravityTurnOutcome.ts:148-164
apps/server/src/provider/acp/AntigravityTurnOutcome.ts:148-164 Major · Stability & Availability · Heavy lift
Do not finalize an unobservable checkpoint replay as successful.
When the native session log is unreadable, watchAntigravityPromptOutcome uses the typed-only path. An Antigravity checkpoint replay that returns end_turn then produces no failure because no type-17 payload is available. The ACP adapter finalizes the turn as completed, keeps the provider thread active, and permits resumeThread. The failed checkpoint can therefore remain resumable and replay on the next prompt.
Change this boundary to distinguish “no log evidence” from “successful prompt.” The fallback must prevent reuse or invoke the existing fresh-session recovery path instead of returning undefined. Preserve the current typed refusal handling. The exact implementation should avoid marking ordinary successful prompts as checkpoint failures when no native log is available.

On CodeRabbit's second finding ("Do not finalize an unobservable checkpoint replay as successful", AntigravityTurnOutcome.ts:148-164): not changing this, for two reasons.
No signal to act on. The typed-only fallback runs only when the session's own conversation database cannot be proven or read: the profile root or conversations directory is missing, or the path is a link or a foreign entry. On that path T3 has no evidence about any turn, failed or successful. If those turns were marked broken, every successful turn would throw away its native history and fall back to the context handoff. The finding itself rules that out ("avoid marking ordinary successful prompts as checkpoint failures"), and nothing is left to tell a replayed checkpoint apart from a normal end_turn.
Already the behaviour on main. Without the log, every hidden Antigravity error (quota, API errors, and this one) is already unobservable; this PR does not change that. Recovering when the log is unreadable would be a separate change, not this fix.
