fix(claude): say when Claude Code runs a Fast request at standard speed - #30108
With Fast Mode on for a Claude model, the composer shows Fast and the thread never says otherwise, even though Claude Code is answering at standard speed. Claude Code reports this in one of two ways, and T3's Claude adapter reads neither:
Fast is unavailable for the account (for example, usage credits are disabled for the org). system/init carries fast_mode_state: "off" and fast_mode_disabled_reason.
The API refuses the overage (credits are enabled, but the balance is zero). init and result both still say fast_mode_state: "on". The only signal is a system/notification frame with key fast-mode-overage-rejected and the text "Fast mode disabled · Out of usage credits".
Details, captured frames and transcript data: #30106.
Change
In ClaudeAdapterV2.ts, when the live selection has Fast on:
A system/notification whose key starts with fast-mode becomes a thread notice that uses the CLI's own text.
An init or result frame with fast_mode_state: "off" becomes the notice "Fast mode is off (<reason>), using standard speed". Reading result frames also covers Fast turned on through applyFlagSettings after the process's init.
pending produces no notice, because Claude Code has not decided yet.
Each Claude process announces this at most once, so a long thread does not repeat the notice on every turn. The change reuses the existing system_notice path that model_refusal_fallback and informational frames already take. Unlike those frames, fast-mode frames keep their normal handling afterwards, so init and result are processed as before. There is no contract or client change.
Scope and approval
Fixes#30106. #17651 reports the same symptom and #24657 proposes a fix for it. That PR only reads fast_mode_state / fast_mode_disabled_reason, so it gives no notice in the overage-rejected case, where the CLI keeps reporting "on". This PR covers both signals with a smaller change. The composer label is unchanged, since it still shows the user's selection. Hiding Fast when CLAUDE_CODE_DISABLE_FAST_MODE is set (raised in #17651) is out of scope.
Verification
Problem: I ran Claude Code 2.1.316 directly with --settings '{"fastMode":true}' --output-format stream-json on two real accounts.
Account A emitted init with fast_mode_state: "off" and fast_mode_disabled_reason: "extra_usage_disabled".
Account B emitted init with "on", a fast-mode-overage-rejected notification, and an assistant message with usage.speed: "standard".
Transcripts from T3 threads on both accounts: 75k+ Opus 5.5 responses, all standard.
Regression tests, run with vp test run src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts -t "Fast". Each uses the real frames above:
"tells the user once when Claude Code runs a Fast turn at standard speed": Fast off, then Fast on twice. No notice while Fast is off, then exactly one notice. It fails on main with expected [] to deeply equal [ Array(1) ].
"shows Claude Code's notice when the API rejects Fast while the CLI reports it on": init and result both say on and only the notification signals the rejection. It fails when the notification branch is removed.
"waits past a pending Fast decision and reads the state from the result": init is off/pending and result is off/extra_usage_disabled. It fails on the first commit of this PR.
Other checks: the full ClaudeAdapterV2.test.ts passes (298 tests), as do tsc --noEmit in apps/server and the pre-commit vp fmt, vp lint and knip:check.
Not checked: the notice rendering in a running T3 desktop or web client. It uses the existing system_notice row, so there is no UI change.
Claude Code drops a requested Fast mode to standard speed without failing
the turn: it reports fast_mode_state off on init when the account cannot use
it, or keeps reporting on and only sends a fast-mode-overage-rejected
notification when the API refuses the overage. Surface either signal once
per Claude process as a system notice.

Macroscope's review found this PR approvable — Displays a notice when Claude runs an explicitly requested Fast-mode query at standard speed, with duplicate notices suppressed per live query. The change reports provider behavior without altering speed settings or product defaults.
You can add or adjust custom eligibility rules. Learn more.
ClaudeAdapterV2 detects when requested Fast mode is disabled and emits a system notice once per live query. It handles Fast-mode notification, init, and result frames. Tests cover disabled and pending states, repeated Fast turns, and API rejection while the CLI reports Fast mode on.

Summary
Detect and emit Fast-mode notices apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The adapter selects notice text from matching notifications or init and result frames. It excludes pending states, emits at most one Fast-mode notice per live query, and allows relevant frames to continue through normal handling. Tests cover repeated Fast turns, a non-Fast turn, API rejection while Fast mode is on, and resolution of a pending state.
pingdotgg/t3code#17651 — This issue reports that the Fast mode label remains active during standard-speed requests without a thread notice. This PR adds a notice but does not change the composer label.

The description includes all required sections: Problem, Change, Scope and approval, and Verification. It explains the affected signals, implementation scope, linked issue, tests, observed results, and unverified client rendering.
Full details: Linked Issues check
Explanation
Directly linked issue #30106 has two coding requirements. Both are met at head a938c50.
(A) Account-level unavailability. Requirement: when Claude Code does not apply Fast, the thread states this and gives the reason. Code: claudeFastModeNotice() returns "Fast mode is off (<reason>), using standard speed" for a system/init or result frame with fast_mode_state "off". Underscores in fast_mode_disabled_reason become spaces. Test "tells the user once when Claude Code runs a Fast turn at standard speed" sends init off/extra_usage_disabled and asserts one notice across two Fast turns. It also asserts no notice on a non-Fast turn.
(B) Overage rejection while the CLI reports "on". Requirement: the thread shows the rejection. Code: a system/notification frame whose key starts with "fast-mode" emits its own text as a system_notice when the live selection has Fast on. Test "shows Claude Code's notice when the API rejects Fast while the CLI reports it on" sends init "on", the notification, and result "on". It asserts the notice text. The previous review found this case only by code reading. It is now tested directly.
The result-frame path covers variant A when init predates a Fast change. Test "waits past a pending Fast decision and reads the state from the result" confirms that pending is skipped and the later result "off" produces the notice.
The composer label is unchanged, as the issue does not request a change. Non-coding requirements do not apply.
Observation, not a requirement failure: the emission loop now skips any notice whose frame lacks uuid ("uuid" in message). This also applies to the existing model_refusal_fallback and informational path, which previously emitted without that check. SDK frames normally carry uuid, so the effect should be nil. The repository evidence does not show whether every CLI frame carries it.
Full details: Out of Scope Changes check
Explanation
The whole-PR diff against merge base cefb544 changes two files.
ClaudeAdapterV2.ts: adds claudeFastModeNotice(); adds announcedFastModeNotice (initialised to false) on ClaudeLiveQueryContext; and reworks the notice emission. The emission loop now handles the existing CLI notices (model_refusal_fallback, non-info informational) and the new Fast-mode notice through the same system_notice path. Only CLI notices return early, so init and result frames keep their normal handling. These changes support the Fast-mode notice objective of #30106.
ClaudeAdapterV2.test.ts: adds three tests. Each one covers a Fast-mode objective in #30106 (once-per-process notice, API rejection notice, pending-then-result state).
No contract, client, or composer-label change appears in the diff. Hiding Fast under CLAUDE_CODE_DISABLE_FAST_MODE, which the PR description names as out of scope, is not implemented. The once-per-process guard is stated in the PR description. The loop refactor of existing CLI notice handling is needed to share the emission path, so it is not unrelated.

Read fast_mode_state from result frames too, so Fast turned on in a process
whose init predates it is still reported, and do not treat a pending
decision as a downgrade. Cover the overage-rejected notification in its own
test.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Isolate the result-frame notice in the reused-process toggle test.
The toggle fixture always emits an off-state init frame and an overage notification. Its final notice assertion can therefore pass without processing the off state from a result frame. The separate result-frame test starts with Fast already enabled, so it does not cover this post-toggle path.
Make the reused-process toggle turn omit the init and notification frames, provide fast_mode_state: "off" with extra_usage_disabled only in its result, and assert the standard-speed notice immediately after that turn. Run any later turn only after this assertion.

Update the reused-process toggle test so the toggle turn emits no `init` or
overage-notification frames and reports `fast_mode_state: "off"` with
`extra_usage_disabled` only in its result. Assert the standard-speed notice
immediately after that turn, before running any later turn.

selected Claude Opus 5.5 and Fast in the composer, then sent the same prompt on main and the pr. both responses completed at standard speed. main showed no warning; the pr displayed Fast mode is off (extra usage disabled), using standard speed. Fast remained selected.
a second Fast turn in the same Claude session completed with exactly one notice in the thread, confirming duplicate suppression.
a new Standard thread completed with no Fast notice. switching that thread to Fast reused the Claude session and displayed the fallback notice when the result reported Fast off.
