Problem

Codex can reject a turn with usageLimitExceeded while the adapter's last rolling quota snapshot still says 99%, or before it receives any main-allowance snapshot. The failure then stores resetAt: null, disabling reset-based recovery even when an account read can identify the exhausted windows.

Fixes #28284. Related: #27773 and #14169.

Change

When a root usage-limit failure has no known reset, perform one account/rateLimits/read with a three-second request timeout. Prefer the main codex bucket from the returned map, with the legacy single-snapshot fallback.

The RPC runs outside the notification handler so its response can be processed. The terminal failure waits for resolution and carries the resulting reset itself, allowing the run ingestor to persist it before closing its event subscription. Failed native turns are marked before the read to prevent child-completion continuations during recovery.

Successful reads replace the cached snapshot and publish changed usage windows through onUsageLimits. A read is discarded if a main-allowance notification changed the snapshot while it was pending. Sparse rolling notifications retain their existing merge semantics. Known resets skip the read; errors, timeouts and responses without identifiable exhausted windows retain an unknown reset. The existing rule that all exhausted windows must reset is unchanged.

Scope and approval

This is a focused fix for an established missing-reset bug under the small obvious-bug exception in CONTRIBUTING.md. It changes only the Codex adapter's reset recovery and its existing replay tests. It adds no settings, contracts, client defaults, percentage thresholds or localized error-text parsing.

Verification

  • Stale- and empty-snapshot replay cases assert the reset on the terminal failure itself and the published session/weekly usage windows. These assertions failed before their corresponding fixes. Exhausted session and weekly windows resolve to the later weekly reset.
  • Added replay cases for a newer notification arriving before an older read response, and a full read omitting a formerly exhausted window. Restoring the old unguarded merge reproduced an obsolete reset in each case.
  • Other cases cover request failure, a still-unexhausted response, a three-second timeout using TestClock, skipping the read for a known reset, and existing late/deferred reset behavior. The timeout case verifies there is no terminal while the read is pending, then a terminal failure with an unknown reset after timeout.
  • pnpm exec vp run check:changed passed: format, lint, both knip passes, GraphQL consistency, typechecking of three affected packages, and 133 tests across two files. Commit hooks also passed format, lint and knip.
  • No live quota exhaustion or desktop UI run was performed. The production incident did not capture a fresh account read; if the backend still cannot identify an exhausted window, this change retains an unknown reset.

Walkthrough

When a Codex usage-limit failure has no reset time in the cached snapshot, the adapter now performs a bounded rate-limit read. It applies eligible results, reports usage-limit updates, and emits the terminal event with the reset time resolved from the current snapshot.

When a usage-limit failure lacks a cached reset, the adapter reads account limits with a three-second timeout. It applies the response only if the snapshot has not changed and the limit ID is absent or codex. Replay tests cover read failures, timeouts, newer notifications, and reset-time outcomes.

Full details: Title check

Explanation

The title "fix(server): refresh Codex reset times after usage limits" matches the main change. The PR adds a single account/rateLimits/read with a three-second timeout when a root usage-limit failure has no known reset, so the terminal failure can carry a reset time. The title is specific, follows the conventional commit format, and is concise at 57 characters.

Full details: Description check

Explanation

The description follows the template structure. It has Problem, Change, Scope and approval, and Verification sections. The Problem section states the failure mode and the expected behavior. The Change section explains the read-based recovery, the timeout, and the guard against stale responses. The Scope section cites the small obvious-bug exception and explains why no prior issue is needed. The Verification section lists the replay cases, the check command and its results, and what was not tested, including the lack of a live quota exhaustion run. The description has no separate reproduction steps, and the template's request for a model and harness line is answered at the end. The objectives report a later merge of main that added sharing-cap handling, and a commenter proposed a change to the race guard. The description does not cover either point, so the description is not fully current with the PR state. The description is otherwise complete.

Full details: Linked Issues check

Explanation

[#28284] The whole-PR diff (merge base 298cb24 to head ca74001) implements the requested recovery in CodexAdapterV2.ts. Steps: (1) resolveUsageLimitResetAt returns the cached reset when codexUsageLimitResetAt(cache) is non-null, so a known reset skips the read. (2) Otherwise it calls account/rateLimits/read with Effect.timeout('3 seconds') and Effect.option, so errors and timeouts leave the reset unknown. (3) It prefers rateLimitsByLimitId.codex and falls back to rateLimits. It applies the snapshot only when the limitId is absent or 'codex'. (4) Ref.modify applies it only if the cache reference still equals the pre-read reference, so a newer main-allowance notification wins. (5) The returned reset comes from codexUsageLimitResetAt, which keeps the existing exhausted-window rule. (6) emitRootTerminal sends the failure with the resolved resetAt. The issue's sequence (99% primary, null premium update, usageLimitExceeded) reaches this path because the 99% window is not exhausted. The change therefore triggers one read. Unknown resets remain for failed, timed-out, unexhausted, or omitted responses. The change does not add a percentage threshold or parse localized error text. Replay tests named in the PR description cover these cases. I did not read the test diff. Test coverage rests on the PR description and summary. Open uncertainty: the objectives mention a sharing-cap rule from main that skips the read. The reviewed adapter diff does not show that rule. Issue #28284 does not require it, so it does not affect this verdict.

Full details: Out of Scope Changes check

Explanation

The whole-PR changes touch two files: CodexAdapterV2.ts and CodexAdapterV2.test.ts. The adapter diff contains reset recovery only. It extracts backfillUsageLimitResetAt from the rate-limit notification handler, adds resolveUsageLimitResetAt with its guarded snapshot update and usage-window publication, moves the terminalizedNonCompletedNativeTurns update ahead of the read, and defers root terminal emission through emitRootTerminal and emitResolvedRootTerminal. Each change supports the reset recovery in #28284. The PR description states that no settings, contracts, client defaults, percentage thresholds, or localized error parsing changed. The adapter diff agrees with that statement. Changes since the previous reviewed head were not inspected separately. This verdict uses the whole-PR diff against the merge base.

Unverified finding
Verification ran but could not confirm this finding. It is shown for review, not as a verified issue.

Add a fallback so a failed reset resolution still emits the terminal event.

emitRootTerminal now emits the terminal event only inside this forked fiber. If resolveUsageLimitResetAt() dies, for example because adapterOptions.onUsageLimits raises a defect, the fiber ends without calling emitResolvedRootTerminal.

finalizeCodexTurn has already removed the turn from activeTurns by then. The run then never receives turn.terminal and stays running. The read failure and timeout paths are handled by Effect.option. The onUsageLimits call and the Ref operations are not.

Convert non-interrupt failures into an unknown reset so the terminal event is always emitted. This preserves the existing interruption behavior.

Merged current main

Merge commit ca74001873 brings in main (no rebase). It conflicted with #25341, which added failureResetAt so ChatGPT sharing caps never get an allowance reset.

  • backfillUsageLimitResetAt now takes the snapshot and applies failureResetAt per limited item, keeping fix(codex): sharing usage limits offer the existing recovery options #25341's rule.
  • emitRootTerminal skips the recovery account/rateLimits/read for sharing caps, since their reset would be discarded anyway.
  • The test table keeps both sides' rows. Sharing-cap rows expect no read. The replay harness records usage updates and still forwards main's onUsageLimits hook.

Blacksmith vp run check:changed passed: format, lint, both knip passes, GraphQL, typecheck of 3 packages, 155 tests (153 in CodexAdapterV2.test.ts).

Not verified: a real exhausted Codex account, same limit as below.

Earlier verification, written by gpt-6.1-sol:

Verified in a real dev environment

Partial verification of head 02409c7a0af36c52f195b4d72b2fba0ff6083a48 on a Linux Boat VM. No source changes or pushes.

Started github/main (955e2bfbb5) first in a separate worktree, then the exact PR head, with one dev server at a time and isolated verification state. Used repository Playwright with /usr/bin/google-chrome. Added a project pointing at the real repository through the UI. Real Codex gpt-6.1-sol turns completed on both revisions. On the PR, a second turn in the same thread completed and both responses survived a browser reload. Read-only inspection of the persisted runs confirmed gpt-6.1-sol and completed status.

The usage panel handles unavailable quota data on both revisions. The real Codex app-server confirms why:

request: account/rateLimits/read, params: null
response: JSON-RPC -32600
codex account authentication required to read rate limits

Remote Blacksmith checks passed: 146 tests across the Codex adapter and quota helper files. vp run check:changed verification/main also passed format, lint, both knip passes, GraphQL consistency, three package typechecks, and 133 tests. The remote snapshot required fetching main history before that check could find its merge base.

The recording shows model selection, two real turns and the unavailable-quota panel. Startup and reload splash frames are omitted. It does not demonstrate quota recovery.

real codex turns and unavailable quota panel

two real codex responses retained after reload

quota read unavailable through the configured proxy

Not verified

The changed behavior remains unverified in the real app. The configured proxy allows model turns but cannot provide account quota snapshots. No genuine usageLimitExceeded occurred, so the original stale/missing-reset bug could not be reproduced on main, and refresh-on-failure, persisted reset recovery, automatic continuation after reset, read timeout, and notification/read races could not be exercised against a real exhausted account. Passing replay tests does not establish that integration result. No mocks or injected quota responses were used.

Native desktop, iOS and Android clients were not exercised on this Linux VM. Other providers were outside this Codex adapter change.

This fixes most of the concurrent-session case I described in #28284 (comment). One gap remains, and I have a small change for it, verified against this head (02409c7a).

Gap: resolveUsageLimitResetAt drops the read whenever a main-allowance notification replaced the snapshot while the read was pending. With many threads on one shared Codex session, those notifications are usually late reports of the same window from streams that started before the refusal. In that log, all 337 main updates in the burst had the same resetsAt (1793672923), with usedPercent jumping between 74 and 100. Replaying this PR's rule against the recorded timestamps, with the measured account/rateLimits/read latency (p50 569 ms, max 1052 ms), drops the read for 4–5 of 39 refusals, and those store resetAt: null.

Proposed change: keep the cache rule as it is. Only when deciding this failure's reset, still use the read if the notification that won the race reports the same reset for every window both snapshots share. A changed reset still means the window rolled over or the allowance was restored, so newer-notification-read keeps its current expectation.

Tests added:

  • stale-notification-read in the classifies Codex terminal failures table: a 99% snapshot, the refusal, then a 97% notification for the same window (resetsAt: 2000000000) while the read is pending, and a read that reports both windows exhausted. Expected resetAt is the read's reset.
  • codexRateLimitWindowsMatch unit tests: same window at a lower percentage matches; a rolled-over window does not; no earlier snapshot, or no shared reset to compare, matches.

Verification (Linux, Node 26.12.1, pnpm 12.14.0, the repo's vp):

  • The new adapter case fails on 02409c7a without the change: expected null to equal '2033-05-19T07:20:00.000Z'. It passes with the change.
  • vp test run apps/server/src/provider/codexUsageLimits.test.ts apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts: 146 passed before the change, 150 passed after (1 adapter case and 3 unit cases added). newer-notification-read still expects null and passes.
  • All 25 server test files matching codex*.test.ts or *UsageLimit*.test.ts, including UsageLimitRecoveryWorker.test.ts and CodexReplayFixtures.integration.test.ts: 399 passed.
  • vp lint --report-unused-disable-directives on the four changed files: clean. tsc --noEmit in apps/server: no errors in the changed files. The other errors were identical with and without the change, and come from root scripts that my filtered install did not cover.

Not verified: a live refusal against Codex. The change assumes Codex reports a new resetsAt when a window rolls over or a reset credit restores the allowance. If Codex can restore the allowance without changing resetsAt, this would keep the old reset for a failure whose read raced that restoration.

The account/rateLimits/read recovery merged its response with sparse
notification semantics and no freshness check. A read that resolved after
a newer notification restored the older window, and a complete read kept
exhausted windows it no longer reported. Both could give a usage-limit
terminal an obsolete reset.

The read now replaces the snapshot as of its request, and fields a
notification changed while it was pending stay. A lower notification for
the same window (same resetsAt) is a late tick from a concurrent stream,
so the read's higher usage wins there; the same-window reasoning follows
budres's review of #28291.

…eset time

When the first request of a run hit Codex's usage limit, Codex had only pushed a
model-specific rate-limit snapshot, so the failure had no reset time and could not
auto-resume. A usage-limit terminal with no reset now waits on its own fiber for one
shared, bounded account/rateLimits/read and publishes with the codex reset. A failed,
timed-out or defective read still publishes the original failure; interruption ends it.

The read replaces the snapshot as of its request, so windows and resets it omits are
cleared in the published usage cache (replaceWindows), while fields a notification
changed during the read stay; a lower same-reset report is a late tick and loses to the
read, following budres's same-window reasoning in #28291. Late resets after the
terminal still enrich its failure item, keeping retry metadata.

ChatGPT sharing caps never take this path: their reset is not in the ordinary allowance
snapshot, so they publish at once with no reset and never read or inherit one.
