fix(acp): retain delivered instructions when reopening a conversation - #24740
Settling and reopening an ACP thread loses the runtime's instruction-delivery record, so the next prompt appends the same instruction block to an already populated native conversation. Fixes#24465.
Change
Persist a small optional receipt containing the native session ID and a SHA-256 digest of the generated instructions. Only matching instructions for that conversation suppress reinjection. Record successful delivery before terminal events, including ACP v2 work-state completion. Compaction, replacement instructions and session replacement invalidate the receipt; failed or cancelled turns do not establish durable delivery. Existing in-process acknowledgement still supports steering.
Receipt-invalidation persistence failures are logged and retried during cleanup without aborting the runtime event pump. This preserves subsequent runtime events instead of reporting a spurious terminal error.
Scope and approval
Submitted under the focused bug-fix exception for restoring instruction-delivery bookkeeping on an existing conversation; no maintainer approval or completed issue triage is claimed. The optional metadata needs no database migration or backfill. Old records conservatively inject instructions. Client behavior is shared across web, desktop, mobile and remote access. Other ACP-backed driver edits only declare the digest service required by the shared adapter.
Verification
Current head: 755344d, including main at e455200. vp run check:changed passes 201 tests across five files, all 36 affected-package typechecks, GraphQL generation, formatting/lint and Knip.
The regression uses real mock ACP subprocesses and production adapter/session-manager code. It covers first delivery, close/reopen of the same native session, native/protocol compaction, work-state completion, cancelled steering, mode transitions, idle compaction, same-ID rollback and persistence failure. Before the corresponding fixes, reopened prompts wrongly repeated or omitted instructions; the invalidation failure also prevented later runtime events. The existing ownership-matrix regression reproduced that last failure and now passes. Current integration details.
No paid Copilot/Trae calls or real client were used for this verification. External compaction while T3 is disconnected, providers silently rewriting history, and platform-native UI behavior remain unverified. Receipts contain no prompt text.

Macroscope's review found this PR approvable — Reopened ACP conversations remember successfully delivered instructions instead of injecting them again. Session-bound receipts and invalidation after compaction or session replacement keep the change limited to instruction-delivery bookkeeping.

Walkthrough
The ACP adapter records instruction identity in provider-thread metadata and uses it across session resume. It updates that metadata during prompt delivery, compaction, replacement, and rollback. ProviderSessionManager persists matching invalidations. Tests cover instruction reuse and reinjection across session transitions.

Provider-thread metadata can store a native session ID and instruction digest. Prompt resolution checks the current session and digest. Resume merges remembered metadata into the returned provider thread. Provider driver environments include the Effect Digest service.
Delivery recording and invalidation packages/provider-acp/src/server/adapter.ts
Active turns track instruction state, digest, and compaction marker. Prompt delivery, compaction, replacement sessions, and rollback update instruction metadata. Completed turns record delivery when the compaction marker remains current.
ProviderSessionManager conditionally persists instruction invalidations when session, run, and native thread ownership match. Subscription closure retries persistence. Tests cover ownership and completion cases.
Reopen and compaction validation apps/server/src/orchestration-v2/Adapters/AcpAdapterV2.test.ts, apps/server/scripts/acp-mock-agent.ts, apps/server/src/orchestration-v2/testkit/fixtures/provider_thread_resume/kiro_transcript.ndjson
The test harness covers session updates, reopening, rollback, compaction, interaction-mode changes, and work-state completion. The mock agent can emit an accepted-prompt message when its environment flag is enabled. The resume fixture uses a direct second-turn prompt.
Reopening an ACP conversation should no longer duplicate unchanged instructions. A transient failure while persisting an invalidation no longer tears down the live session; it is logged and retried during cleanup. No remaining merge-blocking issue was found.

updated the original branch, keeping @nateEc's commits and persisted-receipt design. merged current github/main without rebasing.
registered resumed threads before idle updates and reconciled their cached metadata.
persisted idle receipt invalidations through the session manager, including reapplying them after queued completion writes, session release, and repeated cleanup. conditional writes preserve newer session/run ownership and other stored metadata.
restored generation-checked compaction-marker cleanup on successful prompt acknowledgements, including cancelled prompts. cancelled turns still do not establish durable receipts.
removed the duplicate successful-completion thread update and narrowed its helper to invalidation.
verification: reproduced the reopen failure on current main, missing invalidation after resume, lost idle persistence, queued-completion overwrite, and cancelled-prompt reinjection before their fixes. blacksmith vp run check:changed passes formatting, lint, both knip passes, graphql generation, affected typechecks, and 135 tests. all 24 registry/grok replay tests also pass. no new test files or ui changes.
@nateEc, a remaining low finding is that a session_info_update before the first resumed turn can omit the receipt from a partial metadata payload and cause conservative reinjection. recorded as a known limit rather than extending the review loop. external compaction while disconnected and real account-based provider/client flows remain unverified; verification used production adapter code with real local mock acp subprocesses and the session manager's sqlite persistence path.
head: cb0b4371bb7c93867dc731188639ce09a51d6553. two independent reviews found the ordering/cleanup cases above; each now has a failing-before/passing-after regression. the allowed review round and follow-up are complete; the final cleanup correction was verified with the existing checks and has not had another independent review. final ci is green, including all eight server shards, web tests, build/typecheck, lint, release smoke, and the correctness check. coderabbit rate-limited the latest review. human lifecycle approval remains.
Merged current main in 7f20d1b. Instruction-state invalidation now lives in main's shared replacement-session commit path, so rollback and forced replacement clear stale instruction receipts while preserving unrelated native metadata. Both main's shutdown/question behavior tests and this PR's instruction-invalidation ownership tests are retained. vp run check:changed passes, including 140 tests across 3 files and affected-package typechecks. The existing request for maintainer review of the persistence approach still applies.

pr: identical prompts and flow resumed the same native session with zero instruction blocks in the follow-up (1,363 characters, including normal per-prompt context). both turns returned ORCHID-24740, proving conversation memory survived.

real compaction invalidation: submitting /compact reached the server, which refused before sending an acp prompt with This provider does not support context compaction. the local registry adapter does not expose this capability. a normal turn immediately afterward succeeded. no synthetic compaction event was injected.
native ios/android, electron shell, macos/windows, remote/relay/tunnel connections, other acp providers, rollback/session replacement, interrupted mode transitions, acp v2 work-state ordering, and database-conflict fault injection were not exercised. this verifies the real codex acp reopening path and the neighboring behaviors listed above, not every branch in the diff.
merged current main without rebasing, preserving native compaction dispatch and instruction invalidation. pushed a4e10f9a7757523a119ee7ad7ac378f8c2dc40d5 to the existing contributor branch without force.
addressed both current review findings: invalidation now checks the current runtime's attachments, and subscription cleanup logs each failed invalidation independently so remaining writes continue and failed entries remain retryable. extended the existing ownership table with detached-close and failed-close cases; both failed before the fixes and pass afterward. no new test files or ui changes.
blacksmith verification passed: 154 focused adapter/session-manager tests, then vp run check:changed check-base/main with formatting, lint, both knip passes, graphql validation, 36 affected-package typechecks, and 167 tests in four files. the comparison base was supplied in the remote snapshot only; pr history was not rewritten. two independent reviewers approved the final head.
follow-up e10de58ad81c6aab4bc8613c86a0d59cf63614e6: ci exposed a stale kiro resume replay expectation. the recorded mismatch showed the expected second instruction block versus the correct bare resumed prompt. changed only that second-prompt fixture line; all 17 kiro replay cases now pass, including resume, which reproduced the ci timeout before the correction. both independent reviewers approved this final head.
ci is pending on the follow-up. real account-based provider/client verification was not repeated this run; the earlier verification comment retains that evidence and its limitations. this update verifies the cleanup/ownership changes through existing sqlite persistence regressions and the production adapter's subprocess tests.
remaining review finding: ProviderSessionManager.ts:2086-2095 maps an invalidation persistence failure into an event-stream error. the pump exit handler then releases the provider session with runtime_error. this is valid; the pending invalidation could instead remain queued for cleanup retry while the pump logs the failure and continues. no third patch was made because the task limits this run to two fix rounds (ownership/cleanup fixes, then the kiro replay correction). this remains a merge-readiness blocker.

apps/server/src/orchestration-v2/ProviderSessionManager.ts:2086-2095 Minor · Stability & Availability · Quick win
Do not fail the event pump when invalidation persistence fails.
The pump maps a failure from persistInstructionInvalidation to ProviderAdapterEventStreamError. That error ends Stream.runForEach. The exit handler then calls releaseEntry with reason: "runtime_error", so one transient projection-read or event-sink write failure can tear down the live provider session and fail its active run.
This persistence step is a best-effort cleanup. The pending entry stays in entry.instructionInvalidations, and subscription close already retries it and logs failures (Line 1687-1699). Treat a failure in the pump the same way: log it and continue publishing the event.

Addressed the remaining event-pump persistence failure from #24740 (comment) in 755344d. Existing maintainer changes and the Kiro replay correction were preserved; main e455200 was merged without rebasing.
The pump now logs an instruction-invalidation persistence failure and continues publishing provider events. The pending invalidation remains available for subscription cleanup retry. The existing ownership table now injects a one-shot EventSinkWriteError during event delivery and verifies that the session stays live, no runtime close occurs, the event is delivered, and cleanup eventually persists the invalidation. It failed before the fix and passes after it.
Also adapted the instruction digest and four ACP-backed driver environment types to main's split Digest/SecureRandom API, and moved the mock's startup flag alongside its existing startup configuration. check:changed passed: formatting, lint, both knip passes, GraphQL validation, 36 affected-package typechecks, and 201 tests in five files. Earlier real-provider verification remains documented above; no real provider turn was repeated for this persistence-only follow-up.
