fix(claude): keep the cleared conversation after process replacement - #24168
Claude /clear starts a new native session, but T3 retains the old session ID. After a server restart or process replacement, replies resume the pre-clear conversation and lose the context shown in the thread.
Change
Persist the session selected by the root Claude init event while preserving the T3 thread identity. Keep query replacement, turn attachment, and background routing on that session, including adoption during asynchronous turn preparation and subscription shutdown.
Use the target run attempt's recorded session for historical fork and rollback. Rollback closes the superseded live query before returning to the earlier conversation, and stale resume input follows the adopted session.
Reuses this existing PR and its original fixes. No new command, option, contract, or persisted field.
Verification
Latest bot follow-up at 91b52c8aee: focused tests passed, server typecheck passed, and scoped lint passed. The new real-adapter replay fails before the fix because fork receives A with B's cursor, and passes afterward: red log, green log. Run finalization now records the native session observed during the run; later idle adoption leaves finished attempts unchanged. Subscription shutdown holds the existing event permit while draining unread updates, preserving their order before newer refused updates. The narrow scheduler race has no deterministic test hook; existing session-manager tests pass.
Initial focused server tests: 418 passed across eight files (Claude adapter, session manager, provider switching, run execution, turn start, fork execution, checkpoint rollback, and projection store).
After merging main 6e79211743 into 65d8f2aa7b, all 418 tests passed again. Server typecheck and scoped lint passed; four warnings are on unchanged lines. Post-merge validation. The merge had no conflicts; main's only overlap was ProjectionStore imports.
Deterministic red tests reproduced lost terminal-tail adoption, adoption during attachment and system-prompt reads, immediate restart before a new-session prompt, and historical fork/rollback. Red/green summary.
Final rollback regressions fail before the fix and pass afterward, including a late background notification and stale resume input. Final red/green summary.
The runtime uses the real web dev client and authenticated Claude Opus 5.5 on Linux with isolated state. No render paths changed; the visual fixture matrix is empty. Independent proof audit passed for f7828e7c2e and independently reran 14 focused tests. Packaged macOS desktop was not run. Historical fork/rollback proof is focused service and adapter tests.
Implemented and verified with Claude Opus 5.5 in the Claude Code harness and GPT-6.1 Sol in the Codex harness, orchestrated through T3 Code.
Macroscope's review found this PR requires human approval — This Claude lifecycle fix adds a concurrency-sensitive session-identity adoption path that rekeys live turn, wake, background-task, monitor, goal, and resume state. The production behavior is cross-cutting rather than a small self-contained correction, so human review is warranted.

Rollback context includes the target attempt’s native session ID. Rollback and fork setup use attempt-specific session state. Turn-start history detection and finalized attempts account for adopted sessions.
Issue 15854 concerns Claude session identity when interruption occurs before system/init. This PR changes adoption after system/init, but the pre-init interruption case is not clearly resolved.
Issue 30097 concerns idle compaction resuming the pre-clear session. This PR changes the adopted session used for later resume input; the compaction scheduling behavior is separate.
Possibly related PRs
pingdotgg/t3code#29961: Both address Claude /clear; this PR persists the adopted session, while that PR resets cached context usage.
pingdotgg/t3code#21599: Both change Claude rollback across native conversation boundaries.
Historical rollback can leave an old process running, and a failed session can remain unusable until manually closed or the server is restarted. These recovery paths should be fixed before merging.

Explanation
[#24152] requires that replies after Claude /clear use the post-clear native session B after the process is reopened. The reviewed head adopts the session from the root system/init event in ClaudeAdapterV2.ts. Query replacement, turn attachment, background routing, and resume input use the adopted session. ProviderSessionManager.ts, EventSink.ts, ProviderEventIngestor.ts, and RunExecutionService.ts persist adopted provider-thread state, including idle and shutdown paths. AttemptNativeSession.ts exists at head. withAttemptNativeSession updates the native thread reference only when the attempt's native ID differs. ProviderTurnStartService.ts, CheckpointRollbackService.ts, and ProjectionStore.ts use the target attempt's recorded session for native fork and rollback. Regression tests cover /clear adoption, restart and resume into B, historical fork, rollback with stale resume input, and the real-adapter fork replay. The previous PASS remains supported by the current change summary. Remaining uncertainty: the summary does not show a deterministic test for the narrow scheduler race. The description says this race has no test hook. The fresh authenticated Claude run was not repeated in the review. The issue's coding requirement is met by the code and tests described.
Full details: Out of Scope Changes check
Explanation
The changed production and test files support the session-adoption lifecycle described in #24152 and the PR description. Query routing and adoption in ClaudeAdapterV2.ts implement the core fix. ProviderSessionManager.ts and ProviderSessionManager.test.ts cover persistence of adopted provider-thread updates during idle release, subscription shutdown, and persistence failure. ProviderAdapter.ts adds optional persistEvent and discardEvent hooks. These hooks carry the adopted state to persistence and do not add a user-facing command, option, contract, or persisted field. EventSink.ts, ProviderEventIngestor.ts, and RunExecutionService.ts apply the same persistence path. CheckpointRollbackService.ts, ProjectionStore.ts, ProviderTurnStartService.ts, and AttemptNativeSession.ts implement historical fork and rollback from the attempt's recorded session. The added fixture and integration tests exercise these paths. The summary does not show changes to unrelated subsystems. The objectives mention OpenCode replay tests, but the reviewed change summary lists no OpenCode file. That mention does not establish an out-of-scope change.

With no run subscribed, ProviderSessionManager published the adopted
provider thread to an empty subscriber set, so the projection kept the
pre-clear native ID and a later resume reopened the old conversation.
An unsubscribed provider_thread.updated now goes straight to the
ingestor, like session-scoped runtime requests.

Claude keys its task store by native session unless
CLAUDE_CODE_TASK_LIST_ID is set. Moving the cached list to the adopted
session merged pre-clear tasks into the new session's list. Drop the
cached entry instead, so the next task tool reloads the right store.

Resolve the adopted session before opening the next query.
When a turn uses an old providerThread snapshot after an idle /clear, openQuery receives the old native ID before attachTurn runs. It closes the live query and opens the old session. attachTurn can then synchronize to that reopened old session.
Resolve the current thread by its stable provider-thread ID before calling openQuery. Do not rely on the idle provider_thread.updated event reaching the turn-start projection first.

A turn built before the idle adoption reached the projection carried
the pre-clear native ID, so openQuery replaced the live process and
resumed the old session. The adapter now records each adoption and
redirects a turn that still names the replaced session.
The pre-clear todo list also stayed the thread's active list when no
task tool ran afterward. Adoption now supersedes it, unless
CLAUDE_CODE_TASK_LIST_ID shares the list across sessions. An idle
adoption publishes the supersede at the next turn, because nothing
persists adapter events while no run is subscribed.

@coderabbitai On the outside-diff finding Resolve the adopted session before opening the next query (ClaudeAdapterV2.ts:9657): fixed in 3461eec.
The adapter now records each adoption as old native ID → new native ID. At the start of startTurn, before openQuery, a turn whose provider thread still names a replaced session is redirected to the adopted one, and its conversation head is cleared. This works whether or not the idle provider_thread.updated has reached the projection yet (63401a5 now persists that update). The chain cannot loop, because adoption removes the new ID as a key before adding old → new. New test keeps the cleared session when the next turn carries the pre-clear thread: before the fix, the stale turn replaced the live process (2 processes); now the live query is reused and the turn stays on the cleared session.
Verified the existing review follow-ups on 3461eecd90: idle provider-thread updates have a persistence path without a run subscriber, stale next-turn input retains the adopted session, and per-session todo plans are retired while explicitly shared task lists are preserved. The updated adapter and ProviderSessionManager suites pass all 256 tests. All code-review threads are resolved; maintainer lifecycle approval and live authenticated verification remain outstanding.
pingdotgg#24153)
Two deterministic failing tests for races in the existing fix:
- ProviderSessionManager routes a provider_thread.updated to a run
subscriber that is finalizing its terminal; closing the subscription
drops it, so the cleared session is never persisted.
- ClaudeAdapterV2 resolves the /clear alias before attachment prep; a
root init arriving during prep makes openQuery replace the live
process and resume the pre-clear session.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…ession as new (pingdotgg#24153)
The /clear attempt records the pre-clear native ID, so after a provider
session restart the next turn reports nativeThreadHasTurns: false for the
cleared session. A fresh Claude adapter then opens it with sessionId,
which Claude rejects because /clear already wrote its transcript.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…tart (pingdotgg#24153)
- Persist provider thread updates a settled run never read when its
subscription closes, so a /clear adoption queued behind the terminal is
not dropped. Subscriptions pull one signal at a time so unread events are
still queued at close.
- Resolve /clear aliases together with the live query under the SDK lock,
after async query prep, so a turn whose attachments are still loading
keeps the adopted process.
- Treat a native id no attempt started on as provider-adopted, so a fresh
process resumes the cleared session instead of creating it again.
- attachTurn overrides only the native identity and head of the turn's
provider thread.

…e-clear session (pingdotgg#24153)
A turn that replaces the Claude process snapshots its native id before
reading the append system prompt file. A /clear adopted during that read
is lost: the replacement resumes the pre-clear session.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…t lost (pingdotgg#24153)
A turn that replaced the Claude process picked the native session before
reading the append system prompt file, then closed and reopened outside the
SDK lock. An init adopting a /clear during that read was overwritten by a
resume of the pre-clear session. The reads now run first and the decision,
close and open happen together under the SDK lock.
A provider thread update routed while the last run subscriber closed could
also be dropped. Close now ends the queue before draining it, so an update
is either drained or refused, and a refused update is persisted runless.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…he turn ran in (pingdotgg#24153)
Adopting a /clear moves the provider thread to the new native session, so
forking or rolling back to a pre-clear turn asked the new session for a
message only the old one has. Fork and rollback now use the native session
their run attempt recorded, and a rollback into a session /clear left stops
following the adoption alias.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…ssion live (pingdotgg#24153)
Rolling back into the pre-clear session leaves the live query on the session
/clear adopted, so its late background notification republishes that session
over the rollback. resumeThread with the pre-clear thread also returns the
pre-clear session instead of following the adoption.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
… follows the adoption (pingdotgg#24153)
Rolling back into the pre-clear session now closes the live query still
running the session /clear adopted, and forgets that session first so its
exit cannot republish it over the rollback. resumeThread with a pre-clear
thread follows the adoption the same way a turn does.

It passed. This appears to be an unrelated PDF-loading timing failure; no PDF code or test timeout was changed.

Resolved the new conflict with main (cbd3e149a1) in 5278cdd94c. Main's supplied-session-title option is retained inside this PR's adoption-aware query-opening loop, including its new SDK version. The adoption lock, pending-update persistence, and model-usage ledger are preserved.
All 611 selected regression tests across 11 files and all 25 affected package typechecks passed, along with formatting, lint, GraphQL generation and the first dependency-analysis pass. The focused PDF scroll/zoom browser case that failed on the earlier CI head also passes on this new head. This update triggers fresh CI and supersedes the earlier request to rerun the failed 52ca18460c run.
Resolved the conflict with current main (cefb544af3) in 1c9165018e. The Claude adapter retains this PR's adoption-aware query-opening loop and incorporates main's replacement-process subagent bookkeeping and cleanup into that live path. The incoming duplicate query-opening block was removed. Main's other adapter changes, including root-refusal diagnostics and transcript resume handling, are retained.
Updated this PR's existing test fixtures for main's Effect crypto-service migration (SecureRandom with NodeCrypto.layer). Local checks passed: 626 tests across 11 selected suites, all 26 affected package typechecks, formatting, lint, generated GraphQL checks and the first knip pass. Validation ran sequentially with one test worker and a 1 GiB Node heap cap; the second knip pass remains for CI. This integration did not repeat the authenticated Claude run or native-client verification. Fresh CI will run on the pushed merge commit.
Fixed both server-shard failures in cda6fb6ad7. They shared the same OpenCode nested-background replay failure: an unchanged child-thread snapshot was stored under the run, then stored again without a run after the subscriber closed. The unread-update fallback now skips a snapshot already present in the projection, comparing with the schema's optional-field defaults applied. It still normalizes rewind effects and writes changed snapshots.
Extended the existing session-manager scenario table. The added case fails on the previous code with two stored snapshots instead of one. With the fix, both affected replay suites pass all 29 tests, and the broader selected checks pass 627 tests across 11 suites and all 26 affected package typechecks. Formatting, lint, generated GraphQL checks and the first knip pass also pass. Checks ran sequentially with one test worker and a 1 GiB Node heap cap; the second knip pass remains for CI. No live-provider or native-client verification was repeated. The push changes two files, with 27 lines added and two removed; fresh CI is pending.
Please review the current head, cda6fb6ad7. The summary still refers to 5e1c4d9 and says reviews are paused. The previously reported rewind/release findings have been addressed; the latest follow-up also fixes the duplicate runless snapshot writes exposed by the OpenCode replay suites. No-op checkpoint rollback preserves the current native session and conversation head. Actual rollback and native fork now share the attempt-session substitution helper.
The older outside-diff release-save finding was still applicable. Release now tries each pending update up to three times, continues to later updates if one fails, and keeps the live entry with its unsaved update when retries are exhausted. A later release can save it.
Extended existing test cases rather than adding separate suites. The three regression cases fail on the previous code: no-op rollback selects the wrong session, a transient release failure loses the snapshot, and a persistent failure removes the entry after one attempt. With the fix, the persistent-failure case also verifies a later update is saved and the earlier update remains available for retry.

Close the superseded live query during rollback when its native id differs from the target attempt's id.
The rollback closes only the query for the corrected thread id and the adopted id. When the attempt's native id is not in the adoption map, closeLiveQueryForNativeThread skips the live query whose id differs. That query can remain open after rollbackThread returns. The rollback then restores files and returns the attempt's thread. The next turn replaces the stale query, but a running background task can block that replacement. The stale process can also run during checkpoint restore.
Close the query that is actually live, as the code does for the corrected id:

Problem
Claude
/clearstarts a new native session, but T3 retains the old session ID. After a server restart or process replacement, replies resume the pre-clear conversation and lose the context shown in the thread.Change

Verification
Latest bot follow-up at
91b52c8aee: focused tests passed, server typecheck passed, and scoped lint passed. The narrow scheduler race has no deterministic test hook; existing session-manager tests pass.Initial focused server tests: 418 passed across eight files (Claude adapter, session manager, provider switching, run execution, turn start, fork execution, checkpoint rollback, and projection store).
After merging main
6e79211743into65d8f2aa7b, all 418 tests passed again. Server typecheck and scoped lint passed; four warnings are on unchanged lines. Post-merge validation. The merge had no conflicts; main's only overlap was ProjectionStore imports.Deterministic red tests reproduced lost terminal-tail adoption, adoption during attachment and system-prompt reads, immediate restart before a new-session prompt, and historical fork/rollback. Red/green summary.
Final rollback regressions fail before the fix and pass afterward, including a late background notification and stale resume input. Final red/green summary.
