fix(server): accepted webhook deliveries survive a server restart - #23579
A webhook delivery the server answered with HTTP 202 was lost if the server restarted (crash, desktop update, npx t3 restart) before its run started. Immediate (non-coalesced) deliveries were only queued in memory behind the task's permit. After a restart, the delivery log row stayed accepted forever, the prompt never ran, and a relay replay came back duplicate.
Repro on 8d3b061121 with the dev server: one webhook task without dedupe settings, two concurrent POST /api/hooks/<task>/<token> (both 202), SIGKILL the server, restart. 75 s later both deliveries were still accepted, scheduled_task_webhook_pending was empty, and nothing had dispatched.
Change
Persisted before the 202.acceptImmediateWebhookDedupe now writes the delivery to scheduled_task_webhook_pending (state dispatching, due now, empty coalesce_key). The write happens in the same transaction that logs accepted, using the table coalesced deliveries already use.
Shared drain. The immediate path starts the per-task drain at once instead of forking runWebhookDelivery. The coalescing scheduler worker now shares that drain as drainPendingWebhooks. After a restart, the existing webhook-coalescing worker picks up the row and re-runs it with the same deterministic command id, so a re-run is idempotent.
Queue cap. It now counts persisted rows only. The in-memory webhookQueued counter is removed.
Relay redelivery. A redelivery reuses the delivery_id. releaseFailedDelivery deletes its pending row in the same transaction as the claim release, and the drain's cleanup deletes only completed rows. Together these keep a redelivery from colliding with, or being deleted by, the old row's cleanup.
Open-window flag. Queued immediate deliveries are not reported as open coalescing windows (coalesce_key <> '').
Replaced tasks. A drain stops once its task's permit has been replaced (task deleted and recreated), so it cannot take the new task's deliveries.
vp test run apps/server/src/scheduledTasks: 8 files, 82 tests passed.
an accepted delivery queued behind a run still launches after a restart failed on main with the accepted delivery was never launched after the restart and passes here.
a replaced task's old drain leaves the new incarnation's delivery alone fails without the permit check (expected 'dispatch_failed' to equal 'accepted') and passes with it.
an immediate delivery queued behind a run is not shown as an open window covers the open-window flag.
Runtime transcript, dev server on this branch, same sequence as the repro:
At SIGKILL time both deliveries were persisted as dispatching.
After restart, the webhook-coalescing worker drained both and attempted the launch.
The seeded task pointed at a missing project on purpose, so no real agent ran. Both therefore ended dispatch_failed: The run failed to start. (ThreadLaunchError … failed during resolve-project) instead of staying accepted.
Not exercised end to end: the T3 Connect relay and tunnel delivery paths. They enter the same triggerWebhook admission, and the unit tests cover relay claim release and redelivery.
Server-only change; no UI surface.
Model and harness: Claude Opus 5.5 (with Claude Sonnet 5.5 as second reviewer) in Claude Code via T3 Code.
Real server (node apps/server/src/bin.ts --base-dir <fresh tmp dir>), a valid scratch project, and real claudeAgent turns. Each row: SIGKILL the server while deliveries are queued, then restart it. Before = main 8d3b061121, after = head e3c21846c2.
Both POSTs 202, pending table empty at kill. 120 s after the restart, still accepted with 0 threads: lost (log)
Both rows dispatching at kill. After the restart both launched and completed, 1 thread each (log)
R2: two concurrent immediate deliveries behind an in-flight one
The in-flight run was cancelled by restart recovery, and r2-a/b never launched (log)
Rows at kill: running, dispatching, dispatching. All 3 completed with 1 thread each (project total 3). Re-running the running row reused its thread (log)
R3: coalesced delivery (unchanged path)
The pending row survived and produced 1 completed run (log)
Environment: fs-24, Linux 6.1.102 x86_64, Node v24.22.1. Provider claudeAgent / claude-haiku-5-5 (Claude Code 2.1.303); the prompt forbids tools and edits. Isolated --base-dir per side and row, ports 47912–47917. bin.ts has no --home-dir or --no-seed and never seeds from ~/.t3.
Verification log
Audit pass 1 (code-read matrix): audit1-matrix.md. It confirmed R1–R4 can be reproduced on a real server and R5 cannot. Concern C1: re-running a running row must not create a second thread. The after-R2 run shows it does not.
Audit pass 2: AUDIT2.md. The driver sha256 and all 11 uploads match the local copies, and the transcripts record the expected SHAs. after-R2 was rerun independently from the uploaded driver and matched on the first try (rerun log). VERDICT: SUFFICIENT.
How the queue was made: an uninstrumented race. burst.mjs posts the deliveries concurrently and SIGKILLs the server on the last 202. A running provider turn does not hold the per-task lock on either side; only the launch does, so the "in-flight run" is a launch in progress. The kill-time dumps show deliveries still queued in every capture.
Duplicates also return HTTP 202 {"deliveryId":…}. The duplicate outcome is visible only in the delivery log, which the transcripts dump.
The driver's threads_with_marker column reads 0 everywhere because of a broken text match. It is not evidence; the per-delivery thread counts and project totals are.
Unverified: R5 at runtime (it needs a launch stub that blocks, as the unit test has), and T3 Connect relay and tunnel delivery.
Model and harness: Claude Opus 5.5 (lead, audits, capture) in Claude Code via T3 Code; the captured runs used Claude Haiku 5.5.
An accepted, non-coalesced delivery that waits behind a running delivery
for the same task is only held in memory. This test closes the service
while it waits, starts a fresh service over the same database, and expects
the delivery to launch. It fails on main.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
An immediate (non-coalesced) delivery that the server answered with
`accepted` lived only in a forked fiber waiting on the task's permit. A
restart before its run started lost it, and a relay replay was then
treated as a duplicate.
The immediate path now writes the delivery into
scheduled_task_webhook_pending in the same transaction as its `accepted`
log, due now and ready to drain, and starts the existing per-task drain
at once instead of forking the run directly. The scheduler's drain picks
up anything left after a restart and reruns it with the same command id.
The in-memory queued counter is gone: the persisted rows already count
toward the per-task queue cap. Releasing a failed delivery now drops its
row in the same transaction, so a relay redelivery the release admits
never collides with or is deleted alongside the old row. Immediate rows
use an empty coalesce key and are not reported as open windows.

…new task's delivery
Every accepted delivery now runs through the per-task drain, which selects
pending rows by task id. When a task was deleted and recreated under the
same id while a drain was mid-run, that old drain could loop, pick up a
delivery the new incarnation's drain had already launched, hit the
already-running guard, and mark it dispatch_failed, releasing its replay
key and relay claim.
A drain now stops before selecting another row once its permit is no
longer the task's current one, so only the current incarnation's drain
takes rows. Rows it leaves behind stay persisted for the next drain.

Macroscope's review found this PR requires human approval — Accepted immediate webhooks now survive restarts through persisted recovery snapshots. Those snapshots also newly store raw headers and query values for ordinary deliveries, so the credential-retention implications need confirmation despite the existing redacted delivery logs.

Webhook deliveries are persisted as pending rows and processed by a shared per-task drain used by immediate and scheduled dispatch. Capacity checks use persisted active rows. The drain handles completed and failed rows, and delivery queries distinguish coalesced windows from immediate rows. Tests cover restart recovery and task replacement.

Immediate dedupe stores dispatching rows using the shared request encoder. Capacity checks count persisted pending, dispatching, and running rows. Pending-window queries require a nonempty coalesce key.
Immediate and scheduled dispatch use a shared drain under the task permit. It removes completed rows, marks failed rows before releasing them, and stops when the permit is replaced.

Tests cover queued delivery recovery after restart, pending-window reporting, task replacement, retry behavior, and completion bookkeeping.
No merge-blocking production risk is established; improve the restart test’s persistence boundary for confidence.

Explanation
The title "fix(server): accepted webhook deliveries survive a server restart" matches the main change. The PR persists immediate webhook deliveries before the HTTP 202 response, so they resume after a restart. The raw summary and objectives both describe this as the primary change. The title is a conventional commit that is short, specific, and in plain language.
Full details: Description check
Explanation
The description covers the required template sections. Problem: it explains the lost-delivery bug, the expected behavior, and a reproduction on a specific commit. Change: it lists the persisted-before-202 write, the shared drain, the queue cap change, relay redelivery handling, the open-window flag, and the replaced-task guard. Scope and approval: it links issue #18810 and cites a maintainer comment as the triage approval. Verification: it lists test results, a runtime transcript, and the parts it did not exercise. It also names the model and harness. Two items are weaker. The approval link is a reference to the issue and a comment, and the reviewer should confirm that the comment contains explicit maintainer approval of scope. The description also contains a long evidence section with external file links that the reviewer cannot check from the description alone. Neither gap makes the description largely incomplete.

Explanation
[#18810] The previous PASS remains supported. The incremental diff from the previous reviewed head (e3c2184) to head (ad40333) changes only one hunk, in ScheduledTaskService.webhook.test.ts. The restart test an accepted delivery queued behind a run still launches after a restart now calls drainPendingDispatch(workers) and then asserts that Queue.clear(launches) includes the delivery's commandId. It no longer waits on a timed launch queue. No production source changed in this increment, so the implementation evidence from the prior review still holds: acceptImmediateWebhookDedupe persists a dispatching row before the 202, the shared drain serves immediate and coalesced rows, and the startup worker resumes them. The replacement-isolation and open-window tests also remain in place. The test now checks the same restart-launch behavior through a different mechanism. Relay and tunnel paths share the admission path. Runtime verification of those paths is not established by the PR, as the PR states.
Full details: Out of Scope Changes check
Explanation
The incremental change is limited to one test hunk in ScheduledTaskService.webhook.test.ts. It updates the restart-recovery test to use the shared pending-dispatch drain. The change supports [#18810] verification and does not touch unrelated files. The whole-PR file list at head stays within apps/server/src/scheduledTasks. The prior assessment found no unrelated changes in that scope, and this increment does not alter that finding.

Fold the open-window assertion into the restart test.
This test repeats the setup of the restart test:
upsert
triggerWebhook
Queue.take
a second triggerWebhook
Its only new assertion is pendingWindow === false. Add that assertion to the first phase of the restart test, after the second triggerWebhook. The restart test then covers both behaviors with one fixture and no new it.
As per path instructions: "Hold changed tests to the rules in docs/internals/testing.md." The rule: extend existing coverage before you add a new test.

Make the restart test reopen the database.
SqlitePersistence.layerMemory keeps one live SqlClient across both fresh service layers. The test can therefore pass if a regression loses the pending row when SQLite is closed and reopened. Use a temporary file-backed database for the first service, close it, and reopen it for the second service so the test covers durable accepted-delivery recovery after restart.
