[Bug]: Codex image views no longer show in the work log #32193

Before submitting

  • I searched existing issues and did not find a duplicate.
  • I included enough detail to reproduce or investigate the problem.

Area

apps/server

Steps to reproduce

  1. Start a thread with a Codex model.
  2. Ask the agent to inspect a local image (for example "look at ./screenshot.png and describe it"), so Codex calls its view_image tool.
  3. Expand the work log / thinking section for that turn.

Expected behavior

A "Read " row with the eye-icon image preview appears for each image Codex inspected, as it does for Claude image reads and as it did for Codex before the V2 orchestrator (#4436).

Actual behavior

No row or preview appears. The agent's own text makes clear it looked at the image, but the work log shows no trace of it.

Codex reports each inspection as an imageView thread item ({ id, path }) via item/started and item/completed. CodexAdapterV2 has no branch for that item type, so both handlers return early and no turn item is projected. In a real local thread, the provider event log contains both native events for an image view, and the projection has no turn item for that native id or path.

Impact

Minor bug or occasional failure

Version or commit

Installed 0.1.6; also present in v0.1.7 and main @ 337c585. The Codex adapter has lacked imageView handling since the V2 orchestrator landed in de34391 (#2829).

Environment

Linux desktop app, Codex provider.

Logs or stack traces

# provider event log (path redacted)
{"method":"item/started","params":{"item":{"type":"imageView","id":"exec-8884864d-…","path":"<workspace>/…/ref43-compare.png","startedAtMs":1793850048094,"completedAtMs":null},…}}
{"method":"item/completed","params":{"item":{"type":"imageView","id":"exec-8884864d-…","path":"<workspace>/…/ref43-compare.png","startedAtMs":1793850048094,"completedAtMs":1793850048094},…}}
# orchestration_v2_projection_turn_items for that provider turn: 260 rows, none matching the id or path

Workaround

None in the UI.

A fix is proposed in #31580.
