feat: surface exact SCRATCH protocol details in DREAM plans

Summary

• sync the pinned DREAM protocol snapshot with the verified SCRATCH tranche
• render exact protocol presentation, SPT/IDT dilutions, preparations, review notes, pharmacy warnings, and SCRATCH links in the builder, print view, and copy/email text
• fail closed on malformed saved protocol indices and protect the frozen positional baseline with independent hash and append-only guards

Clinical safety

• no source dose, concentration, dilution, ratio, volume, or preparation values were interpreted or changed
• under-review records remain marked for review
• invalid or empty restored protocol selections block Preview/Print until a valid option is chosen
