Keep approval inspection, stale journal cleanup and URL publication in one write section so a concurrent atomic record replacement cannot escape as an unhandled bounded-read failure. Co-authored-by: xianlubird <xianlubird@gmail.com> Co-authored-by: Matt Van Horn <mvanhorn@users.noreply.github.com>

Propagate the late opt-out as a skipped injection
When the Codex switch turns OFF after syncModelsToCodex's initial gate but before publication, returning false here is indistinguishable from “this provider was never approved.” injectCodexConfigImpl consequently follows its unapproved-external-provider branch and returns a successful applied result instead of status: "skipped"; syncModelsToCodex then preserves any catalog/cache-write flags, so ocx sync can restart Codex and report that the user should approve a provider whose consent is still present. Return a distinct desired-state skip result and propagate it through the injector, with a caller-level regression for this race.

Avoid locking validation-only provider checks
This unconditional mutation-lock acquisition also covers validateOnly, even though apply returns at line 139 without reading intent or publishing anything. The sync preflight calls this path with validateOnly: true; because the SQLite mutation lock uses busy_timeout=0 and injectCodexConfig does not translate ConfigMutationLockError, any concurrent configuration write now aborts an otherwise read-only ocx sync preflight. Keep the mutation lock around the actual desired-state recheck and publication path, but retain the previous lock-free behavior for validation-only and already-current returns.
Useful? React with 👍 / 👎.
Concurrent fail-fast lock contenders can all return retryable conflicts. Assert that such a round preserves the preimage, then require one uncontended sync to publish before checking crash-restorable output. Only real approved-provider writes acquire the config mutation lock. Read-only validation and unapproved provider inspection retain the Codex file lock without creating a persistent config mutation database. Make the race test reach the bounded reader before adding a one-shot guard.
injectCodexConfigImpl calls injectApprovedCodexProvider, which reads the approval record inside withConfigWriteLockHeld. The test replaces the record only when .ocx-write.lock is absent, so this callback cannot fire on the normal path.
The callback also spies on the node:fs namespace, while readBoundedCodexConfig uses a named statSync import. The spy therefore does not intercept the reader. The test can pass without exercising the atomic replacement.
Install a test seam that controls the bounded reader’s actual statSync call, trigger the replacement at the first record observation, and assert that the hook fired. Add a fired guard only if the corrected hook can receive multiple observations.

A preceding writer can atomically replace config.toml and release between destination lookup and lock acquisition. Observe its committed inode under the acquired lock while retaining canonical-retarget and live-handle checks. Keep strict publication guards for changes after acquisition. Maintainer landing note: PR checks are green on this head. The concurrent sync regression (codex-provider-routing "concurrent sync processes leave a complete restorable config") passes here in hosted Linux and in the macOS control job of the branch dispatch (run 38890799907). A final lane=all on the dev tip follows. |
