Summary

  • Avoid validation/read pathname races and FIFO blocking in LazyCodex role-mirror loading. Based on dev at e0af52c8a2701dccd81fa5e672c92744e59d2e29.
  • Bind LazyCodex role-mirror validation and reads to one descriptor, reject non-regular or substituted files, and close the descriptor in finally.
  • Preserve POSIX O_NOFOLLOW | O_NONBLOCK, bounded FIFO read/write regressions, symlink substitution coverage, and unreadable-route handling. Windows retains descriptor/path checks without claiming POSIX no-follow semantics.
  • Compare device and inode identities with bigint stats so distinct values above the safe-integer limit are not treated as equal. Preserve the existing route-test context for unreadable-file handling.
  • The precision fix, its regressions, and local validation limits are documented in the validation follow-up. The exact-head CI and review follow-up records evidence for 6e9a6b76ae46da069c4d4f4584a37861fd5612da; the initial-tree results below remain historical evidence.

Verification

Linux / Bun 1.4.0, exact tree e45e4cab81e2d4d78d4909599348c0ba83989216:

  • bun test tests/clients/omo-role-models.test.ts tests/server/codex-agent-role-routes.test.ts: 19 pass, 0 fail, 93 assertions.
  • Negative control: removing only O_NONBLOCK makes both FIFO cases hit their bounded three-second deadlines. Source restored afterward.
  • bun node_modules/typescript/bin/tsc --noEmit, bun scripts/structure-ssot.ts, bun scripts/privacy-scan.ts, bun scripts/file-size-ratchet.ts, and git diff --cached --check: pass.
  • The import-connected changed-test run was stopped because it attempted an external provider request outside the offline validation scope. It is incomplete and is not passing evidence. The earlier concurrent attempt was interrupted during shared-runner contention.
  • Focused validation substitutes for a full local suite because the broad selector crosses live-provider integration paths and the shared environment is resource-constrained. No full-suite or native Windows/macOS pass is claimed; the POSIX FIFO cases skip Windows. Hosted checks associated with PR head 7bf3553689d03071f9a9e12edad125ebb40f14d3 have passed: Cross-platform CI, including its ci aggregate, and Enforce PR target branch, attempt 2. The latest check inventory is 21 successful and 9 scope-skipped jobs; skipped platform lanes are not claimed as tested.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.

Summary by CodeRabbit

  • Bug Fixes
    • Configuration files that are symlinks or special files are rejected as invalid, preventing accidental reads or writes to symlink targets and avoiding indefinite waits on pipes.
    • Files whose paths no longer match the opened file are rejected, and missing files continue to be reported as absent. Unreadable files retain their existing error handling.
  • Documentation
    • Clarified file validation behavior across supported platforms, including differences in symlink handling.

Carry luvs01#698 onto current dev, preserving its implementation and regression coverage.

📝 Walkthrough

Walkthrough

The role-model loader now opens files through a descriptor, checks that the descriptor and path identify the same regular file, and reads through that descriptor. Tests cover symlink replacement, device and inode identity collisions, FIFO paths, and access-denied handling. Documentation describes platform-specific behavior.

Layer / File(s) Summary
Open, validate, and read role-model files
src/clients/omo-role-models.ts, structure/clients/integrations.md, tests/clients/omo-role-models.test.ts, tests/server/codex-agent-role-routes.test.ts
The loader validates and reads through one descriptor. On non-Windows platforms, it opens without following symlinks and without blocking on FIFOs. Tests cover symlink replacement, device and inode identity collisions, FIFO reads and writes, and EACCES handling.

Priority: ⬇️ Low

Estimated code review effort: 3 (Moderate) | ~20 minutes

Change: Bug fix

Merge Risk: ⚪ Minimal · up to 6e9a6

The role-model loader changes appear ready to merge after normal checks. No actionable merge-blocking risk remains in the supplied evidence.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 6e9a6

The change strengthens role-configuration reads without expanding the observed management API’s access or privileges. No introduced security concern was established. Windows special-file behavior and deployed access boundaries remain incompletely validated.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The affected sink is the process-user role-model mirror, alongside the route’s authoritative Codex role file. Exploiting pathname substitution requires influence over the relevant filesystem namespace. No expansion of request-selected paths or caller authority was identified against the named dev baseline; deployment-level environment and identity control remain unknown.

Trust Boundaries and Controls

  • observed — POSIX opening rejects final-entry symlinks and avoids waiting on FIFOs before type validation. Descriptor-bound reads prevent later pathname replacement from redirecting the read. These checks establish file type and path identity, not trusted content provenance or protection against every parent-directory race. Windows does not receive the POSIX flag guarantees.
  • observed — Mirror persistence retains integration ownership enforcement and atomic temporary-file replacement. The no-follow write helper replaces the final directory entry rather than following a substituted final symlink. These write controls are unchanged, and their point-in-time checks do not make the entire read-modify-write sequence transactional.

Resilience and Maintainability Implications

  • inferred — Atomic replacement contains torn-write failures but does not prevent concurrent snapshot writers from losing updates. Repeating the same mirror model is idempotent, and a repeated route request still attempts mirror synchronization after an unchanged TOML result. Interruption between the two writes can leave the mirror stale; these limitations predate the PR rather than constituting introduced concerns.

Published the precision fix in 6e9a6b7.

The production delta from 7bf3553 is limited to requesting { bigint: true } from both fstatSync(fd) and lstatSync(path). Descriptor-bound reads, regular-file checks, POSIX no-follow/nonblocking flags, and finally cleanup remain intact. Synthetic regressions cover distinct dev and ino values (2^53 and 2^53+1) that collide as numbers: both read and write reject before reading, close the matching descriptor, and preserve fixture bytes.

Verification on Linux / Bun 1.4.0, exact tree 8c75ee11a927b52b48f734bdf1198b3c5e3ff618:

  • bun test tests/clients/omo-role-models.test.ts tests/server/codex-agent-role-routes.test.ts: 21 pass, 0 fail, 109 assertions, including existing symlink/FIFO and regular-file read/write behavior. Both new precision cases failed with the original numeric-stat implementation and passed with the fix.
  • bun run typecheck, bun run structure:check, bun run privacy:scan, bun scripts/file-size-ratchet.ts, and git diff --check: passed.
  • Independent static review of the exact diff: no findings. This is not a maintainer approval or a Windows runtime test.

The import-connected run against the previous head is failed and incomplete, not passing evidence: its partial log contained 5,905 pass lines and 205 failure lines across 28 files, then it was interrupted after roughly four minutes while a provider-management test remained running. Five selected failures reproduced identically on the previous head and this candidate: coordinator teardown ownership refusal, Desktop cleanup assertion, fixture-permission assertion, local catalog HTTP 403, and a mocked-upstream 5-second timeout. Read-only diagnostics found a non-root-owned system temporary directory and umask 077. A separate credential-free local fixture returned 403 Domain forbidden for 0.0.0.0 without reaching its handler, while 127.0.0.1 reached the same handler and returned 200. These support environment explanations for the representatives; not all 205 failures have proven root causes. Remaining HTTP/capture assertions, discovery-capable timeouts, and other assertions retain uncertainty. No full-suite or Windows/macOS pass is claimed; no security settings were changed.

Publication used the existing GitHub connector because Git HTTPS authentication was unavailable. The connector commit has the same complete tree as tested local candidate 6931e76b8b0915ed03a048278b573dd22d261212, exactly one parent 7bf3553689d03071f9a9e12edad125ebb40f14d3, and was applied with force: false; both the branch and PR head were read back. New-head hosted CI and CodeRabbit review are being checked. Older-head CI is not evidence for this head, and this update does not assert merge readiness.

Exact-head follow-up for 6e9a6b7:

  • Cross-platform CI completed successfully, including all four test shards and the ci aggregate. React Doctor and enforce-target also passed. The latest-per-name check inventory is 21 success / 9 scope-skipped / 0 pending or failing. Skipped Windows/macOS full-test lanes are not claimed as tested.
  • CodeRabbit's actual new-head review completed with no actionable comments and a successful status. It marked the precision finding addressed in 6e9a6b7, and that thread is resolved. Its separate docstring-coverage warning remains; no unrelated source change was added for it.
  • The local broad-run limitations and baseline comparisons still apply; hosted success does not establish individual root causes for every local failure.

The PR remains open and Ready. No merge, auto-merge, force-push, deployment, or security-setting change was performed. This records completed validation and review evidence, not a claim that all maintainer or merge requirements are satisfied.

Superseded by the integration in #6487, with reviewed follow-up fixes in #6490 and Windows validation repairs in #6494/#6495, all merged into dev.

Descriptor-bound, nonblocking role-mirror reads and exact file-identity checks were carried.

Original carry commit: b2d27e35a263817bb0b115dac5a97ab7f9206f4a. Attribution to @luvs01 is preserved in the integration history and merge trailers. The final integrated candidate passed the complete cross-platform CI run.

Closing this PR as superseded, not claiming that its original head was merged. Thank you for the contribution.
