Release codexclaw 0.2.40

Version bump and CHANGELOG for 0.2.40, which ships the 2026-09-27 issue train (#269-#272) and the 2026-09-30 issue train (#278-#280) already merged into dev.

• Versions: package.json, package-lock.json (13 workspace entries), cli, gui and the nine component packages -> 0.2.40; plugin.json -> 0.2.40+codex.20260929183231; inventory.json regenerated. check-versions.mjs 0.2.40 is OK. The only remaining 0.2.39 is a fixture cache path in pabcd-state/test/hook.test.ts:181.
• CHANGELOG: [Unreleased] becomes [0.2.40] - 2026-09-29, with entries for interview: distinguish assistant-inferred assumptions from user-confirmed requirements at handoff #275, dispatch: receiptSatisfiesPacket accepts unrelated verifier result for multiple required commands #276, dispatch: optionally declare verifier write effects before shared-read execution #277 and the loop: link pending user decisions to goalplan work phases so blocked-on-decision work is visible and each question is asked once #262 options, plus Compatibility and Verification sections.

Local: npm run build, npm test (3737 tests, 0 failures), inventory, gate.mjs, platform-smoke.mjs. No hook file or hook handler changed since 659de59b; the manifest changed only its version line.

After this merges: promotion PR dev -> main, then release.yml with the audited main SHA.
