pin codes and file access permissions cycle for CMS/LMS users on every conference entry

Moodle and Elgg users experience pin code and file access permission cycling 
because of the way ephemeral users are creating as documented here:

Need to determine how frequently this is a problem in common use cases (how 
often will elgg / moodle users dial in using pin code?) -- perhaps the solution 
is to make those features hidden.
