feat(launch): describe pooled Hub models in harness pickers - #120

Hub catalogs now expose one id per pooled model, such as @you/qwen3:8b, however many relays serve it. Launch pickers label each model with the Foldwise hub and the pool serving it, Claude Code's modelPicker uses its options shape, and docs and examples use pooled ids.
