feat(invitation): accept Hub invitations for memberships and grants - #116

Conversation
The Hub now offers org membership and keg grants as invitations that take effect only when accepted. tap namespace add-member and tap keg grant report whether a change was immediate or an invitation, and tap invitation lists, accepts, declines, and withdraws them. MCP gains a read-only invitation_list tool. Accepting access is the account holder's consent decision, so accept and decline stay off MCP; the parity exemption records why.
