[Bug]: a schema-adding Better Auth plugin on the bundled SQLite file is refused with a remedy (urlcode-auth migrate) that cannot work, while test and audit pass #1140

Description

@jimhoyd

Runtime revision and environment

main after #1135. Reproduced by the #1137 assessment, in the fourth test of test/account-composition.test.ts.

Minimal reproduction

Configure the auth extension with a Better Auth plugin that adds schema (for example admin) through auth({betterAuth: {plugins}}), on the bundled SQLite file. Activation refuses and tells the operator to run npx urlcode-auth migrate. That CLI cannot help: it does not read host.mjs, so it never sees the plugin and cannot create the plugin's tables.

urlcode test and urlcode audit pass regardless, because hermetic runs migrate with the plugin loaded. So a project can look ready and still refuse to serve.

Expected, one of:

  • urlcode-auth migrate accepts the host file (--host-file) and migrates with the configured plugins.
  • Activation refuses with a remedy that actually works (an owner database migrated with Better Auth's own tooling), and audit reports the mismatch instead of passing.

Relevant output

The PR for #1137 improves the refusal text and the README; the underlying behaviour is unchanged.
