docs: file each migration under the release that shipped it - #77

Four changes listed as Unreleased shipped in 2.31.0, and the Kafka group-id change shipped in 2.30.2. Consumers read this page to learn what a bump changes, so it names the release now.

docs/migrations.md listed everything under Unreleased, but all of it has shipped. A consumer moving to 2.31.x reads this page to learn what changes, so it should say which release each change arrived in.
