docs: clarify ep_plugin_helpers helper name in AGENTS.md#95

The auto-generated AGENTS.md listed ep_plugin_helpers/pad-toggle and ep_plugin_helpers/pad-toggle-server as if they were two separate helpers. They're sub-paths of the same canonical helper, padToggle — the client and server entry points respectively. Re-label so a future agent can find the helper API by name.
