fix(chat): add bounded diagnostics for conversation fallbacks - #2292
ConversationUnavailable intentionally returns HTTP 200 with a scrollback
fallback, so ordinary HTTP status logs cannot distinguish a native conversation
from an ownership rejection. An intermittent fallback could recover before its
actual rejection stage was captured.
Change
Add static Copilot rejection-stage metadata without changing existing
exception messages, ownership decisions, or HTTP response bodies/statuses.
Log fallback, pending-read and native-recovery episodes, including recovery
through an unchanged 304 response.
Reuse bounded connection-diagnostic episodes and their per-key 30-second
throttle; additionally cap all conversation output at 16 lines per 30
monotonic seconds, even when pane keys rotate.
Only enumerated reason labels, opaque episode IDs and validated request
correlation leave the logger. Pane/session IDs, paths, argv, transcript text,
exception text and credentials are not logged.
The refined stages distinguish failed process RPCs from successful empty or
ambiguous process sets, invalid reports, unproved handoffs, peer conflicts and
missing foreground/resume evidence. Deeper log-reader rejection subtypes remain
grouped as unproved foreground evidence.
Validation
Initial diagnostics, shared episode logger, pane routes and Copilot suites: 105 tests passed, 869 assertions, including preventing orphan recoveries
for globally suppressed new failures.
Typecheck passed; independent review reran both diagnostic suites
(18 tests, 47 assertions) and found no substantive issues.
Response regressions preserve HTTP 200 fallback, 503/no-store pending reads,
and recovery through HTTP 304.
The full fast run completed with 8,387 passing tests, one failing test and
34 skips. Its sole failure was the unchanged installer's wall-clock duration
assertion: 18,488 ms versus a 19,000 ms minimum. Workflow syntax, generated-file
checks, typecheck and build all passed, and the report verifies unchanged source
throughout the run. Both the installer and its test are byte-identical to the
previously instrumented baseline that demonstrated a backward wall-clock step
during a real 20-second wait. No assertions, timeouts, clocks, permissions or
trust settings were changed. This is not a successful full-fast result.
This PR is independent of the stable-process-identity fix and does not retain
history when ownership cannot be proved.
Review corrections
Maintainer commits edb82ec0 and 2ddad703 are preserved by normal merges,
without rewriting history. They correct expected-shell logging, preserve
episode transitions when the global output budget is spent, and distinguish an
agent pane with no folder from an ordinary shell. The pending-response test
also now derives the canonical exception message instead of pinning its wording.
The reviewed handoff follow-up separately classifies a logged session claimed
by a peer as copilot_peer_conflict, while preserving the rejection and its
existing no_session_path message. Its contract assertion fails before the
correction.
The clean 063afe43 isolated focused run, including native Copilot contracts,
the shared logger, diagnostics and routes, passed 115 tests. Independent
follow-up reviews found no substantive issues. These results used the
repository's check run isolation; earlier direct parallel contract runs shared
the default test session and are not used as final evidence.
The final full-fast attempt on clean 063afe43 finished with 8,392 passes,
one failure and 34 skips. Only the same unchanged installer duration assertion
failed (18,467 ms versus 19,000 ms); workflow syntax, generated-file freshness,
typecheck and build passed. Its report confirms unchanged source and correctly
does not claim successful overall verification. No test or timing control was
changed to bypass this failure.
Added diagnostics for conversation unavailability and recovery, including clearer identification of Copilot-related failure stages.
Diagnostic records are sanitized and rate-limited to help keep logs bounded; they omit session IDs and transcript paths.
Pending conversation reads continue to return a retry response, while other unavailable conversations retain the existing fallback behavior.
The change adds specific Copilot failure reasons and bounded diagnostics for conversation failures and recoveries. Pane routes emit diagnostics for unavailable, pending, and successful reads while retaining their existing response behavior.
ConversationUnavailable accepts allowlisted Copilot diagnostic reasons. Transcript resolution assigns specific reasons to process lookup, process validation, handoff, peer conflict, and foreground or resume failures. Tests assert the process lookup and process-set reasons.
Bounded conversation diagnostic records server/connection-diagnostics.ts, server/conversation-diagnostics.ts, server/conversation-diagnostics.test.ts
Conversation diagnostics classify pending reads and other unavailable errors, report recoveries, and limit output to 16 lines per 30-second window. Connection diagnostics accept conversation events and UUID request IDs. Tests cover record mapping, coalescing, bounds, timing, and writer failures.
Pane routes record diagnostics for unavailable, pending, and successful conversation reads. Existing fallback and pending-read responses remain in place. Tests check response behavior and diagnostic records.
devswha/ovis#2068: Establishes the pending-read error and 503 response that this change instruments.
devswha/ovis#2283: Adds the process lookup retry whose final failure now receives a specific diagnostic reason.
devswha/ovis#1955: Defines the two-process handoff branch that this change labels with a specific failure reason.

Thank you, @YuuuXie. A strict review found no privacy leak and response bytes identical to main, but three medium issues that this PR introduces. Since the branch allows maintainer edits, the merge pass fixed them in edb82ec (on top, no force push):
A plain shell pane was logged as a conversation failure. Every pane without an agent answers no_recognized_transcript by design, so 20 ordinary shells logged a conversation.failed every 30 s each and used up the whole 16-line budget, which would hide real Copilot failures. That reason (without a Copilot stage) is now treated as the expected view: it logs nothing, and it closes an open episode if the pane's agent left.
The line cap dropped state, not just output.unavailable()/recovered() returned before touching the episode when the budget was spent, so fail -> success (or 304) while the budget is full -> fail merged two episodes. The budget now lives in the writer: ConnectionDiagnostics lets a budgeted sink decline a line (write returns false), the failure then stays unseen (so no orphan recovery) and is counted as suppressed, and episodes open and close regardless.
A new test pinned the 503 message's wording. It now compares against new ConversationReadPending().message.
Tests: a route test drives a real shell pane through the conversation resolution path (no diagnostic line), and unit tests cover separate episodes under a spent budget and the shell reason; all four fail on the previous head. The rotation test now expects a recovery for a key retired from the bounded map to log nothing. bun run check fast passes.
Thanks for the corrections in edb82ec. I am incorporating that commit without rewriting history, together with the reviewed handoff peer-conflict diagnostic correction and its isolated contract regression. I will preserve your changes and publish the follow-up after validation; the production candidate has not been promoted.
Preserve the existing rejection and HTTP behavior while recording the proven conflict stage. Add a contract assertion that failed before the correction.

One more fix in 2ddad70 (on top, no force push). The re-review confirmed all three issues resolved, but found that my shell rule was too broad: no_recognized_transcript is also thrown when herdr gives a pane no folder (conversation.ts, before the agent is resolved), so a Copilot pane with an empty cwd was treated as a shell, logged nothing, and closed an open Copilot failure episode as recovered. That throw now carries a fixed agent_cwd_missing diagnostic when the pane names an agent, so it is logged as a failure and keeps its episode; a pane with no agent stays silent. A route test drives a Copilot pane with an empty cwd through the real resolver, and a unit test checks the episode; both fail on the previous head. bun run check fast passes.
Both maintainer corrections (edb82ec and 2ddad70) are preserved locally via normal merges. The previously prepared handoff peer-conflict label correction also has a regression that fails before the change. The final isolated focused run now passes 115 tests; the clean full-fast attempt is still running. I will recheck the live PR state/head immediately before any publication and will not recreate or push to a branch if the PR merges first.
Merging 063afe4 (external, @YuuuXie). The merge pass read the whole diff before approving CI; a strict reviewer found no privacy leak and response bytes identical to main, and three medium issues the PR introduced, which the merge pass fixed on the branch (edb82ec, 2ddad70; see the comments above). The final head passes re-review with no findings. Conversation fallbacks (ConversationUnavailable, scrollback with HTTP 200) and pending reads (503) now log a bounded, sanitized episode (fixed reason code and server request id; no session ids, paths, pane ids or exception text), with a recovery line when the pane reads again (including a 304), and each Copilot ownership rejection names its stage. Verified: 20 plain shell panes over 1,200 polls log nothing; a Copilot pane with no folder is logged as agent_cwd_missing and keeps its episode; the 16-lines-per-30-s cap bounds output without merging separate episodes or logging a recovery for an unseen failure; other users of ConnectionDiagnostics behave as before; route responses match main byte for byte. Related tests (110) and typecheck pass. @YuuuXie then added 44ef36f, so a handoff session a peer already claims is logged as copilot_peer_conflict (CodeRabbit's point; same response and denial), with a contract check; the merge pass read that delta. Thank you, @YuuuXie.
## Release 0.4.20
The release PR, prepared by step 1 of
[docs/development.md#releasing](https://github.com/devswha/ovis/blob/main/docs/development.md#releasing),
initially on main `bd676637a16471e8744d62c7311026ddb43a39eb` (#2335,
xterm 6), then merged with final-scope main
`ef482f2e66b3344658fb548fa1110744a92929c9` (#2338) and test-only
follow-up main `269e9ce19558b8d197959250075da56b25b9668f` (#2345):
- **Version:** `version` is 0.4.20 in `package.json` and
`herdr-plugin.toml`.
- **Changelog:** `bun scripts/changelog-fold.ts 0.4.20` initially folded
122 fragments into the 2026-10-29 section of `CHANGELOG.md`, linked
their PRs, and deleted the fragments. After merging main, the modified
`letta-native-chat.md` from #2338 was folded by hand with its current
wording and PR link. No other late fragment landed; `changelog.d` holds
only `README.md`. Four additional entries for external PRs without their
own fragments bring the section to 157 entries.
- Added 22 `by @login` credits across all 19 outside-contributor PRs in
the release range. Preserved the existing credit to @Yoonwoo-Ha for
#1634. See the complete ancestry-based audit below.
- **Release summaries:** en, ko, ja and zh each have 5 new, 5 improved
and 5 fixed lines, matching 0.4.19's shape. They include the xterm 6
improvement: terminal frames no longer mix old and new output. Every
English line is at most 80 characters.
- **Remote bundle:** `REMOTE_BUNDLE_VERSION` rises from 38 to 39 because
the bundle inputs changed since remote-v38.
- **Telemetry:** the guard confirms no Worker deploy or deployed
fingerprint update is needed.
- **iOS upload note:** CURRENT_PROJECT_VERSION raised to 3 for App and
OvisNotifications (last uploaded: 2, source: `docs/native-app.md:99`,
commit `54fdadc1c65da5d50a6afef411e3ed8f23b6c255`). Both targets inherit
project-level Debug and Release settings, and every
`CURRENT_PROJECT_VERSION` entry is 3. `app/store/ios.md` now records the
prepared build. This is the last documented upload, not a live App Store
Connect query; check for later uploads before uploading. The marketing
version remains 1.0.
- The changelog explicitly distinguishes delivery: removing WebKit's
form bar (#2109) requires the new iOS app build, while Korean
composition (#2307) comes with the PC update and needs no new iOS build.
The Korean-input summary says the same in all four languages, with no
change to list counts.
## Validation
- Final `dori heavy release-check -- bun run check fast`: passed, exit
0; workflow syntax, generated types/themes, typecheck, build and unit
suite (8,591 pass, 0 fail, 34 platform skips in 549 files). This is the
merged tree containing #2338, #2345, the contributor corrections and iOS
build 3.
- `bun test scripts/changelog-fold.test.ts
server/release-notes.test.ts`: 33 pass, 0 fail.
- `bun scripts/release-notes.ts 0.4.20`: passed, with all patch-note
lists and the full changelog.
- `bun scripts/release-guards.ts` and CI mode with `--base
ef482f2`: passed on the final merged
tree: "Release 0.4.20: remote bundle, telemetry Worker and release
metadata are in order."
- `bun scripts/release-guards.ts --release 0.4.20`: expected exit 1 only
because "remote-v39 is not published." The merge pass publishes it after
merging.
## Merge pass
**Do not merge before the owner's OK.**
- [ ] Merge only after the owner's OK and required checks pass.
- [ ] After merging, push `remote-v39` on the merged commit and wait for
the **Remote PC bundles** workflow to publish the manifest and all
archives.
- [ ] Then run **Release** on `main` with version `0.4.20`, without `v`.
The Release workflow creates the app tag; do not push it by hand.
- [ ] From this PR's merge until the Release run is done, merge nothing
touching `server/`, `shared/`, `bun.lock` or `patches/`.
- [ ] A changelog fragment merged after this PR must be moved into
0.4.20 by hand.
This lane stops at the open PR. No tag, workflow dispatch, merge or
telemetry deploy was performed.
**Completed release follow-up:** merged main `ef482f2e` including #2338
(`fix/letta-no-live-box`), folded its revised fragment into 0.4.20, and
retained the contributor-credit commit `184dc120` and iOS-build commit
`0ffa3d45`. The Letta summary already describes conversation support
without promising a live-terminal box, so it needs no change.
Merged test-only #2345 from main `269e9ce1`: the two usage browser
checks now wait for the earlier shared account-order save and start the
sign-in check from a cleared order. No product behavior or late fragment
was added; `changelog.d` still contains only `README.md`. The iOS
delivery clarification and resolved review thread are retained.
## Complete external-contributor audit
Checked every merged PR whose merge commit is in `git log
v0.4.19..origin/main`: 133 PRs through final-scope main `ef482f2e`, or
134 including test-only #2345 on `269e9ce1`, with 19 PRs by 12 external
non-bot authors. Both rescans found no additional external-author gaps;
#2345 is by the maintainer. Added the four missing entries: #2271 and
#2274 by @CharleoYang, #2290 by @DravenDou, and #2282 by @fivetaku. The
tab-reopening summary now includes the history-page improvement in all
four languages; list counts remain 5/5/5.
Full external PR list checked: #1911@jjsteffen; #2068@YuuuXie; #2072@Noobear; #2087@JJLiebig; #2145@mattmarcin; #2176@jpcarranza94; #2231@philipprass-prog; #2235@vuongnmhvn; #2270, #2271, #2274@CharleoYang;
