deriveStackManifest kept example.env or inline ${VAR:-x} defaults on
secret variables and treated ${VAR:-x} secrets as optional, so stacks
like weavestream, plausible, and immich shipped with placeholder database
passwords instead of fjord generating random secrets. Clear the default on
secrets and only mark a secret optional when docs.env says so or its
fallback chains to another variable.
