Add flu vaccine claim page and implement questions for it

The flu vaccine service page should be added and should have the following questions:

• Where is the vaccine to be received?
• Date of birth
• When is it to be received?
• Have you received at least two flu vaccines previously?
• How many flu vaccines have you received this year?
