Release of refactor into main (owner OK'd 2026-10-19).
Includes:
Database: one new nullable column users.notes, added automatically at startup by _add_missing_columns. TAVILY_API_KEY is already set on lumen-api-sg. After deploy: re-run the 30-question audit (tax C23 should now be right).
