Description:

The live site was hit with password-reset spam on 2026-11-03. An audit of the account forms found that resend-verification had no limit. It also found that sign-up told anyone whether an email had an account, and that each form had its own hand-written limits, so a new form could ship unguarded.

Protection (security.clj)

  • One policy: auth-guards is the policy for every account route, written as data: limits, Turnstile mode, answer style, or {:none "reason"}.
  • One guard: guard enforces the table, and record! counts login failures. Handlers in routes/auth.clj contain no limit numbers and make no direct checks.
  • Removed: the per-form rule functions and login's older tracking code. Login now counts through the same store as the other forms.
  • Ordering: a failed challenge never uses up an allowance, and no refusal ever comes from an address lookup.
  • Route test: a route-table test fails if an auth route is missing from the table, or if the table names a route that doesn't exist. A pin test fixes the limits that login, register, re-verify and reset must have.
  • Resend verification is now limited the same way as reset: 3 an hour per address, 10 an hour per host, always answering 200.

Turnstile:

  • Off by default: nothing changes unless TURNSTILE_SITE_KEY and TURNSTILE_SECRET_KEY are both set.
  • Where it runs: on sign-up, reset and resend every time. On sign-in, only after a failed attempt, tracked by typed name plus connection, so it reveals nothing.
  • Fail open: the server verifies the token with Cloudflare and lets the request through if Cloudflare can't be reached.
  • Blocked script: the form tells the visitor to allow challenges.cloudflare.com in their ad blocker.
  • Browser side: auth-form-page :challenge places the widget, and two shared interceptors attach the token and reset the widget for every form.

Sign-up never reveals a taken email:

  • check-email is removed.
  • A taken address gets the same answer as a new one, after the same checks, with the password hashed so timing matches.
  • The owner of that address is emailed instead.
  • With no mail server, a taken address gets a plain refusal (owner decision).

Notices: a single claim-notice! handles the sign-in-attempt notice and the taken-sign-up notice. The first is sent immediately; follow-ups go out only while attempts continue, no sooner than 6 hours apart, with a count. They never contain a credential, a username or a reset token.

Code moves (no behaviour change):

  • Handlers moved from routes.clj to routes/auth.clj: 2927 → 2046 lines.
  • Screens and the widget moved from views.cljs to views/auth.cljs: 12426 → 11695 lines.
  • Events moved from events.cljs to events/auth.cljs: 8928 → 8542 lines.
  • The page wrappers moved into index.clj.
  • The auth card's inline styles moved into Garden; 20 before/after screenshots are identical.

Checks:

  • Tests: lein test runs 1476 tests with 0 failures.
  • Lint: 0 errors.
  • Build: fig:prod compiles.
  • End-to-end: auth-flows.js passes 43/43 (with Turnstile off).
  • Browser: 18/18 checks pass with the Turnstile test keys and a mail sink.

Docs: docs/kb/auth-abuse-limits.md and the handoff on agents/develop.

Related issue (if applicable): none

Summary by CodeRabbit

  • New Features
    • Added optional Cloudflare Turnstile checks for sign-up, password reset, verification-email requests, and sign-in after failed attempts. Checks are enabled when configured and fail open if Cloudflare is unreachable.
    • Updated account screens with password-strength guidance, confirmation checks, and clearer validation and security-check messages.
    • Added account-owner email notices for sign-in attempts and registration attempts involving an existing address, with limits on repeat notices.
    • Added optional Redis-backed sharing of abuse limits across app containers, with in-memory counting when Redis is unavailable.
  • Bug Fixes
    • Sign-up responses no longer reveal whether an email address is already registered.
    • Added a startup warning when the account-email link URL is not configured.

Managed mode, sign-up form only. The server checks the token before the per-host
sign-up allowance, so a bot that fails the check does not use up a real visitor's
signups. If Cloudflare cannot be reached the signup goes through. Off unless both
TURNSTILE_SITE_KEY and TURNSTILE_SECRET_KEY are set; the secret is never printed.
…tes.auth

default-title, default-description, default-image-url, index-page-response,
default-index-page and index leave routes.clj for index.clj, which already
holds index-page; routes.clj refers them so call sites are unchanged.
reset-password-page moves to routes/auth.clj (its auth/ prefixes dropped).
Tests retargeted.
registration-content-style, registration-left-column-style and
registration-header-style become .registration-content (background, border,
colour), the existing .registration-left-col and .registration-header. Two
named colours, auth-card-ink and auth-banner-ground, replace the literals.
Computed styles and screenshots at 1280x900 and 390x844 are unchanged.
The password meter's fill width stays inline: it is the measurement.
:turnstile-reset returns :orcpub.dnd.e5.views.auth/turnstile-reset, registered
in views/auth, so events/auth no longer requires views/auth.
security/auth-guards is now the policy as data (limits, challenge, answer style per
route) and security/guard and record! are the only code that reads it. The per-form
predicates, the separate login counters and the turnstile-refused? helper are gone;
login counts through the same sightings. Handlers name their route and pass what
they know. Tests iterate the table instead of repeating per form.
Sign-in-attempt and taken-sign-up notices go through one table-driven claim-notice!:
the first at once, then only when new attempts came in, at least 6 hours apart and
at most 4 in a rolling day. Every attempt is counted and a follow-up says how many
came in since the previous notice. Still pages only, no credential, sent off-thread.
…ection

auth-form-page takes :challenge (:always or :after-failure) and places the Turnstile
widget and its blocked-script message itself; pages no longer place a widget. One pair
of interceptors in events/auth attaches the current token to register, login, reset and
resend requests and resets the widget when they fail.

Walkthrough

This pull request extracts account operations into a dedicated authentication namespace and adds optional Turnstile checks and shared abuse-limit storage. It updates authentication routes, client events and forms, deployment configuration, and tests.

Changes: Account authentication and abuse controls

Layer / File(s) Summary

Adds optional Turnstile settings and CSP sources. Security policies apply route-specific limits and challenge checks. The default store uses memory; Redis can share sightings and notice state across app containers.
Account handlers and route integration

Resend verification still tells a caller which addresses have verified accounts, which conflicts with the PR's goal of not revealing account existence. The fix is one line and a test case. The other open items are small UI and test-quality issues that do not block the account flows.

Failed Turnstile tokens on account forms reach Siteverify with no bound.

On :turnstile :always routes, security/guard runs challenge-refusal before any allowance is spent. Each nonblank token that fails calls Cloudflare Siteverify synchronously, with a 4000 ms timeout. Nothing in the route path bounds these calls, so repeated invalid tokens can hold Jetty workers and outbound connections. Limit counts are not consumed on failure, so the per-address and per-host allowances do not stop this traffic.

Review comment at @src/clj/orcpub/security.clj around lines 392 - 441:
Bound Siteverify calls for `:turnstile :always` routes by adding an appropriate
limit before `challenge-refusal` invokes `turnstile-verify` in `guard`; do not
rely on `spend-limits`, which runs only after challenge verification. Preserve
the existing behavior that a failed challenge does not spend the route’s normal
allowances, and ensure the bound limits repeated verification attempts without
bypassing existing refusal checks.

Add a test for the Turnstile token on :re-verify.

The changed CLJS test only adds a namespace require. No repository test checks that :re-verify sends :turnstile-token in :query-params. Removing challenge-request from :re-verify would leave the suite passing, while resend requests would go out without the token. Add an event test that dispatches :re-verify with a token in db and asserts the :http :query-params include :turnstile-token.

Add a test for the `:re-verify` event that dispatches with a Turnstile token in
`db` and asserts the resulting `:http` `:query-params` contain
`:turnstile-token`. Exercise the event’s `challenge-request` behavior so the
test fails if that interceptor is removed from `:re-verify`.

421-434: Minor · Potential issue · Heavy lift

Unverified finding
We couldn't verify this from the code. It is shown for review, not as a verified issue.

The taken-address path hashes a password but does not perform the new account transaction; it instead looks up an owner and potentially enqueues a different notice. Its work may remain distinguishable by response time despite identical response maps.

Timing measurements may reveal which candidate addresses already have accounts.

This can occur when: An attacker repeatedly compares valid signups for candidate addresses with signups for known-fresh addresses.

than sign-up alone.

Review comments at @src/clj/orcpub/routes/auth.clj:
- Around line 558-577: Update do-verification so its write of
:orcpub.user/verified? false is conditional on the account still being

- Around line 408-416: Update notify-taken-owner! to build
signup-attempt-message links from the configured branding/app-url rather than
base-url request, and ensure no notice is enqueued when branding/app-url is
unset. Preserve the existing owner lookup and notice-claim safeguards.

Review comments at @src/clj/orcpub/security.clj:
- Around line 555-570: Update the failure path in notify-taken-owner! so a
failed outbox/enqueue! does not leave the notice recorded as sent; release the
claim or defer recording it until enqueue succeeds. Preserve the existing notice
throttling behavior for successful enqueues.
- Around line 143-147: The resend limit uses the process-local sightings atom,
allowing requests to bypass the limit across app instances. Update the
resend-counting flow associated with sightings to use shared storage, ensuring
counts are consistent across instances while preserving the existing limit and
window behavior.
- Around line 329-343: Update turnstile-verify and challenge-refusal to treat
non-string Turnstile tokens as blank before calling s/blank?. Preserve the
existing :fail response and refusal reason behavior so malformed token types are
rejected as challenge refusals rather than causing a server error.
- Around line 143-171: Update sight! and its sightings retention logic so
repeated failures with new keys cannot cause unbounded map growth; bound
retained keys using the existing login-challenge-cap pattern, or prune by each
limit’s own window rather than retaining every key for prune-horizon. Preserve
the atomic record-and-read behavior of sight!.
- Around line 500-507: Update busiest to count only sightings from the last hour
before taking the maximum for each limit. Derive the one-hour cutoff and exclude
older timestamps while preserving the existing limit grouping and maximum
calculation.
- Around line 312-325: Update post-siteverify and turnstile-verify so HTTP 4xx
responses return the Siteverify response body to the existing :fail path, rather
than being treated as verifier outages; reserve :unreachable for transport
errors and 5xx responses.
- Around line 357-375: Update `mark-login-challenge!` so reaching the cap never
clears unexpired challenge marks: prune expired entries first, then preserve all
remaining live marks. Define a bounded behavior for a new key when the map is
still at capacity, ensuring it does not evict live marks or allow unbounded
growth.

RESEND, and SUBMIT request flow to track whether a Turnstile-challenged request
is pending and skip dispatching another request while it is pending. Set the
flag when the challenged request starts, and clear it through the existing
request completion or widget callback flow so later requests can proceed.
- Around line 30-33: Update the challenge-reset interceptor’s request flow to
retain the Turnstile token sent by that request, then emit :turnstile-reset only
if the currently stored token still matches it; otherwise leave the newer
challenge untouched.

owner-notice test with a bounded wait for the queued message, using the existing

Ignore submits while a Turnstile request is pending.

JOIN, RESEND, and SUBMIT dispatch a request on every click, and none of them disables while a request is in flight. The widget token stays in :turnstile-token until a response or widget callback clears it. A second click before that point sends the same single-use token. Cloudflare rejects the second copy, and its failure handler stores a refusal message after the first request has succeeded. Set a pending flag when the challenged request starts, and skip the dispatch while the flag is set.

Update the :challenge-request interceptor and the JOIN, RESEND, and SUBMIT
request flow to track whether a Turnstile-challenged request is pending and skip
dispatching another request while it is pending. Set the flag when the
challenged request starts, and clear it through the existing request completion
or widget callback flow so later requests can proceed.

Not changed. A second submit with the same single-use token is refused by Cloudflare (timeout-or-duplicate), shown as "try the check again", and the widget resets; nothing is sent twice that matters. A pending flag would add state to every form for no protection gain. The suggested :re-verify token test is added in cbbe7a9 (a-resend-carries-the-turnstile-token).

@codeGlaze Understood. I won’t press for a pending flag. My concern was a late failure response leaving an error message after success, not duplicate account actions.

Thanks for adding a-resend-carries-the-turnstile-token in cbbe7a9f.

Scope the Turnstile reset to the request that failed.

challenge-reset sends a global :turnstile-reset on every protected failure. A late failure can clear the token and reset the widget of a different auth form that the user has since opened and solved. The user must then solve an unrelated challenge again. Record the token each request sent, and reset only when the stored token still matches it.

Update the challenge-reset interceptor’s request flow to retain the Turnstile
token sent by that request, then emit :turnstile-reset only if the currently
stored token still matches it; otherwise leave the newer challenge untouched.

Not changed. A reset after a failure at worst discards a fresh token and the widget issues another (managed mode, usually invisible); it never lets a request through. Same reasoning as the pending-flag thread.

@codeGlaze Understood. The concern is an unnecessary challenge reset, not a security bypass. I accept your decision to keep the current behavior and will not press for this change.

Sightings, sign-in challenge marks and notice claims go through one small protocol.
Memory stays the default and behaves as before. With REDIS_URL set, a Redis store
(carmine 3.4.1, loaded only then) shares them between app containers; any call that
fails or times out is answered from memory, warned about at most once a minute, so
limits never switch off.

re-verify answers a verified account with the same bare 200 as an unknown
address. The per-host allowance test now reads the key guard writes. The
Turnstile notes name every form it covers, an absolute APP_OG_IMAGE is kept
as is, the character page's X-Frame-Options lands in :headers, and the
widget reset clears the blocked flag only when a widget exists.
Every current browser ignores ALLOW-FROM, and the CSP's frame-ancestors 'self' already decides
framing on every page, so the header did nothing. Whether World Anvil should embed sheets again
is tracked in docs/TODO.md on agents/develop.
