refactor: rename bmad-preview-ticketing to bmad-ticket and drop the extras bundle

Companion to bmad-code-org/BMAD-METHOD#2979.

• bmad-ticket replaces bmad-preview-ticketing and is now in both the planning and build bundles, on by default.
• bmad-spec and bmad-walkthrough are also in both bundles. The extras bundle is removed.
• The build bundle no longer lists bmad-create-epics-and-stories and bmad-sprint-planning, which bmad-bmm removed.
• Tests match the method record's skill list; the non-default bundle test builds its own bundle. README updated.

Merge together with bmad-code-org/BMAD-METHOD#2979.
